Analysis and implications of validation results for a clinical data-based early warning model for geriatric sepsis.

OBJECTIVE: Sepsis poses a serious threat to the health and lives of older adults and is difficult to identify accurately in its early stages. To address this issue, our team previously developed an early warning model for sepsis in older adults based on machine learning algorithms and conducted preliminary validation. To further validate the model's performance for clinical use, we collected additional clinical data. In the process, we gained some insights. METHODS: We collected clinical data from patients aged 60 years or older admitted to the Emergency Department and Intensive Care Unit of

NEWS2 to assess suspected COVID-19 in the community: a service evaluation of a primary care assessment centre.

BACKGROUND: Primary care has played a central role in the community response to the coronavirus disease-19 (COVID-19) pandemic. The use of the National Early Warning Score 2 (NEWS2) has been advocated as a tool to guide escalation decisions in the community. The performance of this tool applied in this context is unclear. AIM: To evaluate the process of escalation of care to the hospital within a primary care assessment centre (PCAC) designed to assess patients with suspected COVID-19 in the community. DESIGN AND SETTING: A retrospective service evaluation of all adult patients assessed betw…

Real-world data-driven early warning system for risk-stratified liver injury in hospitalized COVID-19 patients-Machine learning models for clinical decision support.

OBJECTIVE: To develop and validate a real-world evidence-driven early warning system for the risk-stratified prediction of coronavirus disease 2019 (COVID-19)-associated hepatic dysfunction in hospitalized patients, leveraging interpretable machine learning models to provide clinically actionable decision support for timely intervention. METHODS: A retrospective single-center cohort study was conducted utilizing high-resolution electronic health records (EHRs) from 983 hospitalized COVID-19 patients. Clinical features (e.g., laboratory results, medication exposures, and disease progression ma

Explainable artificial intelligence model to predict acute critical illness from electronic health records.

Acute critical illness is often preceded by deterioration of routinely measured clinical parameters, e.g., blood pressure and heart rate. Early clinical prediction is typically based on manually calculated screening metrics that simply weigh these parameters, such as early warning scores (EWS). The predictive performance of EWSs yields a tradeoff between sensitivity and specificity that can lead to negative outcomes for the patient. Previous work on electronic health records (EHR) trained artificial intelligence (AI) systems offers promising results with high levels of predictive performance i…

Caregivers' Recognition of Pediatric Asthma Triggers and Exacerbation Warning Signs: A Cross-Sectional Study.

Background Pediatric asthma exacerbations are frequently triggered by environmental exposures and are often preceded by identifiable warning signs. Caregivers play a central role in recognizing these triggers and early symptoms and in initiating appropriate management to prevent severe outcomes. Objective This study aimed to assess caregivers' recognition of pediatric asthma triggers and exacerbation warning signs, to describe caregiver response behaviors, and to identify factors associated with caregivers' responses. Methods A cross-sectional study was conducted between October and December 2

Awareness level of cancer risk factors and warning signs among adults in Mansoura District, Egypt.

BACKGROUND: Cancer remains a leading cause of mortality worldwide, with prevention and early detection dependent on the public awareness of its risk factors and warning signs. However, knowledge of cancer-related symptoms and modifiable risk factors remains limited in many populations, particularly in low- and middle-income countries. Limited research exists examining general cancer awareness in Egypt's general population. OBJECTIVE: To assess public knowledge of cancer's early warning signs, risk factors, and prevention strategies in Mansoura, Egypt. METHODS: A descriptive cross-sectional s

[Artificial intelligence-driven early warning for severe pneumonia: pathways and challenges from data integration to clinical decision-making].

Severe pneumonia, characterized by rapid progression and high heterogeneity, is a leading cause of patient mortality. Early identification of high-risk patients is crucial for improving outcomes. Traditional risk stratification tools, such as CURB-65 score and pneumonia severity index (PSI), primarily rely on static variables, which inadequately capture the dynamic evolution of the disease and offer limited capabilities for individualization. In recent years, artificial intelligence (AI) technologies, particularly machine learning, deep learning, and multimodal fusion models, have demonstrated

Development and evaluation of a multi-source machine learning model to predict critical illness and death in patients with substance misuse.

OBJECTIVES: Patients with substance misuse are at high risk for clinical deterioration, and pre-hospital encounters constitute important risk factors. We sought to incorporate these risk factors into a novel prediction model using linked electronic health record, emergency medical services (EMSs), and claims data. MATERIALS AND METHODS: Using 23 454 hospital encounters, we developed machine-learning models to predict mechanical ventilation, vasopressor initiation, or death within 12 hours of a vital sign or laboratory measurement. Models were compared to the modified early warning score. RES…

Diagnostic challenge: an atypical presentation of infective endocarditis.

It is recognised that infective endocarditis is frequently a challenging diagnosis to make, as it may present with a range of non-specific symptoms. A middle-aged man was admitted with an 8-day history of profuse non-bloody diarrhoea and vomiting. He had no medical history and no identifiable risk factors for infective endocarditis, and so this in combination with the patient's atypical symptoms presented a diagnostic challenge. The patient was eventually diagnosed with a Staphylococcus aureus right-sided infective endocarditis. This case report explores the events which led to this diagnosis

Obstetric early warning system to predict maternal morbidity of pre-eclampsia, postpartum hemorrhage and infection after birth in high-risk women: a prospective cohort study.

OBJECTIVE: The purpose of early warning systems is to detect deterioration of the patient and to enable timely intervention to prevent possible severe illness. The most common causes of maternal morbidity and mortality after birth are worsening pre-eclampsia, postpartum haemorrhage and puerperal infection. Our aim was to validate the accuracy of the obstetric early warning system and different physiological triggers to predict morbidity on the postnatal ward in high-risk women. DESIGN: A prospective cohort study. SETTING: A tertiary referral hospital in Finland. PARTICIPANTS: High-risk wome

A host transcriptomic signature for identification of respiratory viral infections in the community.

BACKGROUND: Fever-7 is a test evaluating host mRNA expression levels of IFI27, JUP, LAX, HK3, TNIP1, GPAA1 and CTSB in blood able to detect viral infections. This test has been validated mostly in hospital settings. Here we have evaluated Fever-7 to identify the presence of respiratory viral infections in a Community Health Center. METHODS: A prospective study was conducted in the "Servicio de Urgencias de Atención Primaria" in Salamanca, Spain. Patients with clinical signs of respiratory infection and at least one point in the National Early Warning Score were recruited. Fever-7 mRNAs were p…
