Seeing Fear: It's All in the Eyes?

Is an amygdala necessary to experience and perceive fear? Intriguing evidence comes from patient S.M. who lost her left and right amygdalae to disease. Initial testing suggested that S.M.'s most defining symptom was an inability to recognize fear in other people's facial expressions. A fascinating paper by Adolphs and colleagues in 2005 examined one potential mechanism for this impairment: a failure to spontaneously attend to widened eyes, the most distinctive physical feature portrayed in symbolic fear expressions. This study helped to invigorate debates about the brain basis of fear and pave…

Generalization of Fear to Respiratory Sensations.

Interoceptive fear conditioning (IFC), fear generalization and a lack of safety learning have all been hypothesized to play a role in the pathogenesis of panic disorder, but have never been examined in a single paradigm. The present study aims to investigate whether healthy participants (N=43) can learn both fear and safety to an interoceptive sensation, and whether such learning generalizes to other, similar sensations. Two intensities of inspiratory breathing impairment (induced by two pressure threshold loads of 6 and 25 cm H2O) served as interoceptive conditional stimuli (CSs) in a differe…

The role of fear of pain in headache.

BACKGROUND: Recurrent headache sufferers are often fearful of pain, which disrupts thought processes, interferes with daily activities, and may maintain headache-related disability through avoidance and associated negative reinforcement. OBJECTIVE: The aim of this cross-sectional study was to (1) examine differences in fear of pain (FOP) between headache sufferers and non-headache controls; (2) examine differences in FOP across primary headache diagnostic groups; (3) assess the extent to which FOP predicts headache variables (eg., severity, frequency, disability); and (4) determine whether FO…

Verapamil in infants: an exaggerated fear?

The use of intravenous verapamil for tachyarrhythmia in infants is widely considered contraindicated due to the perceived risk of hemodynamic collapse after administration. This article reviews the relatively limited evidence that led to this well-known contraindication and highlights the interesting process by which medical practice may evolve in the absence of persuasive science.

Examining the nature of fear of flying.

BACKGROUND: The nature of fear of flying (FOF) is not well understood. It is commonly assumed to be a fear learned from flight-specific experiences. However, existing literature suggests that FOF is a manifestation of fears of other stimuli (e.g., heights) embedded in the flying situation, but not specific to it. This study compared the level of prediction of FOF attained from flight-specific conditioning experiences (specifically, classical conditioning experiences in direct, observational, and verbal modes) with the level of prediction attained from flight-embedded fears. METHODS: There wer…

Desynchrony of fear in phobic exposure.

Intuitively, phobic exposure would seem to be a very stressful experience. However, it is not clear whether the characteristic feature of a classic stress response, activation of the hypothalamic-pituitary-adrenal (HPA) axis, is present in phobic fear. Some instances of phobic fear have been found to be accompanied by robust increases in cortisol, whereas in other instances a dissociation between subjective-behavioural arousal and the HPA-axis has been found. The latter is referred to as desynchrony of fear. The aim of the current study was to test the hypothesis that phobic fear is similar to…

Approach and avoidance in fear of spiders.

We examined attitudes towards spiders by employing an Approach-Avoidance Task, in which participants respond to pictures by pulling a joystick towards themselves or by pushing it away from themselves. For spider fearfuls, this stimulus-response assignment is either compatible (push spiders away) or incompatible (pull spiders closer). Specific compatibility effects were found: compared to non-anxious controls and control pictures, highly spider fearful participants responded to spider pictures more quickly by pushing than by pulling, even when picture contents was task-irrelevant. Moreover, com…

[Non-fearful panic disorder].

Panic disorder is among the most prevalent mental health disorders. Among other reasons, it is an important disorder due to its influence on all other medical professionals that these patients consult repeatedly. One of the major reasons for sub-optimal diagnosis is the different and less familiar presentations of the disorder. This article will focus on the "non-fearful" variant. Four case reports are described with prevalent presentations of the disorder and a summary of recommendations for better diagnosis of the disorder.

Fears and phobias in people with diabetes.

Phobic disorders are more common in people with diabetes than in the general population. The management of phobic disorders in patients with diabetes, particularly when associated with a fear of hypoglycaemia, is especially challenging and requires close collaboration between psychological medicine and diabetes teams. Difficulty in distinguishing symptoms of anxiety from those of hypoglycaemia, and the real dangers associated with hypoglycaemia, complicate the delivery of psychological interventions that are used routinely in the treatment of phobias. Avoidance of hypoglycaemia can lead to det…

Illness fears in the general population.

OBJECTIVES: Because relatively little is known about illness fears, we sought to estimate the prevalence, risk factors, and morbidity associated with such fears in the community. METHODS: We conducted a brief telephone survey of persons aged 40 to 65 years from randomly selected households in the Johnson County, Iowa, area. Respondents were asked whether a series of illness and medical care items made them no more nervous, somewhat more nervous, or much more nervous than other people. Those who reported more discomfort were asked to what extent this interfered with medical care or caused impa…

Evaluating interventions for fear of contagion.

Fear of contagion, an anxious response to the perceived threat of catching HIV, has been shown to lessen with contact with persons with AIDS. Two different continuing educational offerings presented a panel of persons living with HIV as an intervention to decrease fear of contagion. Presession and postsession questionnaires demonstrated a significant decrease in levels of fear in the one-day program (n = 39), but not in the six-week course (n = 14). Willingness to care for those with HIV was unchanged in either offering. The study provides evidence for the effectiveness of exposure to persons …
