Neonatal infection with herpes simplex virus (HSV) is associated with significant morbidity, high mortality, and long-term neurological sequelae. We report the clinical case of an infant with HSV encephalitis, where the initial diagnosis was established based on cranial ultrasound (CUS) findings. These findings revealed localized, asymmetrically distributed hyperechoic areas in the parenchyma and signs of brain swelling. CUS dynamics on days 7 and 14 after the onset of clinical symptoms demonstrated multiple subcortical and perivascular zones of encephalomalacia in the right hemisphere, accomp

OBJECTIVES: The purpose of this study was to evaluate neuropsychological test data in school-aged children whose fetal sonograms revealed mild isolated cerebral ventriculomegaly without asymmetry of the lateral ventricles.
METHODS: Nine of 52 children 6 years and older with sonographic evidence of mild isolated cerebral ventriculomegaly without asymmetry of the lateral ventricles were able to be recruited for follow-up school-aged neuropsychological testing. The children received a half-day battery of neuropsychological tests, including the Wechsler Abbreviated Scales of Intelligence; Beery-B

The study was designed to determine the effects of two protocols of sedation, medetomidine and medetomidine-butorphanol, on cerebral blood flow (CBF) by transcranial color-coded Duplex ultrasonography in healthy dogs. Transcranial Doppler ultrasonographic examination was performed in 20 dogs before and 20 min after sedation with either medetomidine (group 1) or medetomidine-butorphanol (group 2). The left and right middle cerebral arteries (LMCA and RMCA) were evaluated using the temporal windows, and the basilar artery (BA) was studied through the suboccipital window. Peak systolic velocity (

The authors present a 56-year-old-man with idiopathic glossopharyngeal neuralgia, complicated by cardiac disturbance with asystole and complex partial seizures. The mechanism of epileptic seizures was released due to transient brain ischaemia, which was caused by cardiac disturbance with asystole, during pain episodes. Pacemarker implantation completely abolished cardiac disturbance and complex partial seizures.

Functional tissue pulsatility imaging is a new ultrasonic technique being developed to map brain function by measuring changes in tissue pulsatility as a result of changes in blood flow with neuronal activation. The technique is based in principle on plethysmography, an older, nonultrasound technology for measuring expansion of a whole limb or body part as a result of perfusion. Perfused tissue expands by a fraction of a percent early in each cardiac cycle when arterial inflow exceeds venous outflow, and it relaxes later in the cardiac cycle when venous drainage dominates. Tissue pulsatility i

To date the literature comparing the usefulness of US and MR examinations of the neonatal brain suggests that US is not as effective a modality as MR. However, available studies were done on older equipment and published descriptions of the abnormalities found in the term brain are often incomplete. The purpose of this article is to emphasize technical factors that may be useful to optimize US imaging of the term neonatal brain, to provide a description of the sonographic findings in the brain in full-term neonates with hypoxic-ischaemic injury and to provide some data regarding the accuracy o

Schizencephaly is a rare malformation of the central nervous system. Prenatal ultrasound (US) allows diagnosis of schizencephaly, although prenatal magnetic resonance (MR) imaging is even more specific in detection of gray matter lining the defect, communication with the ventricle, and other associated structural abnormalities. Six cases of schizencephaly were evaluated at one institution; prenatal US was performed in all cases, and fetal MR imaging was performed in three cases. As with many malformations, more severe cases of schizencephaly often manifest in utero. All of the cases studied we

The interpretation of ultrasound images remains a difficult task and the opinion of different doctors is generally not unequivocal. Therefore, there is a growing interest in the field of computer-aided diagnosis. In the field of medical image processing, computer-aided diagnosis includes image enhancement to facilitate visual interpretation, automatic indication of affected areas, organs and other regions of medical interest, the performance of automatic measurements and image registration. In this article, we introduce a new algorithm for ultrasound image enhancement that employs a multivaria

Tuberous sclerosis complex (TSC) is more prevalent than previously believed and most cases result from new mutations and, hence, do not have affected relatives. Many patients who meet the new diagnostic criteria have unrecognized skin lesions and/or occult lesions in internal organs. As early diagnosis and appropriate intervention can reduce morbidity and mortality from the neurologic. renal, and pulmonary complications of TSC, physicians should be alert for the dermatological signs of the condition and for the incidental finding of TSC lesions during diagnostic testing. Patients with TSC may

Dissection of the internal carotid artery is an important cause of ischemic stroke in children and young patients. Trauma and/or an underlying structural defect of the arterial wall have been suggested to be predisposing factors. The typical patient presents with ipsilateral headache or neck pain, ipsilateral Horner's syndrome and delayed ischemic symptoms. Diagnosis is given by ultrasound, transcranial Doppler, magnetic resonance imaging, magnetic resonance angiography and conventional angiography. Treatment of this type of injury includes anticoagulation therapy, antiplatelet therapy and sur

This study examined the dynamics of the middle cerebral artery (MCA) blood flow response to hypocapnia in humans (n = 6) by using transcranial Doppler ultrasound. In a control protocol, end-tidal PCO2 (PETCO2) was held near eucapnia (1.5 Torr above resting) for 40 min. In a hypocapnic protocol, PETCO2 was held near eucapnia for 10 min, then at 15 Torr below eucapnia for 20 min, and then near eucapnia for 10 min. During both protocols, subjects hyperventilated throughout and PETCO2 and end-tidal PO2 were controlled by using the dynamic end-tidal forcing technique. Beat-by-beat values were calcu

We investigated the developmental outcome and features of brain imaging in 33 infants with moderate or severe depression of background EEG activities immediately after birth. Lesions due to perinatal injury were observed in 25 infants on neonatal ultrasonography and/or childhood MRI. These findings strongly correlated with their gestational age. Periventricular leukomalacia and posthermorrhagic porencephaly were observed in preterm infants. On the other hand, perinatal injury of mature brain type, such as multicystic encephalomalacia, parasagittal infarct, bilateral basal ganglia/thalamic lesi

To evaluate haemodynamic effects of carotid endarterectomy on cerebral circulation, transcranial Doppler sonography was performed on 69 patients operated for unilateral carotid stenosis greater than 70%. Pre- and post-operative Vm and PI on bilateral middle-cerebral arteries, Vm PI and flow direction on ipsilateral anterior cerebral artery and Vm on basilar artery were evaluated. All patients presented more or less evident preoperative haemodynamic alterations of cerebral circulation and after surgery showed a normal distribution of cerebral blood supply.

47 infants with myelomeningocele were investigated by ultrasound after the surgical closure of the cele and followed up to 12 months. The aim was the early diagnosis of hydrocephalus and the recording of morphological features of the ventricular system in comparison with hydrocephali of other origins, especially of Chiari-Syndrome. 31 infants needed a shunt due to rapid progress of the ventricular dilatation and clinical signs of a raised intracranial pressure: in 25 of this group (81%) the ultrasound investigation demonstrated a Chiari-Syndrome. Of 16 further infants who definitely did not ne
