[Local anaesthetics containing epinephrine for use in the hand and fingers].

In the Netherlands there is no consensus regarding the addition of epinephrine (adrenaline) to local anaesthetics for use in the fingers. This is based on the persistent belief that the addition of epinephrine to local anaesthetics for use in the fingers is contraindicated due to the risk of ischaemic complications. Ischaemic complications arising from the use of modern anaesthetics containing epinephrine have, however, never been described. The addition of epinephrine results in a reduced arterial blood-flow and has several benefits, such as reduced blood loss and extended duration of anaesth…

Epinephrine sprayed on the papilla for prevention of post-ERCP pancreatitis.

BACKGROUND: Epinephrine sprayed on the papilla may reduce papillary edema and thus prevent acute pancreatitis after endoscopic retrograde cholangiopancreatography (ERCP). The aim of this study was to determine the efficacy of this technique for prevention of post- ERCP pancreatitis. METHODS: Patients scheduled for ERCP were recruited into this study. We randomized the patients to have 10 ml of either 0.02% epinephrine (epinephrine group) or saline (control group) sprayed on the papilla after diagnostic ERCP and prospectively analyzed the occurrence of post-ERCP pancreatitis between the groups…

A comparison of vasopressin and epinephrine for out-of-hospital cardiopulmonary resuscitation.

BACKGROUND: Vasopressin is an alternative to epinephrine for vasopressor therapy during cardiopulmonary resuscitation, but clinical experience with this treatment has been limited. METHODS: We randomly assigned adults who had had an out-of-hospital cardiac arrest to receive two injections of either 40 IU of vasopressin or 1 mg of epinephrine, followed by additional treatment with epinephrine if needed. The primary end point was survival to hospital admission, and the secondary end point was survival to hospital discharge. RESULTS: A total of 1219 patients underwent randomization; 33 were exc…

Increased concentrations of plasma epinephrine and norepinephrine in patients with eclampsia.

We measured plasma catecholamine concentrations on admission (after eclamptic fit) and after 6 days of delivery in 21 eclamptic patients and on admission in 15 normotensive pregnant women in Bangladesh. Plasma epinephrine and norepinephrine concentrations in eclamptic patients were significantly higher on admission than those of normotensive pregnant women (P < 0.0001). Plasma catecholamine concentrations and mean arterial blood pressure had return to be almost normal as normotensive pregnant women after 6 days of delivery, resulting in no correlation between mean arterial blood pressure and p…

Effects of papaverine pilocarpine and epinephrine on fistulized rabbit conjunctiva.

Pretreatment with pilocarpine hydrochloride has been demonstrated to increase the number of myofibroblasts in fistulized conjunctiva in rabb … solutions of 4% pilocarpine hydrochloride, 0.3% papaverine, a combination of 4% pilocarpine and 2% epinephrine, or artificial tears for 3 months. Four of five rabbits in each group underwent thermal sclerostomies in both eyes; the fifth rabbit in each group served as a nonsurgical control. The mean number of myofibroblasts per high-power field was greater in …

Cardiovascular effects of local anesthesia with epinephrine in periodontal treatment.

To assess if the association of epinephrine with local anesthesia involves cardiovascular risks, 14 patients undergoing periodontal treatment were observed. Systolic and diastolic blood pressures and mean and maximal heart rates were analyzed. The comparison of basal and postanesthetic periods revealed a trend (nonsignificant) toward an increase in systolic blood pressure and a small, statistically significant increase in diastolic blood pressure; the heart rate increased in only a few patients, and never by more than 10 beats/min. The whole group showed a statistically significant reduction i…

Use of dilute epinephrine as an aid in facial nerve monitoring.

Facial nerve monitoring during otologic and neurotologic procedures has been previously described, and its use is becoming routine. Although these procedures are done under general anesthesia, lidocaine is often used as a vehicle for epinephrine to aid hemostasis during the procedure. The routine use of lidocaine in these preparations presents the theoretical and sometimes real problem of anesthetizing the facial nerve at the start of the procedure, thereby invalidating subsequent attempts at monitoring and stimulation. We present the data from our experience with 74 patients using an epinephr…

Evidence for retrograde degeneration of epinephrine neurons in Alzheimer's disease.

Alzheimer's disease (AD) is associated with a progressive loss of locus ceruleus neurons. These noradrenergic neurons receive a major afferent projection from epinephrine neurons in epinephrine cell groups in the brainstem. The epinephrine neurons have a specific enzymatic marker, phenylethanolamine N-methyltransferase (PNMT), which allows them to be identified chemically and immunohistochemically. We have previously reported a decrease in PNMT in brains of patients with AD. We now report that the decrease in PNMT activity in projections to the locus ceruleus is not due to the loss of epinephr…

Desensitization to topical epinephrine in the rabbit eye: attenuation by dexamethasone.

