Nonconvulsive status epilepticus associated with Alzheimer's disease mimicking symptomatic focal epilepsy following the resection of a frontal parasagittal meningioma.

BACKGROUND: Epilepsies are frequent in patients with Alzheimer's disease (AD); however, epilepsies in AD can easily go unrecognized because they usually present as focal impaired awareness seizures or nonconvulsive status epilepticus (NCSE) and can overlap with other symptoms of AD. CASE DESCRIPTION: We performed an epilepsy surgery in a 69-year-old woman with progressive cognitive impairment and consciousness disorder, who was diagnosed with focal NCSE related to the resected meningioma in the right frontal parasagittal region. Intraoperative electrocorticography revealed localized periodic …

Medial temporal lobe epilepsy associated with misplacement of a ventricular shunting catheter.

We report the case of a 35-year-old woman who developed right medial temporal lobe epilepsy associated with misplacement of a ventricular sh … mporal lobe, through the posterior and inferior horn of the right lateral ventricle. Intraoperative electrocorticography revealed frequent paroxysmal activity in the hippocampus, so hippocampectomy as well as removal of the shunting catheter was performed. Postoperatively, the patient became seizure-free, and pathological examination revealed hippocampal sclerosis.

Inhibiting effect of vagal nerve stimulation to seizures in epileptic process of rats.

OBJECTIVE: Our previous work suggested that sensitivity of hippocampal neurons is changed in process of epileptic activities, and closely parallel to the dynamic characteristic of epileptic activity of the neurons. This study investigated the sensitivity of epileptic brain to vagal nerve stimulation (VNS) in epileptic process. METHODS: Epileptic model was evoked by penicillin. Left vagal nerves were stimulated to inhibit the seizures induced by penicillin. The electrocorticography (ECoG) and electromyography (EMG) were recorded to analyze inhibiting effect of VNS in epileptic process. RESULT…

Neural Correlates of Music Listening and Recall in the Human Brain.

Previous neuroimaging studies have identified various brain regions that are activated by music listening or recall. However, little is known about how these brain regions represent the time course and temporal features of music during listening and recall. Here we analyzed neural activity in different brain regions associated with music listening and recall using electrocorticography recordings obtained from 10 epilepsy patients of both genders implanted with subdural electrodes. Electrocorticography signals were recorded while subjects were listening to familiar instrumental music or recalli…

Closed-Loop Decoding and Intervention of Pain: A Novel BMI Strategy Integrating θ-Band Detection and Mechano-Electro-Biological Coupled Hydrogels.

Neuropathic pain (NP) lacks objective biomarkers and effective therapies. We developed a closed-loop brain-machine interface (BMI) using a novel hydrogel electrode for integrated NP management. Clinically, we identified enhanced prefrontal θ-band (4-8 Hz) activity as a specific NP biomarker. To achieve stable long-term recording, we engineered a PFAPT hydrogel with superior biocompatibility, anti-swelling, and low impedance. In rats, it enabled stable electrocorticography (ECoG) acquisition for 28 d, outperforming screw electrodes, and validated θ-band alterations in NP. Incorporated into a cl…

Automated classification of seizure onset pattern using intracranial electroencephalogram signal of non-human primates.

Objective. To develop and validate a machine learning framework for the classification of distinct seizure onset patterns using intracranial EEG (iEEG) recordings in a non-human primate (NHP) model of penicillin-induced seizures.Approach.iEEG data were collected from six NHPs, comprising 1, 496 frontal and 549 temporal lobe seizures. Seizure onset patterns were manually categorized into five types: Sharp Activity (5-15 Hz), Low Amplitude Fast Activity (15-30 Hz), Delta Brush (1-3 Hz with bursts), High Amplitude Spike (2-5 Hz), and Polyspike. A Random Forest classifier was trained using features

Spreading depolarization: A phenomenon in the brain.

In 1944, the physiologist Leão while studying epilepsy in the rabbit noticed a sudden temporary cessation of electrocorticographic (ECoG) activity accompanied with a large negative slow potential change recorded by extracellular electrodes, that is later known as spreading depolarizations (SDs). The depression of the brain electrical activity was slowly propagating through the cerebral cortex. The mechanism of propagation is still controversial. SDs and seizures are following each other interchangeably, yet the puzzle needs more investigation to be clarified. SDs have an obvious effect on bloo

Bidirectional propagation of low frequency oscillations over the human hippocampal surface.

The hippocampus is diversely interconnected with other brain systems along its axis. Cycles of theta-frequency activity are believed to propagate from the septal to temporal pole, yet it is unclear how this one-way route supports the flexible cognitive capacities of this structure. We leveraged novel thin-film microgrid arrays conformed to the human hippocampal surface to track neural activity two-dimensionally in vivo. All oscillation frequencies identified between 1-15 Hz propagated across the tissue. Moreover, they dynamically shifted between two roughly opposite directions oblique to the l

Functional alterations in cortical processing of speech in glioma-infiltrated cortex.

