INTRODUCTION: A pneumocele is an abnormal dilation of an air-containing sinus beyond the normal margins of bone, with associated bony thinning. A delayed ethmoid pneumocele after sinus surgery has not previously been reported.
METHODS: A case report of a patient with a delayed ethmoid pneumocele after sinus surgery. The diagnostic workup, operative approach, and postoperative results were evaluated.
RESULTS: A 57-year-old female with a history of endoscopic sinus surgery 15 years prior presented with right eye proptosis and severe orbital and facial pressure. A maxillofacial computed tomogra

Ethmoid sinus osteomas are rare, benign, encapsulated neoplasms of the paranasal region. They can lead to various complications such as sinusitis, orbital cellulitis, proptosis, and diplopia. The treatment protocol of these lesions changed remarkably as powered instrumentation of functional endoscopic sinus surgery evolved and otolaryngologists gained experience. Here, the authors report a 30-year-old female who was diagnosed with a giant right-sided ethmoid sinus osteoma and discuss diagnostic and treatment modalities of these lesions in consonance with the current literature.

The indication for decompression of the optic nerve after indirect trauma is made both by the ophthalmologist and the ENT-surgeon. The ENT-surgeon usually reaches and decompresses the optic canal by a transethmoidal-transsphenoidal route. The majority of authors prefer the transfacial approach to the ethmoid including resection of the crossing plane comprising the frontal process of the maxilla, the ethmoid, the lacrimal and the frontal bone. Hitherto we have knowledge of only one author utilising an endonasal approach to decompress the optic nerve. At the university hospital of Göttingen, the

Typically, the medial orbital wall contains an anterior ethmoidal foramen (EF) and a posterior EF, but may also have multiple EFs transmitting the arteries and nerves between the orbit and the anterior cranial fossa. The aim of this study is to determine a patient-friendly landmark of the medial orbital wall and to specify a precise location of the ethmoidal foramens (EF) in order to standardize certain anatomical marks as safe ethmoidal arteries. Orientation points on the anterior ethmoidal foramen (AEF), posterior ethmoidal foramen (PEF) and middle ethmoidal foramen (MEF) were investigated i

Central giant cell reparative granuloma is an infrequent, benign, proliferating lesion affecting the maxilla, mandible and, rarely, cranial bones. A 16-year-old girl presented with a 6-month history of recurrent nasal bleeding, a mass in the nose, difficulty in nasal breathing, a change in voice, and bilateral proptosis. Radiologically, an extensive ethmoidal mass was seen. Histologic examination revealed a central giant cell reparative granuloma. After endoscopic removal, the patient was symptom-free at the 12-month follow-up. The clinical picture of central giant cell reparative granuloma of

The structure of human ethmoid bone may be characterized not only by the labyrinthine complexity, but also by asymmetry, numerous variations and deformity. Another important specificity is the co-existence of evoluted and nonevoluted structures. These characteristics are probably the result of encephalization and insufficient adaptation due to rapid evolution since man descended to the ground from his arborial life. Kyphosis of the cranial base, displacement of the lamina lateralis anterosuperiorly and rotating of both orbits antero-medialy, restricted the space where the numerous ethmotubinat…

Endonasal dacryocystorhinostomy is technically more difficult than the classic method and apparently less rational. The nasal fossa is a variable and potentially dangerous anatomic region. The uncinate process "opens the door" to the middle meatus and covers, in almost all cases, the junction between the lacrymal bone and maxillary concha. A marker almost always found. Uncinate process resection is a natural way to access the lacrymal apparatus. Associated with transillumination of the lacrymal duct, uncinate process resection, is in our opinion, a reliable and, most importantly, reproducible

Five cases of pathologically proved psammomatoid ossifying fibromas of the sinonasal area are presented. All five cases were examined by CT and in three cases MR imaging was performed before and after injection of gadopentetate dimeglumine. The lesions were located in the sphenoethmoidal area and extended over the nasal cavity or orbit in four cases. In one case, the lesion occurred at the perpendicular plate of the ethmoid bone with preservation of the ethmoidal sinus. On CT, all the lesions were expansile and circumscribed by a thick bony wall. Internal septations of bone density (four cases…

