Adult Burkitt's lymphoma is an uncommon disease. Few cases of spinal involvement in adults with sporadic Burkitt's lymphoma are reported in the literature. We present a case of a middle-aged man who was found to have an epidural mass in the thoracic spine when investigated for back pain and lower extremity weakness. He underwent a laminectomy with resection of the epidural mass. Histologic examination revealed a primary Burkitt's lymphoma of the spinal cord. He was treated with aggressive chemotherapy and is now experiencing remission of his disease. We also present a review of the literature

BACKGROUND: Follicular thyroid carcinoma (ftc) is a malignant neoplasm arising from thyroid follicular epithelial cells and is the second most common thyroid cancer after papillary thyroid carcinoma. meningeal metastasis from ftc with calvarial destruction is exceedingly rare and is often misdiagnosed clinically as meningioma. we reviewed previously reported cases in the literature and, in addition, report and include one case of meningeal metastasis from ftc treated at our center.
METHODS: Using the keywords "meningioma," "follicular thyroid carcinoma," and "cerebral metastatic tumor," we se

Superficial siderosis of the central nervous system (SSCNS) is an uncommon and often unrecognized disorder that results from recurrent and persistent bleeding into the subarachnoid space. Currently, there is no effective treatment for SSCNS. The identification and surgical resolution of the cause of bleeding remains the most reliable method of treatment, but the cause of bleeding is often not apparent. The identified sources of recurrent bleeding have typically included neoplasms, vascular malformations, brachial plexus or nerve root injury or avulsion, and previous head and spinal surgery. An

Excision of neoplasm and trauma involving the anterior cranial base may often result in communication between the intracranial and extracranial compartments. Many techniques have been proposed to obtain a watertight separation. We report our 5 years of experience in the management of anterior skull base defects using a galeal-pericranial flap. Between January 2001 and April 2006, 22 patients were treated for a cranial base reconstruction at the University of Messina. Five of them presented with persistent cerebrospinal fluid (CSF) leak after previous craniofacial trauma. Ten underwent a combin

OBJECTIVE: To evaluate the outcomes from a new surgical technique for lobectomy.
PATIENTS: Two hundred fifty consecutive patients with an average age of 67.3 years underwent simultaneously stapled lobectomy.
METHODS: Video-assisted thoracic surgical non-rib spreading lobectomy (VNSSL) is a new technique that has been evolving for approximately 6.5 years. During 1990, we began using video-assisted thoracic surgery (VATS) for simple, benign diseases. Throughout 1991, VATS was applied to malignant problems, ie, mediastinal masses, staging of lymph nodes, malignant effusions, and coin lesions. A

BACKGROUND: Thymoma is the most common anterior mediastinal neoplasm in adults and often requires surgical resection for curative treatment. Traditional approaches, including open surgery and multiportal thoracoscopic techniques, are associated with significant morbidity. The uniportal subxiphoid approach offers a minimally invasive, patient-friendly alternative, with potential benefits in operative efficiency and post-operative recovery. This technique may be particularly advantageous in low-resource settings, given its simplicity and reduced need for specialized equipment or personnel.

Langerhans cell histiocytosis (LCH) is a rare hematologic neoplasm predominantly affecting children, with adult-onset cases being exceedingly uncommon. Neurological presentations, including spinal cord compression and cranial neuropathies, represent atypical manifestations that often lead to diagnostic delays. We present a 20-year-old previously healthy female who presented with progressive back pain, bilateral lower extremity weakness, and left-sided hearing loss. Imaging revealed a thoracic epidural mass causing severe spinal stenosis with additional diffuse osteolytic lesions. Initial biops

PURPOSE: Monitoring of somatosensory evoked potentials (SSEPs) serves as an early warning system to detect spinal cord injury and is correlated with postoperative sensory findings. It is an indirect indicator of motor function. This study aimed to evaluate the usefulness of intraoperative SSEPs monitoring as a stand-alone tool during spinal surgeries when motor evoked potentials are not available, to prevent and predict new postoperative neurologic deficits. Motor evoked potentials were not used as the equipment needed to record them was not available at the time of this study.
METHODS: This

STUDY DESIGN: Case report.
OBJECTIVE: To describe a patient with nodular fasciitis arising in the lumbar extradural space.
SUMMARY OF BACKGROUND DATA: Nodular fasciitis is a benign proliferation of fibroblasts and myofibroblasts. It commonly occurs in the subcutaneous tissue of an upper extremity, trunk, head, and neck, but rarely arises in the spinal canal.
METHODS: A 7-year-old boy experienced gradually increasing intense radiating pain from the bilateral buttocks to the lower extremities after a bruise on his lower back. Computed tomography and magnetic resonance imaging demonstrated a r

