Therapeutic effects of diosgenin in experimental autoimmune encephalomyelitis.

Multiple sclerosis (MS) is a chronic autoimmune demyelinating disease of the central nervous system. Currently, there is no drug available to cure this kind of disease. Diosgenin is a plant-derived steroid saponin. A previous study in our lab revealed that diosgenin can promote oligodendrocyte progenitor cell differentiation and accelerate remyelination. In the present study, we found that diosgenin dose-dependently alleviated the progression of experimental autoimmune encephalomyelitis with reduced central nervous system inflammation and demyelination. We also found that diosgenin treatment c…

Treatment of hemorrhagic acute disseminated encephalomyelitis with cyclophosphamide.

PURPOSE: Improvements in hemorrhagic and clinical symptoms in a patient with hemorrhagic acute disseminated encephalomyelitis (ADEM) treated with i.v. cyclophosphamide are reported. SUMMARY: A 49-year-old woman was hospitalized with progressively worsening left-sided weakness, dysphagia, diplopia, and vertigo. Shortly before hospital admission, the patient had been treated at another facility with corticosteroids and plasma exchanges. Brain magnetic resonance imaging (MRI) studies conducted at the time of admission showed demyelinating lesions of the pons consistent with ADEM; rapid progressi…

Acute disseminated encephalomyelitis presenting as a brainstem encephalitis.

Acute disseminated encephalomyelitis (ADEM) is a disease characterized by inflammation and destruction of myelin. Acute hemorrhagic leukoencephalitis (AHLE) is a severe form of ADEM known for its particularly poor outcome. We present a case of a young Caucasian female who presented with drowsiness and slurred speech followed by rapid brainstem involvement resembling rhomboencephalitis. Despite multiple diagnostic tests and empiric therapy with immunosuppressants, immunoglobulins, and antimicrobials, she lost most brainstem reflexes within a few weeks and ultimately passed away. Magnetic resona…

Acute demyelinating encephalomyelitis in a child following malaria.

Acute demyelinating encephalomyelitis usually follows viral infections and its occurrence following malarial infection is very uncommon. We report a 12-year-old girl who presented with encephalopathy and generalized convulsions following complete recovery from the Plasmodium falciparum infection. Diagnosis of ADEM was made on the basis of brain MRI findings.

Clinical profile of acute disseminated encephalomyelitis in children.

AIM: To study the clinical profile of acute disseminated encephalomyelitis (ADEM) in children. MATERIALS AND METHODS: All children admitted with ADEM during a period of one and a half years were included in the study. The diagnosis of ADEM was made based on the clinical presentation and suggestive MRI findings. All children were treated with intravenous methyl prednisolone, followed by oral prednisolone and followed up for varying periods up to three and a half years. RESULTS: The sample consisted of 14 children with 11(79%) girls and 3 (21%) boys. The oldest child was 12 years and the young…

What's in a name? Experimental encephalomyelitis: 'allergic' or 'autoimmune'.

New linguistic coinage can signify new practices and fresh perceptions in science: descriptors therefore are not trivial. Here, we consider the shifting valence of 'allergic' and 'autoimmune' in conceptions of experimental encephalomyelitis (EE). Ehrlich's dismissal of the relevance to disease of autoimmunity resulted in its 'long struggle for recognition' notwithstanding the convincing attribution in 1904 of the hemolysis of paroxysmal cold hemoglobinuria. Yet allergy did take hold because of its assumption that harmful effects could be ascribed to an extrinsic agent against which immune resp…

Torticollis as the only manifestation of acute disseminated encephalomyelitis.

Torticollis can be either congenital or acquired. Acquired torticollis is often the manifestation of an underlying central nervous system disorder. Acute painless torticollis should always raise suspicion of a posterior fossa tumor. Acute disseminated encephalomyelitis is an inflammatory demyelinating disease of the central nervous system involving the subcortical white matter, and to a lesser extent, the gray matter. The illness typically has a monophasic course characterized by a variable combination of fever, headache, meningismus, seizures, spasticity, cranial nerve palsies, ataxia, and ps…

Bladder dysfunction in mice with experimental autoimmune encephalomyelitis.

The vast majority of patients with multiple sclerosis (MS) develop bladder control problems including urgency to urinate, urinary incontinen … different defined levels of neurological impairment following induction of experimental autoimmune encephalomyelitis (EAE), an animal model of central nervous system inflammation widely used in MS research. We found that EAE mice develop profound bladder dysfunction characterized by significantly increased micturition frequencies and significantly decreased urine …

Acute disseminated encephalomyelitis: a report of six cases.

