Long-term outcome in Felty's syndrome.

Felty's syndrome has again been shown to be a severe form of systemic rheumatoid disease characterised by severe joint involvement, many extra-articular features, and a high incidence of infection. In addition we have shown that splenectomy was not protective for infections and in fact may on occasion contributed to infection. Furthermore, although most patients had an increase in white blood cell count after splenectomy, 50% of patients without splenectomy showed a similar increase in white blood cell counts at follow-up. Mortality in Felty's syndrome was high, with infection being the main c

Felty's syndrome. Effect of lithium on granulopoiesis.

The effect of lithium carbonate upon granulopoiesis was studied in eight patients with Felty's syndrome. Absolute granulocyte counts increased in all patients receiving 900 mg lithium carbonate daily for six weeks. Increased urinary and serum granulocyte colony-stimulating activity was observed in all patients during lithium therapy. A casual relationship between increases in colony-stimulating activity and increased granulocytes is postulated.

Non-Articular Felty Syndrome Refractory to Granulocyte Colony-Stimulating Factor Therapy.

Felty syndrome (FS), an uncommon manifestation seen in patients with rheumatoid arthritis (RA), usually presents as a triad of erosive arthritis, splenomegaly, and neutropenia. It is extremely rare for RA to present as FS or develop after initially presenting as neutropenia and splenomegaly. In this report, we describe a case of a 55-year-old woman who initially presented with fever and vaginal pain. Her sepsis workup revealed genital herpes in the setting of leukopenia, with an incidental finding of splenomegaly on imaging. The patient was managed with filgrastim and valacyclovir. Two weeks l…

Parenteral gold therapy in the Felty syndrome. Experience with 20 patients.

Most of the patients with the Felty syndrome suffer from such complications as fevers, infections, cutaneous ulcers, and vasculitis. Unfortunately, there are no therapeutic interventions that are predictably beneficial. We report our experience with 20 patients who received parenteral gold therapy for 2 to 114 months (mean, 23.6 months). All had complications of the Felty syndrome. On parenteral gold therapy, 60% had a complete response, 20% had a partial response, and 20% were unresponsive by preselected criteria. No serious complications were encountered. We think that parenteral gold therap…

Case Report: Immunotherapy-induced Felty syndrome in a patient with metastatic melanoma.

Immune checkpoint inhibitors (ICIs) are critical in the modern treatment of advanced melanoma but can provoke immune-related adverse events (irAEs) with increased risk in patients having pre-existing autoimmune conditions. We present a case of Felty syndrome (FS) recurrence precipitated by a single dose of combination nivolumab plus relatlimab immunotherapy in a 71-year-old man with metastatic melanoma and a history of seropositive rheumatoid arthritis (RA) diagnosed several years earlier in the context of spontaneous FS. Following ICI, he presented with a neutrophil count of 0.0 cells/µL but …

Presentation of three cases followed up with a diagnosis of Felty syndrome.

An effective treatment strategy for Felty syndrome (FS) has not been developed so far. In this article, three cases with FS, who responded to different treatment modalities, have been presented. Case 1 was a 52-year-old male patient who initially received methotrexate, and then, he was switched to granulocyte colony-stimulating factor (G-CSF) and cyclosporine treatment when his neutropenia was further deteriorated. The patient needed monthly doses of G-CSF for nearly 6 months, and his steroid dose was increased. Afterwards, his neutropenia improved with cyclosporine, methotrexate, and hydroxyc…

Novel genetic insights: Mendelian randomization implicates Felty Syndrome in atrial fibrillation risk.

OBJECTIVES: Atrial fibrillation (AF) is a common and increasing arrhythmia posing major public health challenges. While management has improved, long-term efficacy and safety remain limited. Felty Syndrome, a severe rheumatoid arthritis (RA) subtype with splenomegaly and neutropenia, offers a chance to examine the autoimmune inflammation-AF link. METHOD: We employed five Mendelian randomization (MR) approaches using genome-wide association study (GWAS) data to investigate whether Felty Syndrome causes AF. RESULTS: We analyzed data from the FinnGen biobank and a separate AF GWAS, selecting 17…