BACKGROUND AND OBJECTIVES: This study was designed to investigate whether single epidural droperidol or continuous epidural droperidol inhibit pruritus and postoperative nausea and vomiting induced by postoperative continuous epidural fentanyl administration, and to identify the optimal method of administering epidural droperidol.
METHODS: 120 ASA I-II patients undergoing subtotal gastrectomy with general anaesthesia combined with epidural anaesthesia were randomly allocated into three groups: control (no droperidol), single injection (droperidol 2.5 mg) and continuous group (droperidol 2.5 m

PURPOSE: The relationship between spine SBRT outcomes and the extent of epidural cauda equina compression (ECEC) by malignant epidural disease has yet to be reported. Our objective was to determine clinical, anatomic and dosimetric factors that predict for local failure (LF) and overall survival (OS) specific to ECEC.
METHODS: Consecutive patients with ECEC treated with spine SBRT were retrospectively reviewed. ECEC parameters including anatomic measurements, lumbar stenosis grading, anatomic disease position and various dosimetric factors, were analyzed for their prognostic utility. Covariat

BACKGROUND CONTEXT: Ewing's sarcoma is the most common childhood malignancy of bone, but it rarely occurs as a primary extraosseous epidural tumor.
PURPOSE: To heighten awareness and treatment options of this rare (epidural) presentation of Ewing's sarcoma.
STUDY DESIGN: Case report.
METHODS: Retrospective review.
RESULTS: We present a 13-year-old boy with progressive low back and bilateral lower extremity pain and normal neurologic examination. Magnetic resonance imaging showed an extradural tumor in the dorsal epidural space from T11 to L1. He underwent a posterior laminoplasty and gross

RATIONALE: Esophageal cancer is one of the leading causes of death worldwide; the treatments vary according to the stage at diagnosis. Advanced esophageal cancer is usually treated by concurrent chemoradiation which is associated with complications including esophagitis, esophageal stricture or perforation, radiation pneumonitis, and/or cardiac toxicity. Herein, we describe epidural abscess, which is a very rare but severe complication that can occur after concurrent chemoradiation therapy for advanced esophageal cancer.
PATIENT CONCERNS: A 75-year-old man developed a fever during concurrent

A 16-year-old boy first presented with a pineal tumour identified by neuroimaging but without positive serum or cerebrospinal fluid markers. The tumour disappeared after 50 Gy cranial irradiation. One year later he returned with spinal epidural metastasis from the pineal germinoma and required emergency surgery. Intraoperative findings showed that the spinal tumour had originated from cerebrospinal fluid dissemination and had passed through the spinal nerve sleeve. The pathologic diagnosis of the tumour was of a pure germinoma metastasis. An epidural tumour frequently requires emergency diagno

A 75-year-old man presented with dysphagia and subsequent paraparesis. He was treated by oesophageal dilatation and by the placement of an 8.4 cm Atkinson tube. It was shown that an oesophago-subarachnoid fistula, a rare complication of carcinoma of the oesophagus, had developed. This resulted from direct extension of the tumour to the spinal cord and caused paraparesis unrelated to treatment.

BACKGROUND: Different types of analgesia have been proposed for the prevention of postoperative respiratory complications. The aim of this prospective, double-blind randomized study was to compare the impact of epidural bupivacaine and opioids versus parenteral opioids on respiratory complications in patients who had undergone major abdominal surgery.
METHODS: One hundred fifty-three patients undergoing abdominal surgery for cancer were randomly allocated to receive either general anesthesia with intravenous fentanyl and postoperative analgesia with subcutaneous morphine (SC group) or general

Spinal epidural abscess (SEA) is a surgical emergency if it causes paraplegia. Staphylococcus aureus and streptococci are the most common causes. Streptococcus gallolyticus has been reported to cause SEA only on three occasions earlier-all were associated with endocarditis or colonic malignancy. We report an older woman with diabetic ketoacidosis who presented with poorly localised back pain, fever and altered sensorium. Her lumbar puncture revealed frank pus, and MRI showed an SEA. She could not be weaned from mechanical ventilation post-surgical decompression, and she succumbed to ventilator

Postoperative epidural haematoma distant to a site of craniotomy is a rare but possibly hazardous complication. We report a 31-year-old female who presented with a history of chronic hydrocephalus due to fourth-ventricular plexus papilloma. Following resection of the posterior fossa tumour with intraoperative placement of a ventricular drainage, she consecutively developed four supratentorial epidural haematomas at different locations, all necessitating evacuation. The clinical manifestations ranged from subtle neurological deficits to signs of tentorial herniation; the ultimate outcome was co
