Successful percutaneous epicardial access in challenging scenarios.

BACKGROUND: This case-series highlights strategies used for successful epicardial access in challenging cases. Percutaneous epicardial access has become a valuable tool for mapping and ablating arrhythmias. However, this technique can be especially difficult in certain circumstances and is frequently avoided. METHODS: All cases of epicardial access for ablation from our institution were reviewed searching for exceptionally difficult cases in patients with complex pericardial and thoracic anatomy. The successful strategies are characterized in this report. RESULTS: Among 144 patients who unde

Hybrid Ablation of Ventricular Tachycardia: a Single-Centre Experience.

BACKGROUND: The long-term results of endocardial and percutaneous epicardial catheter ablation of ventricular tachycardia (VT) in patients with structural heart disease are disappointing. Arrhythmia recurrence after ablation and VTs with an epicardial substrate remain a clinical challenge. The purpose of this manuscript is to elaborate on feasibility and potential advantages of a surgical hybrid ablation (i.e., combined endocardial and surgical epicardial ablation) based on our initial experience consisting of five cases. METHODS: Endocardial electro-anatomical voltage and activation maps wer

Hybrid Surgical Ablation for Ventricular Arrhythmias.

Hybrid surgical ventricular tachycardia (VT) ablation combines surgical epicardial access/exposure with contemporary mapping and ablation techniques adapted from percutaneous catheter ablation procedures. Patients considered for a hybrid surgical approach for VT are those who have had prior cardiac surgery or failed percutaneous epicardial access due to pericardial adhesions. They often represent the most challenging end of the spectrum of patients and usually have undergone multiple unsuccessful ablations. In this review, the indications, preprocedure work-up, ablation techniques, and outcome

A novel diagnostic tool to identify atrial endo-epicardial asynchrony using signal fingerprinting.

OBJECTIVE: Patients with persistent atrial fibrillation (AF) have more electrical endo-epicardial asynchrony (EEA) during sinus rhythm (SR) than patients without AF. Prior mapping studies indicated that particularly unipolar, endo- and/or epicardial electrogram (EGM) morphology may be indicators of EEA. This study aim to develop a novel method for estimating the degree of EEA by using unipolar EGM characteristics recorded from either the endo- and/or epicardium. METHODS: Simultaneous endo-epicardial mapping during sinus rhythm was performed in 86 patients. EGM characteristics, including unipo…

A radical operation was performed with the patient under cardiopulmonary bypass. On epicardial mapping, delayed activation of the RVOT was recorded during sinus rhythm, and clockwise circus movement of the macroreentry current during VT on the right ventricular free wall was documented in each case. The VTs were treated successfully by surgical resec …

Double-QRS Complex Revealing a Multilayer Intramural-Epicardial Ventricular Tachycardia Circuit in Cardiac Sarcoidosis.

BACKGROUND: Cardiac sarcoidosis can create multilayer intramural-epicardial conduction pathways that manifest as subtle electrocardiographic abnormalities during sinus rhythm. A second low-amplitude deflection following the QRS complex ("double-QRS") has been observed clinically, but its electrophysiological mechanism remains unclear. CASE SUMMARY: A 67-year-old man with cardiac sarcoidosis exhibited a sinus-rhythm double-QRS pattern that changed in timing and polarity after corticosteroid therapy and preceded sustained monomorphic ventricular tachycardia. High-density mapping identified 2 in

Anatomic Considerations Relevant to Atrial and Ventricular Arrhythmias.

Knowledge of relevant cardiac anatomy is crucial in understanding the pathophysiology and treatment of arrhythmias, and helps avoid potential complications in mapping and ablation. This article explores the anatomy, relevant to electrophysiologists, relating to atrial flutter and atrial fibrillation, ventricular tachycardia relating to the outflow tracts as well as endocardial structure, and also epicardial considerations for mapping and ablation.

Hybrid Surgical-Catheter Epicardial Ablation of Ventricular Tachycardia: A Case Series.

Background: Epicardial mapping and ablation of ventricular tachycardia (VT) are used in different clinical situations but pericardial adhesions following prior cardiac surgery or previous epicardial procedures may limit a percutaneous approach. The objective of this case series is to evaluate the safety and feasibility of a hybrid approach with surgical epicardial access as a valid alternative when pericardial space is not accessible percutaneously. Methods: After a complete preprocedural evaluation, four patients with prior cardiac surgery underwent hybrid VT ablation under general anesthesia…

Hybrid epicardial ventricular tachycardia ablation with lateral thoracotomy in a patient with a history of left ventricular reconstruction surgery.

Although a hybrid procedure involving surgical access may be feasible for epicardial catheter ablation in individuals with prior cardiac surgery, surgical approaches in thoracotomy are important in patients with advanced adhesions. We performed an epicardial ventricular tachycardia (VT) ablation in a patient with dilated phase hypertrophic cardiomyopathy after left ventricular reconstruction. We gained surgical epicardial access via lateral thoracotomy based on the anticipated VT circuit in the apical anteroseptal area, which was estimated using prior endocardial mapping. The remaining epicard

Studies on the relationship between ST-segment elevations and extent of infarction following coronary artery occlusion in dogs.