The human nervus terminalis (terminal nerve) and the nerves to the vomeronasal organ (VNON) are both associated with the olfactory nerves and are of major interest to embryologists. However, there is still limited knowledge on their topographical anatomy in the nasal septum and on the number and distribution of ganglion cells along and near the cribriform plate of the ethmoid bone. We observed serial or semiserial sections of 30 fetuses at 7-18 weeks (crown rump length [CRL], 25-160 mm). Calretinin and S100 protein staining demonstrated not only the terminal nerve along the anterior edge of th…

The anatomy of the pterygopalatine fossa keeps a traditional level and is viewed as constant, even though a series of structures neighboring the fossa are known to present individual variations. We aimed to evaluate on 3D volume renderizations the anatomical variables of the pterygopalatine fossa, as related to the variable pneumatization patterns of the bones surrounding the fossa. The study was performed retrospectively on cone beam computed tomography (CBCT) scans of 100 patients. The pterygopalatine fossa was divided into an upper (orbital) and a lower (pterygomaxillary) floor; the medial

Pott's puffy tumor, a feature of osteomyelitis of the frontal bone, is a rare entity, especially in adults. Sir Percival Pott originally described this condition as a complication of trauma to the frontal bone. This is also a recognized complication of fronto-ethmoidal sinusitis. We present a rare case of Pott's puffy tumor caused by an insect bite presenting initially as a preseptal cellulitis and explore its pathogenesis and management.

Position of anterior ethmoidal artery and height of lateral lamella of cribriform plate (LLCP) represent critical variants in endoscopy. In 200 maxillofacial computed tomography scans of patients the position of anterior ethmoidal foramen (AEF) in relation to ethmoid roof was recorded. The height of LLCP was measured and classified according to Keros classification.Differences in AEF position and distribution of Keros types, and LLCP height according to sex were assessed through chi-square test (P < 0.05) and 1-way analysis of covariance (ANCOVA) test, respectively (P < 0.05). Differences in LL

A detailed understanding of the relationship between the ethmoidal sinus and the intracranial cavity is essential to prevent intracranial penetration during orbital surgery. The authors analyzed 10 postmortem orbits with their adjacent skull bases of 5 Asian cadavers (3 males and 2 females; mean age of 80 years at death). After removing all orbital contents, skull and brain, the medial orbital wall, ethmoidal cells, and ethmoidal roof were also removed. From the intracranial cavity view, the ethmoidal roof was situated just lateral to the cribriform plate. From the orbital view, the location o

Paranasal sinus mucocele is the cystic formation lined by inflammatory changed mucoperiosteum. This is slow-growing pathology with a tendency to bone erosion. The symptoms are dependent on the direction of the penetration, and are as follows: increasing headaches, deformations of frontal or orbital region, ophthalmic manifestations such as lacrimation, decreased visual acuity, exophthalmos, ocular movement limitation, diplopia. The most common location of the mucocele is fronto-ethmoid region. The most useful diagnostic tests are a magnetic resonance imaging (MRI) and a computed tomography (CT

We report the case of a 28-year-old man presenting with a medial orbital wall trapdoor fracture with a missing medial rectus muscle. We believe this to be the first case report of an adult medial orbital wall trapdoor fracture. Trapdoor fractures most commonly occur in the pediatric population, and those involving the medial orbital wall generally occur in areas with less developed ethmoid air cells. Since the present case followed neither pattern, a different injury mechanism was considered. The ethmoid air cells in this case were well developed, which may have played an important role in the

OBJECTIVE: Naso-orbito-ethmoidal fractures (NOE) fractures are uncommon but critical injuries. This review aims to investigate the patient factors, procedural factors, and postoperative outcomes associated with the surgical management of NOE fractures.
STUDY DESIGN: PubMed and Scopus databases were systematically searched between 1993 and 2023 using the search strategy "(naso-orbito-ethmoidal OR nasoethmoid OR nasoorbitoethmoidal) AND fracture." Articles reporting clinical studies investigating the surgical management of NOE fractures were included. Articles that were duplicates, non-English,
