Effective diagnosis of sleep disorders using EEG and EOG signals.

This work focuses on the diagnosis of various sleep disorders such as insomnia, narcolepsy, periodic leg movement, nocturnal frontal lobe epilepsy, bruxism, REM behavior disorder, and sleep-disordered breathing. We utilize SVM for classifying each of the sleep disorders from healthy controls. The proposed approach is evaluated on the publicly available CAP dataset comprising 108 overnight recordings from healthy controls and patients with sleep disorders. A single feature called gridded distribution entropy derived from Poincaré plots of EEG signal provides 100% accuracy in distinguishing heal

Sleep-wake state and memory function.

Facility of memory formation and retrieval is affected by sleep-wake state. This paper discusses memory function while individuals are awake, but sleepy, at the transitions in state (i.e., wake to sleep and sleep to wake), and in REM and NREM sleep. The basal state of arousal at the time of memory function seems to be critical and the sleepy state, state transitions, and NREM sleep seem to define a continuum of arousal with respect to memory function. While different memory systems have been described, the effect of sleep-wake state on memory systems appears to be non-specific. Finally there i

Scalp-recorded direct current potential shifts induced by hypocapnia and hypercapnia in humans.

Shifts in scalp-recorded direct current (DC) potential were studied in relation to the changes in end-tidal partial CO2 (PACO2) or O2 (PAO2) of the expired gas either during hyperventilation (HV), hypoventilation (HYPO) or inhalation of high CO2 content air during HV or HYPO in 10 healthy subjects. The DC potential was obtained through a chopper stabilized type of DC amplifier from Cz referred to linked earlobes. Each session was comprised of 3 min control, 3 min experimental and 5 min recovery periods. HV induced a negative shift of the DC potential of 765.5 +/- 203.0 microV (mean +/- SEM). I

[Unusual pathological ocular movement caused by dysfunction of the saccade generation mechanism].

We describe a patient with abnormal ocular movement, probably due to dysfunction of saccade generation mechanism in the lower pons involving the omnipause neuron and a feed-back circuit. A 26-year-old man had fever, mild headache and sore throat for a few days and also noted unstable gait and diplopia. These symptoms subsided, but he noticed oscillopsia about 2 months later. On admission in our hospital, the neurological examination revealed no abnormalities except for abnormal eye movements, which were induced by voluntary or involuntary ocular movement and lasted for a few minutes. The abnor

Convergence nystagmus associated with Arnold-Chiari malformation.

A case of convergence nystagmus associated with an Arnold-Chiari type I malformation is presented. The nystagmus appeared in the absence of fixation, was provoked during Valsalva's maneuver and neck flexion and extension, and attenuated on deep inspiration. Sagittal magnetic resonance images showed that the diameter of the cerebral aqueduct increased with the neck in full flexion and in full extension. Surgical foramen magnum decompression considerably reduced the nystagmus and abolished the postural variation of aqueduct diameter. It is postulated that this nystagmus was due to a combination

Contrapulsion of saccades and ipsilateral ataxia: a unilateral disorder of the rostral cerebellum.

Contralateral pulsion of saccades and ipsilateral limb ataxia were manifestations of unilateral damage to the rostral cerebellum studied in a patient with occlusion of one superior cerebellar artery. The saccadic disorder consisted of three elements: horizontal saccades away from the lesion during attempted vertical saccades, resulting in oblique trajectories; hypermetria of contralateral saccades; and hypometria of ipsilateral saccades. Magnetic search coil oculography showed that durations of the horizontal components of oblique contrapulsive saccades were lengthened toward the durations of

Congenital stationary night blindness with myopia: a clinico-pathologic study.

The left eye of a 77 year old male patient was enucleated because of absolute glaucoma. This eye showed subnormal electroretinogram (ERG) and oscillatory potentials preoperatively, but the other eye showed Schubert-Bornschein type ERG and monophasic dark adaptation curve. Light and electron microscopic studies of the left eye showed a normal arrangement of discs of rod outer segments, normal synaptic ends of the photoreceptors, and complete loss of ganglion cells. From the subnormal ERG in the left eye we assumed it was originally Schubert-Bornschein type ERG but inverted to subnormal type ERG

Internuclear ophthalmoplegia associated with ipsilateral downbeat nystagmus and contralateral incyclorotatory nystagmus.

A case of left internuclear ophthalmoplegia combined with downbeat nystagmus and right incyclorotatory nystagmus on left downward gaze, left exotropia and skew deviation is presented. All signs subsided completely within about 3 months after the onset of the disease. The etiology was possibly of vascular origin. The potential site of the lesion causing these oculomotor signs is discussed.

Natural sleep at home and psychosis in patients with Parkinson's disease: A post-hoc analysis of the ZEAL study.

OBJECTIVES: Psychosis remains problematic in patients with Parkinson's disease and sleep disorders are frequent non-motor symptoms of PD. Few research studies have correctly assessed objective sleep architecture to determine the association between psychosis and sleep disorders. A mobile two-channel electroencephalography (EEG)/electrooculography (EOG) recording system is a self-applicable and affordable method for objectively measuring sleep at home. We studied patients with PD who were enrolled in ZEAL study of the randomized placebo-control clinical trial investigating the efficacy of zonis…

[A Patient with Early-Stage Multiple System Atrophy Showing Augmented Nystagmus in Light].

The ability to fix the eyes on a target, visual fixation, is important for the maintenance of equilibrium. The visual suppression (VS) test is one method of measuring the function of visual fixation. The test records caloric nystagmus by electrooculography, and the maximum slow phase velocity of caloric nystagmus in darkness is compared with the slow phase velocity in light with eyes fixed. Lesions of the cerebellum, brain stem, and cerebrum cause abnormalities of VS. We report a patient whose VS became a clue in the diagnosis of a disorder of the central nervous system. A 54-year-old man comp…

[Electrophysiology in ophthalmology].

Electrophysiological examinations are used in ophthalmology to determine the functionality of the neurons of the visual pathway. As electrophysiological examinations are independent of the subjective perception and response of the patient, they are considered to be objective functional diagnostics of vision. Electroretinography (ERG) is used to examine the retina (especially the photoreceptors), electro-oculography (EOG) is used to examine the functionality of the retinal pigment epithelium and visually evoked potentials (VEP) are used to evaluate the integrity of the entire visual pathway. Incidence and anatomy of gaze-evoked nystagmus in patients with cerebellar lesions.

BACKGROUND: Disorders of gaze-holding--organized by a neural network located in the brainstem or the cerebellum--may lead to nystagmus. Based on previous animal studies it was concluded that one key player of the cerebellar part of this gaze-holding neural network is the flocculus. Up to now, in humans there are no systematic studies in patients with cerebellar lesions examining one of the most common forms of nystagmus: gaze-evoked nystagmus (GEN). The aim of our present study was to clarify which cerebellar structures are involved in the generation of GEN. METHODS: Twenty-one patients with
