Elbow arthrodesis is an uncommon salvage procedure indicated for failed total elbow arthroplasty, segmental bone loss near the elbow, and severe postinfectious or posttraumatic arthritis. This video demonstrates the surgical technique for elbow arthrodesis in a patient with segmental bony defect of the ulna and postinfectious and posttraumatic elbow arthritis after a severe left arm degloving injury with Monteggia fracture dislocation and radial head fracture.

Question Our practice is seeing children with relatively minor injuries to their elbows, with a history of "swinging" them when their hands are being held to cross the road. Nothing is usually found on a physical examination. I know that this is likely a "pulled elbow." Can we manage this in the clinic setting rather than sending the family to the emergency department? What would be the best course of action in the clinic setting?Answer Pulled elbow, also called nursemaid's elbow, is a radial head subluxation caused by axial traction or a sudden pull of the extended pronated arm, and it is a v

Post-traumatic and post-operative stiffness of the elbow joint is relatively common and may in pronounced cases markedly interfere with normal upper extremity function.Soft-tissue contractures and heterotopic bone formation are two major causes of limited movement.Extensive recent research has elucidated many of the pathways contributing to these conditions, but the exact mechanisms are still unknown. In the early phase of soft-tissue contractures conservative treatment may be valuable, but in longstanding cases operative treatment is often necessary.Several different options are available depe

BACKGROUND: Paediatric elbow injuries account for a large proportion of childrens' fractures. Knowledge of common injuries is essential to understanding their assessment and correct management.
METHODS: A selective literature search was performed and personal surgical experiences are reported.
RESULTS: We have described the assessment and management of the five most common paediatric elbow injuries: supracondylar humeral fractures; lateral condyle fractures; medial epicondyle fractures; radial head and neck fractures; radial head subluxation.
CONCLUSION: Understanding of the ossification ce

Active elbow flexion is required to position the hand in space, and loss of this function is debilitating. Nerve transfers or nerve grafts to restore elbow flexion may be options when the target muscle is viable, but in delayed reconstruction when the biceps and brachialis are atrophied or damaged, muscle transfer options should be considered. Muscle transfer options are discussed with attention to the advantages and disadvantages of each transfer option.

The elbow is a highly congruent trochoginglymoid joint allowing motion in both flexion-extension and pronosupination across 3 articulations. Therefore, treatment of fractures of the elbow can be technically challenging to manage, even after initial surgery. The posttraumatic elbow is prone to complications such as stiffness associated with heterotopic ossification, instability or subluxation (posterolateral rotatory instability and varus posteromedial instability patterns), and wound complications. This article discusses the pathoanatomy, prevention, and treatment of these complications.

Lateral epicondylitis, originally referred to as tennis elbow, affects between 1% and 3% of the population and is usually found in patients aged 35 to 50 years. Although it was initially thought that lateral epicondylitis was caused by an inflammatory process, most microscopic studies of excised tissue demonstrate a failure of reparative response in the extensor carpi radialis brevis tendon and in any of the associated structures. Most cases of lateral epicondylitis respond to appropriate nonsurgical treatment protocols, which include medication, bracing, physical therapy, corticosteroid injec

Elbow dislocations are a high-energy traumatic event resulting in loss of congruence of a stable joint. The majority of elbow dislocations can be reduced by closed means and treated conservatively. We present a case of an irreducible elbow dislocation with reduction blocked by the radial head buttonholed through the lateral ligamentous complex. We performed open reduction with release followed by repair of the lateral ligamentous complex. Clinicians need to understand this unique variant of an elbow dislocation to appropriately treat this operative injury.

OBJECTIVE: The purpose of this article is to review the embryologic development, anatomy, and histology of the synovial plicae of the elbow. The pathophysiologic features, clinical manifestations, imaging findings, and treatment of elbow synovial fold syndrome will also be reviewed.
CONCLUSION: Elbow synovial fold syndrome is an uncommon entity that is often confused with lateral epicondylitis. Knowledge of the clinical and imaging diagnosis of this entity is essential for the appropriate management of patients.

Elbow arthroscopy has become an accepted surgical option for treating numerous conditions of the elbow, including septic, degenerative, or traumatic arthritis; capsular release; removal of loose bodies; synovectomy or plica excision; and chondral lesions of the capitellum. Surgeon experience, knowledge of elbow anatomy, patient positioning, and portal selection and placement are important factors for successful arthroscopy and avoiding complications. This article describes the basic surgical setup and technique for elbow arthroscopy.

Fractures of the elbow are a very common injury in children. The most common mechanism of injury is a fall on an outstretched upper extremity during play. Ranging in complexity from low-energy nondisplaced occult fractures to high-energy fractures with associated severe soft-tissue and neurovascular injuries, elbow fractures are a challenging problem for all pediatric healthcare providers. Because of the wide spectrum of fracture severity and associated bony and ligamentous injury, a very diverse spectrum of treatment modalities is necessary for optimal results. Management is based on fracture

The loss of elbow extension power is particularly disabling for the nonambulatory patient. Reconstruction of elbow extension can be performed by a deltoid to triceps transfer or by a biceps to triceps transfer provides the most satisfying reconstruction for patients. Although the overall time for rehabilitation can be lengthy, the functional gain is substantial, predictable, and easily appreciated by the patient. Furthermore, the risks to residual preoperative function are practically nil.

This article focuses on various methods to treat the chronically unreduced elbow. There are only a few small series published in the literature on which to base treatment options. Anatomic features pertinent to the discussion of the chronically unstable elbow are highlighted. The spectrum of treatment options includes open reduction, internal fixation and ligament reconstruction, external fixation, and elbow arthroplasty. The indications, operative technique, and results of each treatment modality are explored.

Nonunion of the lateral humeral condyle, cubitus varus, cubitus valgus, and fishtail deformity represent particularly challenging problems to the upper extremity surgeon. Although closed or open reduction and pinning of supracondylar fractures of the distal humerus can restore anatomic alignment and avoid anatomic deformities in most cases, closed reduction is still a common form of treatment. In those hopefully few cases in which reduction is less than optimal, or when a good reduction is performed but subsequently lost between follow-up visits, the aforementioned deformities of the distal hum

Extra-articular elbow arthroscopy has to be considered as the evolution of the elbow surgery to a mini invasive endoscopically assisted surgery developed by the recent advance of the elbow arthroscopy. Various pathologies, such as lateral epicondylitis, ulnar nerve entrapment, distal Biceps tendon rupture, synovial cysts, or olecranon bursitis have been treated arthroscopically. Extra-articular pathologies can be treated through an intra-articular endoscopic approach. The true endoscopic extra-articular technique is proced through a real anatomical space or inside a space of work created de no

The development of joint contracture is a well-recognized complication of elbow injury. Precise causes of the propensity of this joint for ankylosis are understood poorly. Yet, treatment is emerging and therefore the indications and willingness on the part of the surgeon to address this problem is improving. Limited open procedures have emerged during the past several years that are safe and effective by improving arcs of motion of 50-70 degrees in approximately 80-90% of patients. For severe injuries that involve the articular surface, interposition arthroplasty is less documented but has bee
