U.S. Food and Drug Administration approval summary: omacetaxine mepesuccinate as treatment for chronic myeloid leukemia.

On October 26, 2012, the U.S. Food and Drug Administration (FDA) granted accelerated approval to omacetaxine mepesuccinate (Synribo; Teva Pharmaceuticals USA, Inc., North Wales, PA, http://www.tevausa.com) for the treatment of adult patients with chronic phase (CP) or accelerated phase (AP) chronic myeloid leukemia (CML) with resistance and/or intolerance to two or more tyrosine kinase inhibitors (TKIs). The approval was based on the FDA review of data from 111 patients with CML in CP or in AP who had received two or more prior TKIs, including imatinib. Major cytogenetic response was achieved

Minimal Residual Disease as an Early Endpoint for Accelerated Drug Approval in Myeloma: A Roadmap.

The acceptance of MRD-negative complete response as an endpoint that is reasonably likely to predict clinical benefit will allow for the design of streamlined clinical trials for accelerated approval, enabling significantly faster patient access to novel therapies. Cooperative efforts were required to obtain and analyze clinical trial data from multiple sponsors and to determine the best approach to analysis with a relatively limited number of available datasets. The process to evaluate MRD as an intermediate endpoint, undertaken jointly by myeloma researchers and industry, with feedback from

Real-World Experience with 177Lu-PSMA-617 Radioligand Therapy After Food and Drug Administration Approval.

We report our initial real-world experience with 177Lu-PSMA-617 radioligand therapy. Methods: We performed a retrospective review of patients treated with 177Lu-PSMA-617. Pretreatment PSMA PET, laboratory findings, overall survival, a fall in prostate-specific antigen by 50% (PSA50), and toxicities were evaluated. Results: Ninety-nine patients were included. Sixty patients achieved a PSA50. Seven of 18 (39%) patients who did not meet the TheraP PSMA imaging criteria achieved a PSA50. Nineteen of 31 (61%) patients who did not meet the VISION laboratory criteria achieved a PSA50. Sixty-three pat

Differences in drug approval processes of 3 regulatory agencies: a case study of gemtuzumab ozogamicin.

Major discrepancies concerning risk-benefit assessments and regulatory actions are frequent among regulatory agencies. We explored the differences by scrutinizing a case of gemtuzumab ozogamicin (GO) in patients with acute myeloid leukaemia (AML). Assessment reports of GO were retrieved form the websites of the US Food and Drug Administration (FDA), the European Medicines Agency (EMA) and Japanese regulatory agency, and we also reviewed published clinical trials. While GO was approved by the US FDA under the accelerated approval program in 2000, it was withdrawn from the market in 2010, based

Industry promotion of oncology drugs with accelerated approval that failed confirmatory trials.

Industry payments to physician's influence prescribing, raising concern for drugs granted accelerated approval that failed confirmatory studies. We measured industry payments for oncology drugs granted accelerated approval before and after negative confirmatory study results. From 2009 to 2021, of 73 drugs granted accelerated approval, 6 (8.2%) had negative confirmatory studies and available OpenPayments data. These were withdrawn a median of 0.3 (interquartile range = 0.3-0.6) years after announcement of negative results. Two drugs had almost no payments after negative result announcement. Am

From design to clinic: Medicinal chemistry and pharmacology of approved epigenetic drugs.

Many diseases, including cancer, are characterized by increased or decreased expression of specific genes. These changes can occur without genome alteration, primarily modulated by the addition or removal of epigenetic markers, which influence chromatin condensation and architecture. Genes in more condensed chromatin regions have lower expression and the opposite also applies. In the last twenty-two years, small molecule inhibitors of enzymes responsible for chromatin deacetylation or methylation have successfully moved from preclinical discovery to clinical therapy. This review explores the t

A systematic review of regulatory approved drugs and pharmacological interventions for erectile dysfunction.

OBJECTIVES: Erectile dysfunction (ED), or impotence, means having trouble getting or keeping an erection. The National Institutes of Health (NIH) says ED can happen to men at any age, but it's more common as men get older. This review looks at PDE5 inhibitor drugs approved by health authorities to treat ED. We carefully studied available research and clinical data to understand these drugs, how they work, how well they help, their safety, and their role in managing ED. METHODS: A literature-based study was conducted using PubMed, ScienceDirect, and regulatory documents (FDA, EMA) from 2015-20

Changes in evidence used for FDA Novel Drug Approvals: 2016-2024.

BACKGROUND: The 21st Century Cures Act (2017) was enacted during a period of evolving FDA flexibility in evidentiary standards for drug approval. To examine trends before and after implementation, we independently reviewed all novel drugs approved between 2016 and 2024. METHODS: We constructed a database of all novel FDA approvals from January 1, 2016, through December 31, 2024. Each study linked to an approved drug (N = 6,763) was cataloged by study number, sponsorship, and timing of results reporting relative to completion. RESULTS: Since 2016, the number of studies supporting approval has…

Novel drugs approved by the EMA, the FDA and the MHRA in 2025: A year in review.

In the 2025 novel drug mini-review, one can take a full measure of the ingenuity that underlies current drug design and development, despite the year's smaller harvest (46 novel drugs) compared to 2024 (53) and 2023 (70). 54% of the novel drugs are first-in-class (FIC). The emphasis on proteins/antibodies is maintained (~25% novel drugs in 2025), an industry trend that does not seem to abate. Fewer than half of the novel medicines address major or common disorders. Among the FIC drugs, it is worth mentioning the Nav1.8 channel inhibitor suzetrigine, the first non-opioid approved to palliate ac

The Substitute Groups in FDA-Approved Small Molecule Drugs in 2020 - 2024.

The strategic application of substituents is a cornerstone of modern drug discovery, critical for fine-tuning efficacy, safety, and pharmacokinetics. This review systematically catalogs the landscape of functional groups in FDA-approved small molecules (2020-2024), deciphering the rationale behind their use. Through case studies, it translates observed trends into practical design principles for medicinal chemists, highlighting how specific substituents address key challenges in developing novel therapeutics.

Performance of Control Arms in Trials Leading to FDA Cancer Drug Approvals.

BACKGROUND: Selecting an appropriate control arm is essential when evaluating the efficacy of new anticancer therapies. METHODS: We analyzed the progression-free survival (PFS) performance of control arms in trials leading to FDA drug approvals between January 2014 and December 2023 for the treatment of advanced solid cancers. For each phase III or double-arm phase II trial, we extracted (1) the assumed median PFS (amPFS) value for the control arm, as specified in the study protocol for sample-size calculation, and (2) the observed median PFS (omPFS) and its confidence interval (CI) for that …
