A simple technique of orbital implantation of two hollow spheres at enucleation or evisceration is described. The procedure sufficiently replaces lost volume in the enucleated or eviscerated orbit, facilitates prosthesis motility, and diminishes simulated enophthalmos and superior sulcus depression. The risk of implant migration or extrusion is not increased.

PURPOSE: To observe the effects of improved silica ball implantation enclosed by autologous reversal sclera.
METHODS: After the enucleation of eye balls, we reversed, cleared the autologous sclera, enclosed the silica ball and implanted it in the operative eye.
RESULTS: 30 cases with enucleation were implanted the prosthetic eyes. The follow-up period was 6 months to 3 years. The prosthetic eyes had the motility of 10 degrees to 25 degrees and the empty socket syndrome was effectively prevented.

An 18-year-old man with an unsuspected carotid-cavernous fistula underwent enucleation as the result of a severe localized perforating ocular trauma. Potentially lethal bleeding occurred that was difficult to control. Auscultation of the orbit should be performed in all cases before eye enucleation to avoid the possibility of overlooking a carotid cavernous fistula.

To determine the long-term effects of cyclocryotherapy, we conducted a follow-up with a mean of 24.9 +/- 13.9 months in 50 eyes of 46 patients with neovascular glaucoma. A patient with spindle melanoma involving the iris and ciliary body refused enucleation at the time of diagnosis in 1933. The eye developed painful glaucoma 13 years later and was enucleated. Histopathologic examination indicated spread of the tumor intrasclerally and subconjunctivally. The patient is still alive without evidence of metastases, 32 years after enucleation and 45 years after initial diagnosis.