Six children with Acute Disseminated Encephalomyelitis (ADEM) were seen at the Penang Hospital over a two year period (July 1999-June 2001). Diagnosis was based upon typical clinical features and characteristic findings on neuroimaging. Cerebrospinal fluid examination and other investigations were done, where appropriate, to rule out other causes of central nervous system disease. Three children had a prodromal illness. The most common presenting symptoms were fever, seizures, ataxia, focal neurological deficits and labile mood. Two children presented with status epilepticus. All children had …

A residual cystic lesion in acute disseminated encephalomyelitis.

We report a case of acute disseminated encephalomyelitis (ADEM) with a residual cystic lesion on MRI. This seemed to be induced by Japanese encephalitis vaccination. Despite complete clinical improvement with high-dose steroid therapy, the cystic lesion has persisted for 3 years on MRI. There have been no previous reports of residual cystic lesions in ADEM.

Psychopathological changes and cognitive impairment in encephalomyelitis disseminata.

Two hundred and twelve patients with clinically evidenced encephalomyelitis disseminata (ED), hospitalized in a neurological hospital, were observed with regard to psychopathological characteristics and cognitive changes in conformity with ICD-10 diagnostic criteria. The basis of this investigation was a standardized psychiatric interview. The age of the patients averaged 47 years whereas the duration of the disease averaged 14.3 years. 83.5% of the patients had a disease history of more than 6 years. The medium range of EDSS scores was 5.95%, the BPRS 36.7%. In 5.2% of the patients the course…

Genetics of susceptibility to chronic experimental encephalomyelitis and arthritis.

The recent developments in genetic techniques and the development of more appropriate animal models for rheumatoid arthritis and multiple sclerosis make it possible to use a new approach for understanding these complex diseases. Thus it is now meaningful to address the question of which genes are causing the diseases. Several new associations with loci outside the MHC region have now been identified in models for both rheumatoid arthritis and multiple sclerosis. Some of these are shared between diseases - for example loci on mouse chromosome 3 (experimental allergic encephalomyelitis, collagen…

Neuropsychiatric features of acute disseminated encephalomyelitis: a review.

Acute disseminated encephalomyelitis is a disorder associated with significant morbidity and mortality. A description and literature review are presented to focus attention on the myriad neuropsychiatric manifestations of this disease. Common psychiatric symptoms include lethargy, irritability, and confusion. Ataxia, seizures, and other signs representing involvement of various areas of the brain and spinal cord are common neurologic presentations. The cerebrospinal fluid shows only nonspecific abnormalities, whereas magnetic resonance imaging may show various lesions in the white matter repre…

[A case of early-onset acute disseminated encephalomyelitis].

We reported a rare case of early-onset acute disseminated encephalomyelitis (ADEM). After a nonspecific upper respiratory infection, a 14-month-old boy developed oculomotor nerve paralysis, nystagmus, intention tremor and ataxic gait. MRI showed extensive symmetric high signal lesions in th bilateral cerebellar and cerebral white matters. We made the diagnosis of ADEM on the basis of high CSF myelin basic protein, clinical course, symptoms and MRI findings. Following the administration of steroid hormone, his clinical symptoms rapidly improved.

The role of Mycobacterium tuberculosis in experimental allergic encephalomyelitis.

Various detergents used in preparative membrane protein chemistry were added to a complete Freund's adjuvant/water emulsion in order to incr … B breakdown and cerebral edema were observed, similar to those encountered in experimental allergic encephalomyelitis (EAE). We suggest that the immunologic response triggered off by M. tuberculosis largely contributes to the BBB permeability changes observed during EAE, probably by an action on the endothelial cells of the cerebral blood vessels.

Eastern equine encephalomyelitis with hematuria and bladder dysfunction.

We have described a patient with eastern equine encephalomyelitis acquired in central Virginia, an area in which EEE is not ordinarily considered endemic. The unusual presenting features of our patient's encephalomyelitis were bladder dysfunction and hematuria as well as prominent mood and affect disturbances, which are usually seen in association with St. Louis encephalitis. The diagnosis of arboviral encephalomyelitis was considered because of the severe CNS manifestations and spinal cord involvement, and was facilitated by the detection of specific IgM antibodies in both the CSF and serum. …

[Periphlebitis retinae ("Rucker")--a symptom of disseminated encephalomyelitis].

Within a group of neuro-ophthalmic patients (147 patients with either florid or cured optic neuritis (ON), five (3.4%) were found to be suffering from retinal periphlebitis "Rucker". Of these five patients, three were also suffering from Encephalomyelitis disseminata (E.d.). One patient had ON without E.d.; in another there were sufficient grounds for a tentative diagnosis of ON during the follow-up period. The relatively low percentage of patients with retinal periphlebitis associated with a basic demyelinating condition can be attributed primarily to the fact that the group of patients exami…
