Efficacy of an emotion-oriented cognitive behavior therapy for delusions (CBTd-E) compared to waitlist in a single-blinded randomized-controlled trial.

Psychological interventions for delusions may be enhanced by targeting their presumed causal factors. An emotion-oriented variant of cognitive behavioral therapy for delusions (CBTd-E), designed to target affect regulation and maladaptive schemata, was evaluated for its effect on delusions. A single-blind, multicenter, randomized, waitlist-controlled trial was conducted in three German outpatient clinics. Ninety-four patients with psychotic disorders and persistent delusions were randomized to 25 individual sessions of CBTd-E over 6 months (n = 47) or waitlist (n = 47). CBTd-E included two mod

A pilot study of the feasibility and preliminary outcomes of sequential TF-CBT and EMDR online group therapy for adult women with histories of childhood sexual abuse.

BACKGROUND: CSA is associated with post-traumatic stress disorder (PTSD), dissociation, emotion-regulation difficulties and psychopathology. Trauma-focused cognitive behavioral therapy (TF-CBT) and eye movement desensitization and reprocessing (EMDR) are evidence-based interventions, but their sequential delivery in online groups for women with CSA-related complex trauma remains underexplored. OBJECTIVE: To examine feasibility and preliminary clinical outcomes of a 16-session online group intervention combining TF-CBT and EMDR Group Traumatic Episode Protocol (G-TEP), and to explore differenc

Longitudinal changes in brain function associated with symptom improvement in youth with PTSD.

BACKGROUND: Previous studies indicate that youth with posttraumatic stress disorder (PTSD) have abnormal activation in brain regions important for emotion processing. It is unknown whether symptom improvement is accompanied by normative changes in these regions. This study identified neural changes associated with symptom improvement with the long-term goal of identifying malleable targets for interventions. METHODS: A total of 80 functional magnetic resonance imaging (fMRI) scans were collected, including 20 adolescents with PTSD (ages 9-17) and 20 age- and sex-matched healthy control subjec

Lateral frontopolar theta-band activity supports flexible switching between emotion regulation strategies.

Flexible emotion regulation is essential for mental health and well-being. However, neurocognitive mechanisms supporting emotion regulation flexibility remain unclear. Lateral frontal pole (FPl) contributes to flexible behavior by monitoring the efficacy of alternative strategies. This preregistered study examines if FPl also supports flexible use of emotion regulation strategies. It focuses on pre-decision theta-band activity as a potential indicator of this adaptive process. Sixty-three participants performed an emotion regulation strategy-switching task, involving three phases: (1) implemen

Experiences of mothers with borderline personality disorder following an intervention to support mother-infant relationships: "more tools in my toolbox".

Motherhood is inherently demanding, and some of these demands may be amplified for women with borderline personality disorder (BPD). The exacerbation of BPD symptoms from parenting-related stressors can have adverse impacts on mother-infant relationships and infant development. This qualitative study explores Australian mothers' views of the impacts of mother-infant dialectical therapy plus (MI-DBT+). MI-DBT+ is an innovative therapeutic intervention which integrates mother-infant dialectical behavior Therapy (MI-DBT), a known effective intervention for the mental health of mothers with BPD, w

Predictors of treatment dropout in patients with posttraumatic stress disorder due to childhood abuse.

BACKGROUND: Knowledge about patient characteristics predicting treatment dropout for post-traumatic stress disorder (PTSD) is scarce, whereas more understanding about this topic may give direction to address this important issue. METHOD: Data were obtained from a randomized controlled trial in which a phase-based treatment condition (Eye Movement Desensitization and Reprocessing [EMDR] therapy preceded by Skills Training in Affect and Interpersonal Regulation [STAIR]; n = 57) was compared with a direct trauma-focused treatment (EMDR therapy only; n = 64) in people with a PTSD due to childhood

Efficacy of a two-session repetitive negative thinking-focused acceptance and commitment therapy (ACT) protocol for depression and generalized anxiety disorder: A randomized waitlist control trial.