Epicardial ST-segment alterations were determined 15 min. following acute ligation of the left anterior descending coronary artery in dogs, and the extent of the left ventricular wall infarcted at each mapping site was determined 24 hours later. A close correlation was observed between the ST-segment elevation and the percent of left ventricular wall infarcted (r = .891; P less than .001) at the same mapping site. A significant portion of the left ventricular wall remained viable at sites with less than 8 mV elevation; however, the chance of transmural infarction became high at sites with elev

A new insight into the anatomical ablation approach at R-L ILT for VAs with a left ventricular summit origination: electrophysiological characteristics and catheter ablation.

BACKGROUND: Ventricular arrhythmia (VA) originating from the left ventricular summit (LVS) poses particular challenges, with higher rates of ablation failure. OBJECTIVE: To further evaluate the anatomical ablation approach from the subaortic region for LVS VAs and their electrophysiological characteristics. METHOD: The study enrolled 27 consecutive patients with sympatomatic VAs originating from LVS and who received an anatomical ablation approach from R-L ILT in our center. RESULTS: Three different mapping results were obtained as the earliest activation sites (EAS) were observed in the RV

Canine model of Brugada syndrome using regional epicardial cooling of the right ventricular outflow tract.

INTRODUCTION: Myocardial cooling can induce J point elevation (Osborn wave) as seen on ECG of the Brugada syndrome by activating transient outward current (Ito) and causing a spike-and-dome configuration of the monophasic action potential (MAP) in the ventricular epicardium in isolated canine ventricular wedge preparations. We determined the effect of regional epicardial cooling of the right ventricular outflow tract (RVOT) on surface ECG and ventricular vulnerability in the dog. METHODS AND RESULTS: In 12 dogs, a cooling device (20-mm diameter) was attached to the RVOT epicardium, and surfac

Three-dimensional mapping, recording and ablation in simulated and induced ventricular tachyarrhythmias during mechanical circulatory support using the percutaneous heart pump.

PURPOSE: Due to their internal rotating magnets, conventional impeller-driven percutaneous ventricular assist devices (PVADs) yield high-frequency electrogram artifact and electromagnetic interference (EMI) when used with magnetic-based 3D electroanatomic mapping systems. The new percutaneous heart pump (PHP; Abbott, Chicago, IL) is a 14-French, 5-L/min, impeller axial-flow PVAD with a novel design that utilizes an external motor. METHODS: We evaluated the feasibility of 3D mapping and radiofrequency ablation (RFA) in vivo during PHP mechanical circulatory support (MCS) in simulated ventricul

Premature Ventricular Complexes With 2 QRS Morphologies: Cardiac Crux Insights and an Endocardial Ablation Strategy.

BACKGROUND: Premature ventricular complexes (PVCs) from the cardiac crux often have intramural or epicardial substrates, complicating localization and ablation. CASE SUMMARY: A 53-year-old man presented with symptomatic, flecainide-refractory PVCs with a 24-hour Holter burden of 41.8%. An electrocardiogram showed frequent PVCs with 2 alternating QRS morphologies (right and left bundle branch block), both with a superior axis and subtle V1 to V2 and/or precordial differences, suggesting a single crux focus with preferential exits. Proximal coronary sinus mapping did not reveal early activation

Cardiac inflammation and fibrosis patterns in systemic sclerosis, evaluated by magnetic resonance imaging: An update.

Systemic sclerosis (SSc) presents high morbidity/mortality, due to internal organ fibrosis, including the heart. Cardiac magnetic resonance (CMR) can perform myocardial function and tissue characterization in the same examination. The Lake Louise criteria (LLC) can identify recent myocardial inflammation using CMR. Abnormal values include: (a) myocardial over skeletal muscle ratio in STIRT2-W images >2, (b) early gadolinium enhancement values >4, (c) epicardial/intramyocardial late gadolinium enhancement (LGE). The diagnosis of myocarditis using LLC is considered if 2/3 criteria are positive.

Coronary Venous Mapping and Catheter Ablation for Ventricular Arrhythmias.

Catheter ablation is an effective treatment method for ventricular arrhythmias (VAs). These arrhythmias can often be mapped and targeted with ablation from the left and right ventricular endocardium. However, in some situations the VA site of origin or substrate may be intramural or epicardial in nature. In these cases, the coronary venous system (CVS) provides an effective vantage point for mapping and ablation. This review highlights situations in which CVS mapping may be helpful and discusses techniques for CVS mapping and ablation.

Feasibility of label-free multiphoton luminescence imaging to track the fate of inhaled gold nanoparticles in ex vivo lung tissue.

SIGNIFICANCE: Cardiac panoramic optical mapping is a powerful approach for studying action potential dispersion and mapping arrhythmia triggers and propagation pathways over the entire surface of the heart. However, tissue type (muscle, connective tissue, and infarct scar) is also important for interpreting mapping data and is difficult to identify using optical mapping data alone. AIM: Panoramically map transmembrane potential and tissue type from the surface of infarcted hearts for correlative analysis of cardiac structure and function. APPROACH: We developed a multimodal panoramic imaging

Nonpharmacologic treatment of supraventricular tachycardia.

During the last 2 decades, more than 1,000 patients have undergone surgical division of an accessory pathway; many refinements in epicardial mapping and surgical techniques have been made. In institutions where the procedure is routinely performed, the rate of successful accessory pathway interruption now approaches 98%. Concomitant risk of complete heart block among patients with posteroseptal pathways has declined to less than 5%. Among patients with other organic heart disease, myocardial preservation techniques result in mortality rates within 1 to 2%; among those with significant cardiac …