Desensitization of ocular adrenergic receptors was examined in vivo in albino rabbit eyes. Repeated topical administration of 2.0% l-epinephrine bitartrate (equivalent to 1.1% free base) to the rabbit eye resulted in the loss of ocular hypotensive response on the second day of daily or twice-daily treatment. Concurrent topical administration of dexamethasone phosphate (0.1%) prolonged the pressure-lowering effect of 2.0% l-epinephrine for up to 5 days. Loss of pressure-lowering response was accompanied by 100% inhibition of ciliary process beta-adrenergic-sensitive adenylate cyclase activity f…

Possible involvement of epinephrine in the cardiovascular effect of naloxone in humans.

The opioid-receptor antagonist naloxone was administered intravenously in a 0.8-mg bolus to five healthy volunteers, aged 21 to 31, in a double-blind study designed to investigate the effect of endogenous opioids on blood pressure, heart rate, and urinary excretion of catecholamines in healthy adults. Three hours after administration of naloxone there were significant increases in systolic blood pressure (P less than 0.001) and heart rate (P less than 0.05). The amount of epinephrine excreted in urine during the four hours after administration of naloxone or placebo was significantly higher (P…

The effects of epinephrine infusion in patients with von Willebrand's disease.

Epinephrine infusion causes variable increases in the components of the Factor VIII (antihemophilic factor) complex in patients with von Willebrand's disease. The increase in antihemophilic factor procoagulant activity was greater than that of Factor VIII-related antigen and von Willebrand factor activity in two patients with von Willebrand's disease. Similar increases in the three individual factors were demonstrated in two other patients. A 4-10-fold increase in Factor VIII-related properties was identified in each of these individuals after infusion. One patient has been studied with very s…

Comparison of 2% lidocaine with epinephrine and 4% articaine with epinephrine in local anesthesia of maxillary molars affected by molar incisor hypomineralization: a randomized controlled trial.

The use of local anesthesia is essential for pain control during the treatment of MIH-affected molars. This study aimed to compare injection-related pain and anesthetic efficacy between 2% lidocaine with epinephrine (LE) and 4% articaine with epinephrine (AE) in the MIH-affected maxillary molars of pediatric patients. Twenty-six children aged 6 to 12 years requiring restorative treatment of MIH-affected maxillary molars were enrolled. Participants randomly received one of the two anesthetic solutions (LE or AE) at the first visit; the other solution was administered at the second visit. Inject…

Lidocaine With Epinephrine Versus Bupivacaine With Epinephrine as Local Anesthetic Agents in Wide-Awake Hand Surgery: A Pilot Outcome Study of Patient's Pain Perception.

PURPOSE: Wide-awake local anesthesia hand surgery has many advantages over other forms of anesthesia, including faster recovery, lower cost, and improved patient safety; however, few studies compare postoperative pain and analgesic consumption after long- and short-acting anesthetics. This is important because surgeons seek to minimize opioid consumption during the opioid epidemic. METHODS: This was a double-blinded, prospective, randomized, parallel design pilot study. We randomized 61 patients to receive carpal tunnel surgery with a short-lasting regional anesthetic (lidocaine, 29 patients)

Addition of Intramuscular Epinephrine to Standard of Care by Paramedics to Decrease Time-to-Initial Epinephrine Dose in Pediatric Out-of-Hospital Cardiac Arrest: A Simulation Trial.

OBJECTIVES: Survival rates for pediatric out-of-hospital cardiac arrest (POHCA) are low at around 10%. Paramedic services administer critical interventions including epinephrine. While typically administered via intravenous (IV) or intraosseous (IO) routes, obtaining these access points in out-of-hospital emergencies is challenging. We aimed to evaluate the time to first dose epinephrine and dosing accuracy in a simulated POHCA event. METHODS: Paramedics were randomized to one of three epinephrine administration routes: 1) IV or IO; 2) intramuscular (IM) by autoinjector; or 3) IM by needle/sy…

Use of endotracheal epinephrine for refractory unanticipated post-intubation bronchospasm.

Epinephrine is a commonly used agent for a variety of indications and clinical scenarios within the emergency department. It is commonly administered during cardiopulmonary arrest and serves as an option when there is a loss/lack of access. A female patient presented with respiratory depression secondary to opioid overdose, requiring emergent intubation. Shortly after intubation, she developed severe bronchospasm unresponsive to standard therapies including inhaled beta-agonists, intravenous corticosteroids, intramuscular epinephrine and sedation. As a last-resort intervention, 0.5 mg of epine…
