Impact of Specific Alerts in Potassium-Increasing Drug-Drug Interactions.

Alerts in potassium(K+)-increasing drug-drug interactions (DDIs) are often ignored due to their low specificity. Although different approaches have been implemented to address DDIs, subsequent clinical studies revealed poor adherence to such alerts. We therefore suggest a novel alert concept currently being evaluated in a randomized clinical trial in a large teaching hospital. Highly specific reminders (to monitor K+) and alerts (of hyperkalaemia) are displayed to the physicians of the intervention group, whereas reminders and alerts are suppressed in the control group. Preliminary analysis sh…

Potential drug-drug interactions in Alzheimer patients with behavioral symptoms.

The use of multi drug regimens among the elderly population has increased tremendously over the last decade although the benefits of medicat … s often results in addition of yet another drug to treat the symptoms, which in turn increases drug-drug interactions. Cognitive enhancers, including acetylcholinesterase inhibitors and memantine, are the most widely prescribed agents for Alzheimer's disease (AD) patients. Behavioral and psychological symptoms of dementia, including psychotic symptoms and behaviora …

Bedaquiline: a review of human pharmacokinetics and drug-drug interactions.

Bedaquiline has recently been approved for the treatment of pulmonary multidrug-resistant tuberculosis (TB) as part of combination therapy in adults. It is metabolized primarily by the cytochrome P450 isoenzyme 3A4 (CYP3A4) to a less-active N-monodesmethyl metabolite. Phase I and Phase II studies in healthy subjects and patients with drug-susceptible or multidrug-resistant TB have assessed the pharmacokinetics and drug-drug interaction profile of bedaquiline. Potential interactions have been assessed between bedaquiline and first- and second-line anti-TB drugs (rifampicin, rifapentine, isoniaz

The importance of drug-drug interactions in the DAA era.

With the licensing of the direct acting antivirals telaprevir and boceprevir the topic of drug-drug interactions has come to the forefront. These first generation hepatitis C virus protease inhibitors are metabolized by and inhibit the key drug metabolizing enzyme CYP3A4, which means that knowledge of drug-drug interactions has become an essential component of the evaluation of a patient starting triple therapy. The number of potential co-medications means that many drugs will be used in hepatitis C virus patients where there are no pharmacokinetic study data. Here we have to use the data that…

Potentially harmful drug-drug interactions in the elderly: a review.

BACKGROUND: Elderly patients are vulnerable to drug interactions because of age-related physiologic changes, an increased risk for disease associated with aging, and the consequent increase in medication use. OBJECTIVE: The purpose of this narrative review was to describe findings from rigorously designed observational cohort and case-control studies that have assessed specific drug interactions in elderly patients. METHODS: The PubMed and International Pharmaceutical Abstracts databases were searched for studies published in English over the past 10 years (December 2000-December 2010) using…

Pharmacogenetics and drug interactions: role in antiepileptic-drug-induced teratogenesis.

Metabolism of antiepileptic drugs (AEDs) is a potentially major factor in AED-induced teratogenesis. When the parent compound is the teratog … bolite. Genetic defects in detoxification pathways and the inhibition of these pathways by specific drug interactions probably have a greater impact on teratogenic risk than does high activity of metabolic activation. Insight into these factors involving AED metabolism might provide a rational basis for prevention by adjustment of medication, and in the future, for …

Invited commentary: building rigorous clinical evidence for drug-drug interactions.

Drug-drug interactions (DDIs) are a major cause of preventable adverse drug events. However, the clinical relevance of specific DDIs is often uncertain due to limited evidence beyond preclinical findings and pharmacokinetic studies. In this editorial, we discuss the recent study by Bea et al. (Am J Epidemiol. 2025;000(00):0000-0000), which assessed whether the pharmacologic interaction between hydrocodone and non-dihydropyridine calcium-channel blockers is associated with the risk of opioid overdose, finding no increased risk. We highlight the methodological strengths of the study, including t…

Patients with dementia: prevalence and type of drug-drug interactions.

BACKGROUND: Patients with Alzheimer's disease (AD) and other dementias are more frequently exposed to polymedication, mainly due to the presence of comorbidities, are particularly vulnerable to drug-related problems, and present greater risk of adverse effects due to drug-drug interactions (DDIs). PURPOSE: To assess the prevalence of clinically relevant interactions in dementia patients using a routine database, we describe the most frequent interactions and risk factors associated with them to facilitate specific interventions and programs to prevent and minimize them. METHODS: An observati…

Effects of Tenapanor on Cytochrome P450-Mediated Drug-Drug Interactions.

Tenapanor (RDX5791, AZD1722) is an inhibitor of sodium/hydrogen exchanger isoform 3 in development for the treatment of constipation-predominant irritable bowel syndrome and the treatment of hyperphosphatemia in patients with chronic kidney disease on dialysis. We aimed to investigate whether tenapanor inhibits or induces cytochrome P450s (CYPs). In vitro experiments assessing the potential of tenapanor to affect various CYPs indicated that it could inhibit CYP3A4/5 (IC50 0.4-0.7 μM). An open-label, phase 1 clinical study (NCT02140268) evaluated the pharmacokinetics of the CYP3A4 substrate mid

A drug-drug conditioning paradigm reveals multiple antipsychotic-nicotine interactions.

Clinical studies indicate a reciprocal impact between nicotine use and antipsychotic medications in patients with schizophrenia. The present study used a conditioned avoidance response (CAR) test (a behavioral test of antipsychotic effect) and examined the specific drug-drug interactions between nicotine and haloperidol or clozapine. Following acquisition of the avoidance response, rats were first tested under either vehicle, nicotine (0.2, 0.4 mg/kg, sc), haloperidol (0.025, 0.05 mg/kg, sc), clozapine (5.0, 10.0 mg/kg, sc), or a combination of nicotine and haloperidol or nicotine and clozapin…
