Blood cell aggregates are clinically useful biomarkers in a number of medical disorders. This protocol provides accurate and quantitative analysis of cell aggregates using a small volume of whole blood and imaging flow cytometry. We describe steps for sample collection, staining, and measurement. We then detail gating procedures and analysis of cell morphology. Sample preparation artifacts, activation, and morphological changes of cells are mitigated by omitting erythrocyte lysis and leukocyte isolation while maintaining high-throughput accurate imaging of leukocytes and platelets.

Platelet and erythrocyte agglutination is known to happen in vitro due to EDTA or temperature-induced cold antibodies. Leukocyte agglutination is far less common, and its etiology is not always known. The 2 cases presented herein are of low-grade B-cell lymphomas consistent with splenic marginal-zone lymphoma that presented with lymphocyte agglutination. In Case A, the lymphocyte aggregates were not resolved by warming the sample or by non-EDTA anticoagulation. Of the 229 patients with ulcerous bleeding operated on, 27 (11.8%) died. In 19 of them, the death was caused by hemorrhagic syndrome with severe disorders in rheologic blood properties. Rouleaux formation of washed erythrocytes was studied in 32 normal (HbAA) individuals and 30 sickle cell anaemia (HbSS) patients.
