Establishing the Priorities for Emergency Care Research in Australia and Aotearoa New Zealand.

Emergency departments across Australia and Aotearoa New Zealand provide care to nearly 11 million patients annually, yet only a small proportion of care is supported by high-quality evidence. Identifying unanswered research questions that matter most to patients, consumers, whanau, carers and clinicians is essential to reducing research waste and duplication and improving emergency care. The Australasian College for Emergency Medicine, through its Clinical Trials Network, is undertaking a Priority Setting Partnership with the James Lind Alliance and supported by the College of Emergency Nursin

The emergency department arrival mode and its relations to ED management and 30-day mortality in acute heart failure: an ancillary analysis from the EURODEM study.

BACKGROUND: Acute heart failure patients are often encountered in emergency departments (ED) from 11% to 57% using emergency medical services (EMS). Our aim was to evaluate the association of EMS use with acute heart failure patients' ED management and short-term outcomes. METHODS: This was a sub-analysis of a European EURODEM study. Data on patients presenting with dyspnoea were collected prospectively from European EDs. Patients with ED diagnosis of acute heart failure were categorized into two groups: those using EMS and those self-presenting (non- EMS). The independent association between

Effectiveness of a multifaceted intervention to improve emergency department care of low back pain: a stepped-wedge, cluster-randomised trial.

BACKGROUND: Overuse of lumbar imaging is common in the emergency department (ED). Few trials have examined interventions to address this. We evaluated the effectiveness of a multifaceted intervention to implement guideline recommendations for low back pain in the emergency department. METHODS: We conducted a stepped-wedge, cluster-randomised trial in four EDs in New South Wales, Australia. After a 13-month control phase of usual care, the EDs received a multifaceted intervention to support guideline-endorsed care in a random order, based on a computer-generated random sequence, every 4 weeks

The Emergency Medical Treatment and Active Labor Act: Clinical Practice and Ethical Considerations.

The Emergency Medical Treatment and Active Labor Act (EMTALA), enacted in 1986, established the legal foundation for access to emergency medical care in the United States. This review examines EMTALA's historical origins, core requirements, and implications for pediatric emergency practice. The law emerged in response to widespread patient dumping practices in the 1980s, when hospitals routinely transferred or refused care to uninsured patients. In the 4 decades since, amendments and regulatory clarifications have expanded the law's scope and strengthened enforcement mechanisms. The modern ite

Medications that reduce emergency hospital admissions: an overview of systematic reviews and prioritisation of treatments.

BACKGROUND: Rates of emergency hospitalisations are increasing in many countries, leading to disruption in the quality of care and increases in cost. Therefore, identifying strategies to reduce emergency admission rates is a key priority. There have been large-scale evidence reviews to address this issue; however, there have been no reviews of medication therapies, which have the potential to reduce the use of emergency health-care services. The objectives of this study were to review systematically the evidence to identify medications that affect emergency hospital admissions and prioritise t

Self-harm and Violence Presenting to Emergency Care Registry (SAVER) project: protocol for a mixed-methods study.

INTRODUCTION: Intentional injuries can be broadly classified into those that are self-inflicted (eg, suicide, self-harm), and those that are inflicted by others (eg, homicide, assault). Many risk factors are the same for all intentional injuries. It is widely accepted that there needs to be a public health approach to self-harm and interpersonal violence prevention, including surveillance of presentations to emergency departments. Self-harm and interpersonal violence are important causes of morbidity and mortality in Wales. Interpersonal violence surveillance is already operationalised in Wale

Emergency management of patients with bleeding disorders: Practical points for the emergency physician.

Emergency department (ED) physicians are often the first point of contact for patients who present with bleeding symptoms. Work up and management of bleeding in the emergency room can be a daunting task as it requires: (1) accurate diagnosis of the bleeding cause, of which there is long list of common and rare etiologies (2); appropriate investigations and interpretation of the results; and (3) timely management of bleeding symptoms to prevent limb- or life-threatening complications. Crucial to the management of a bleeding patient is a thorough yet focused history exploring bleeding symptoms,

Safety and efficacy of intranasally administered medications in the emergency department and prehospital settings.

PURPOSE: The safety and efficacy of medications that may be administered via the intranasal route in adult patients in the prehospital and emergency department (ED) settings are reviewed. SUMMARY: When medications of appropriate molecular character and concentration are delivered intranasally, they are quickly transported across this capillary network and delivered to the systemic circulation, thereby avoiding the absorption-limiting effects of first-pass metabolism. Therapeutic drug concentrations are rapidly attained in the cerebrospinal fluid, making intranasal administration a very effect