This parallel randomized controlled trial aimed to evaluate the effect of acceptance and commitment therapy (ACT) focused on disrupting repetitive negative thinking (RNT) versus a waitlist control (WLC) in the treatment of depression and generalized anxiety disorder (GAD). Forty-eight participants with a main diagnosis of depression and/or GAD were allocated by means of simple randomization to a 2-session RNT-focused ACT intervention or to the WLC. The primary outcomes were emotional symptoms as measured by the Depression, Anxiety, and Stress Scales-21. Process outcomes included ACT- and RNT-r

Attending to emotional cues for drug abuse: bridging the gap between clinic and home behaviors.

Classical conditioning models of addiction provide keys to understanding the vexing discrepancy between substance abuse patients' desire to abstain when they are in therapy sessions and their tendency to relapse. Experiments using these models demonstrate the power of environmental relapse cues and support clinical approaches, including active exposure, aimed at helping patients recognize and withstand them. Internal cues, including emotions and somatic states such as withdrawal, can trigger urges as powerfully as external cues such as people, places, and things associated with prior abuse. Th

Exploring psychosocial experiences of behavior therapists dealing with children having autism spectrum disorder.

Behavior therapists play an important role in the life of children with an autism spectrum disorder (ASD). Literature reported that child's improvement depends on the quality of therapy they receive. Therefore, the current study aims to explore the psychosocial experiences of behavior therapist working with children having ASD, and suggest the coping strategies which they use to deal with those stressors. The study utilizes a qualitative research design with a phenomenological research approach to explore the psychosocial experiences. For this purpose, the researchers conduct semi-structured i

Expressed emotion and early treatment response in family-based treatment for adolescent anorexia nervosa.

This study examined the effect of pre-treatment levels of parental expressed emotion (EE) on early treatment response for adolescent anorexia nervosa (AN). Data were collected from 121 adolescents, ages 12-18, who met DSM-IV criteria for AN excluding the amenorrhea criterion, and their parents. Participants were randomized to family-based treatment (FBT) or adolescent-focused therapy (AFT). To examine the effects of different thresholds of EE, we used two different levels of EE in analyses. Results demonstrated that adolescents who had at least one parent with elevated EE indicated by a lower

In-session reflective functioning in psychotherapies for borderline personality disorder: The emotion regulatory role of reflective functioning.

OBJECTIVE: The capacity for understanding mental states (reflective functioning; RF) is considered essential for self-growth, social learning, and emotion regulation. Impaired RF is thought to play a central role in borderline personality disorder (BPD). We examined whether asking patients to consider mental states in-session has a down-regulatory effect on emotional arousal in treatments for BPD. METHOD: Early-, middle- and late-phase videotaped sessions from a randomized-controlled trial of transference-focused psychotherapy (TFP; n = 30), dialectical behavior therapy (DBT; n = 29), and sup

Current understanding of bipolar disorder: Toward integration of biological basis and treatment strategies.

Biological studies of bipolar disorder initially focused on the mechanism of action for antidepressants and antipsychotic drugs, and the roles of monoamines (e.g., serotonin, dopamine) have been extensively studied. Thereafter, based on the mechanism of action of lithium, intracellular signal transduction systems, including inositol metabolism and intracellular calcium signaling, have drawn attention. Involvement of intracellular calcium signaling has been supported by genetics and cellular studies. Elucidation of the neural circuits affected by calcium signaling abnormalities is critical, and

Art therapy and emotional pain: a scoping review of physiological and biological measures.

INTRODUCTION: The increasing prevalence of mental health disorders and emotional pain poses a critical challenge to social well-being and healthcare equity. Visual art therapy is well established as a clinical and nonclinical intervention for emotional pain that promotes self-regulation and psychological insight. However, there is a lack of research that clearly maps the previous studies that use both subjective and objective measures to examine the impact of art therapy on emotional pain. METHODS: This scoping review focuses on studies that use brain or physiological measurement in investiga

Emotional regulation strategies in daily life: the intensity of emotions and regulation choice.