Recent developments in the biology of malignant gliomas have demonstrated that glioma cells interact with neurons through both paracrine sig … temporal patterns of activity with subsequent impact on cognitive processing. Here, we use subdural electrocorticography to sample both normal-appearing and glioma-infiltrated cortex during speech. We find that glioma-infiltrated cortex engages in synchronous activity during task performance in a manner similar to normal-appearing cortex but recruits a diffuse spatial …

Resection of tuber centers only for seizure control in tuberous sclerosis complex.

Our previous studies suggest the tuber center is the seizure focus in tuberous sclerosis complex (TSC). We report findings from 5 epilepsy surgeries in 4 children with TSC and focal motor seizures from single tubers in primary sensorimotor cortex in which resection was limited to the cortex in the tuber center. Intraoperative electrocorticography showed epileptiform activity in the tuber center, with or without propagation to the tuber rim and surrounding perituberal cortex. Histopathology showed an abundance of dysmorphic neurons in the tuber center compared to the rim in four paired specimen…

Neural correlates of spatial and nonspatial attention determined using intracranial electroencephalographic signals in humans.

Few studies have directly compared the neural correlates of spatial attention (i.e., attention to a particular location) and nonspatial attention (i.e., attention to a feature in the visual scene) using well-controlled tasks. Here, we investigated the neural correlates of spatial and nonspatial attention in humans using intracranial electroencephalography. The topography and number of electrodes showing significant event-related desynchronization (ERD) or event-related synchronization (ERS) in different frequency bands were studied in 13 epileptic patients. Performance was not significantly di

Biological validation of mathematically derived hypothesis in neuroscience.

Because of the complexity of the central nervous system, it is often difficult to biologically demonstrate the mathematically derived hypoth … rted from the whole brain functional imaging with low temporal resolution, followed by multichannel electrocorticography recordings with higher temporal resolution, non-biased analysis of whose extensive datasets yielded the hypothesis about critical contribution of a particular neuronal pathway. The hypothesis was then validated by using the pathway-selective chemoge …

All spectral frequencies of neural activity reveal semantic representation in the human anterior ventral temporal cortex.

Intracranial electrophysiology offers a unique insight into the nature of information representation in the brain-it can be used to disentangle information encoded in gamma and high gamma frequencies from information encoded in lower frequencies. We used regularised logistic regression to decode animacy from time-frequency power and phase extracted from electrocorticography (ECoG) grid electrode data recorded on the surface of human ventral anterior temporal lobe (vATL). Power in gamma (30-60 Hz) and high gamma (60-200 Hz) produced reliable decoding, indicating that semantic information is, in…

The "Podcast" ECoG dataset for modeling neural activity during natural language comprehension.

Naturalistic electrocorticography (ECoG) data are a rare but essential resource for studying the brain's linguistic capabilities. ECoG offers a high temporal resolution suitable for investigating processes at multiple temporal timescales and frequency bands. It also provides broad spatial coverage, often along critical language areas. Here, we share a dataset of nine ECoG participants with 1,330 electrodes listening to a 30-minute audio podcast. The richness of this naturalistic stimulus can be used for various research endeavors, from auditory perception to semantic integration. In addition t…

Frontal lobe epilepsy with focal neuronal lipofuscinosis - Case report of a rare entity.

Focal neuronal lipofuscinosis is a unique neuronal pathology, characterised by accumulation of lipofuscin within dysmorphic neurons. We report a case of 12-year-old female with drug resistant epilepsy since one and a half years of age. MRI brain showed right frontal dysplasia, and PET showed right frontal hypometabolism. She underwent electrocorticography-guided resection of the lesion. Histopathology revealed cortical dyslamination with several hypertrophic dysmorphic neurons showing intracytoplasmic granular accumulation of lipofuscin which was positive for Periodic acid-Schiff, Luxol fast b…

Electroencephalographic signatures of pain and analgesia in rats.

Pain modulates rhythmic neuronal activity recorded by Electroencephalography (EEG) in humans. Our laboratory previously showed that rat models of acute and neuropathic pain manifest increased power in primary somatosensory cortex (S1) recorded by electrocorticography (ECoG). In this study, we hypothesized that pain increases EEG power and corticocortical coherence in different rat models of pain, whereas treatments with clinically effective analgesics reverse these changes. Our results show increased cortical power over S1 and prefrontal cortex (PFC) in awake, freely behaving rat models of acu…

Continuous Monitoring of Spreading Depolarization and Cerebrovascular Autoregulation after Aneurysmal Subarachnoid Hemorrhage.

Delayed cerebral ischemia (DCI) is a prominent complication after aneurysmal subarachnoid hemorrhage (aSAH). Although vasospasm of proximal cerebral arteries has been regarded as the main cause of DCI, vasospasm of distal arteries, microthrombosis, impaired autoregulation, cortical spreading depolarization (CSD), and spreading ischemia are thought to be involved in DCI after aSAH. Here, we describe a patient with aSAH in whom CSD and cerebrovascular autoregulation were evaluated using simultaneous electrocorticography and monitoring of the pressure reactivity index (PRx) after surgical clippin…