OBJECTIVE: Emotion regulation is an adaptive ability affecting people's physical and mental health, quality of life and functioning. In the present study we focused on the influence of the intensity of experienced emotions on emotion regulation strategies (ERS) that are applied in everyday life. METHODS: For 7 days the participants kept an online diary where every day they described the situation which had aroused their strongest negative emotions. Next, they identified the emotions, their intensity and the type of applied strategies (acceptance vs. reappraisal vs. rumination vs. distraction

Basic emotions reported by individuals with persistent physical symptoms receiving exposure therapy versus healthy lifestyle promotion in primary care.

The importance of fear has been emphasized in research and treatment focusing on persistent physical symptoms (PPS). Recognizing a broader range of emotions may inform theory and personalized care. This study aimed to examine what basic emotions individuals with PPS experience related to their physical symptoms, the correlations between such emotions and overall somatic symptom burden and disability, and whether emotions change with treatment. Analyses drew on data from a randomized controlled trial that compared internet-delivered exposure therapy with healthy lifestyle promotion for patients

Effectiveness of a brief dialectical behavior therapy intensive-outpatient community health program.

Standard dialectical behavior therapy (DBT), with its 12-month format, has a documented record of efficacy. While emerging evidence is supportive of DBT adaptations in community mental health settings and brief, intensive formats, many of these studies are limited by sample size of its DBT group, by omission of program completion rates and specific data from program noncompleters, and by focusing solely on symptom-focused measures-which inadvertently omits observing gains associated with well-being. We used a nonexperimental design to assess client outcomes on pathology-focused and positive-ps

[Complex PTSD following early-childhood trauma: emotion-regulation training as addition to the PTSD guideline].

BACKGROUND: Posttraumatic stress disorder (PTSD) symptoms in individuals who have experienced repeated trauma (sexual and/or physical) in early childhood can lead to problems associated with emotion regulation, interpersonal functioning and self-image. This so-called complex PTSD is often accompanied by a comorbid personality disorder. Although ptsd is associated with structural and functional abnormalities in emotion-regulation areas in the brain, it is not known whether complex PTSD shows similar abnormalities. Experts take the view that before individuals with complex PTSD are given appropr

Intrusive Memory Frequency and Related Inner Tension Following Dialectical Behavior Therapy or Cognitive Processing Therapy for Posttraumatic Stress Disorder: An e-Diary Study.

BACKGROUND: Intrusive memories are a core symptom of posttraumatic stress disorder (PTSD), yet their retrospective assessment is prone to biases, making real-time methods such as e-diaries essential. While trauma-focused treatments target intrusive symptoms, their efficacy has not yet been evaluated using real-time assessments. OBJECTIVE: This study aimed to use e-diaries to assess and compare the effects of dialectical behavior therapy for PTSD (DBT-PTSD) and cognitive processing therapy (CPT) on intrusive memories and related inner tension in a large sample of women with childhood abuse-rel

Addressing Rehabilitation Needs in Pediatric Palliative Care: The Role of Occupational Therapy.

AIMS: Children with life-limiting illnesses face physical, cognitive, and emotional challenges that restrict their activities of daily living. Although these needs require a holistic approach, rehabilitation services, particularly occupational therapy, are often limited in pediatric palliative care. This study aimed to evaluate the unmet rehabilitation needs of children receiving pediatric palliative care in Turkey based on the Person-Environment-Occupation model. METHODS: A sequential explanatory mixed-methods design was used. Quantitative data on self-care, mobility, and caregiver burden we

Altering Dynamics of Autonomic Processing Therapy (ADAPT) trial: a novel, targeted treatment for reducing anxiety in joint hypermobility.

BACKGROUND: Hypermobility is a poorly recognised and understood musculoskeletal disorder thought to affect around 20% of the population. Hypermobility is associated with reduced physiological and psychological functioning and quality of life and is a known risk factor for the development of an anxiety disorder. To date, no evidence-based, targeted treatment for anxiety in the context of hypermobility exists. The present intervention (ADAPT-Altering Dynamics of Autonomic Processing Therapy) is a novel therapy combining bio-behavioural training with cognitive approaches from clinical health psyc
