[Translated article] Fixation of adult femoral neck fractures: Retrospective comparison between cannulated screws and femoral neck system (FNS).

OBJECTIVE: The FNS System DePuy Synthes® (EEUU, 2018) represents a recent alternative treatment for the fixation of femoral neck fractures, providing biomechanical advantages with respect to cannulated screws (3 CS). The objective of this study is to compare the clinical results of both fixation methods. METHOD: A retrospective collection of the 36 subcapital fractures treated with the FNS system was carried out compared with a retrospective search of the last 35 patients treated with 3 CS. Age, sex, fracture pattern, delay until the intervention, length of intervention, hospital length stay,…

Mechanical effects of sagittal variations on Pauwels type III femoral neck fractures treated with Femoral Neck System(FNS).

BACKGROUND: The spatial position of internal fixation play a role in determining the stability of internal fixations, both in clinical practice and research. Researchers have examined the stability of FNS (Femoral neck system) in the presence of coronal plane changes. In our knowledge, due to the biomechanical limitations of the specimens, there are no mechanical studies on the sagittal variation of FNS. This study aimed to investigate the biomechanical behavior of sagittal variations on Pauwels type III femoral neck fractures treated with FNS through finite element analysis. METHODS: Finite …

Finite element analysis of long proximal femoral bionic nail (PFBN) fixation for femoral shaft fractures combined with ipsilateral femoral neck fractures.

OBJECTIVE: Femoral reconstruction nails (RN) and three cannulated screws combined with a plate (TCP) are commonly employed for the treatment of ipsilateral femoral neck and shaft fractures (IFNSF). However, these fixation strategies are associated with a considerable incidence of postoperative complications, reflecting the biomechanical challenges of this fracture pattern. Therefore, this study aims to compare the biomechanical performance of a novel long proximal femoral bionic nail (PFBN) with these conventional fixation methods for the treatment of this complex fracture pattern. METHODS: C

[Femoral head necrosis after femoral neck fracture-A lifelong burden].

Posttraumatic femoral head necrosis after a femoral neck fracture in children and adolescents is a rare but severe complication. If this occurs in older patients a total hip arthroplasty (THA) is mostly indicated. In children and adolescents the primary goal of treatment is preservation of the hip joint. There is no standard treatment or generally applicable treatment recommendation. Various measures for treating this rare entity are discussed in the literature. In addition to conservative measures, retrograde drilling with or without cancellous bone grafting, joint distraction or grafting mea

Spontaneous subcapital femoral neck fracture complicating osteonecrosis of femoral head.

Spontaneous subcapital fracture (SSF) of femoral neck in pre-existent osteonecrosis of femoral head (ONFH) is a rare presentation. Only a few cases have been reported so far and majority of them have been reported to have unilateral hip involvement. We retrospectively reviewed clinical-radiological data of 10 patients (12 hips) with SSF complicating ONFH. All of them underwent uncemented total hip arthroplasty. All the available publications in the English language based medical literature were critically reviewed and results were summarized. The median age of presentation was 32 years (range

Slipped capital femoral epiphysis after treatment of femoral neck fracture.

Slipped capital femoral epiphysis (SCFE) in children after treatment of femoral neck fracture is a very rare condition. This complication should be recognized promptly and treated urgently. The risk of development of this complication can be minimized by anatomical reduction of the fracture and stable internal fixation of the fracture. Five years old male child sustained right sided femur neck fracture and was treated with closed reduction and Hip spica cast application. The fracture healed with a varus deformity. After 7 months, he developed slip of femoral epiphysis with a coxa vara deformit

[Treatment of intracapsular femoral neck fractures with the use of a proximal femoral nail].

PURPOSE OF THE STUDY: The management of intracapsular femoral neck fractures remains an unsolved issue of the present-day trauma surgery of the musculoskeletal system. These fractures are conventionally treated with the use of spongious screws or a dynamic hip screw combined with an antirotational spongious screw. The aim of this study was to evaluate the outcomes of surgical therapy using a different type of implant (proximal femoral nail) for intracapsular femoral neck fractures. MATERIAL: From January 2000 to March 2004, intracapsular fractures were treated with the use of a proximal femor…

Pathological fracture of the femoral neck as the first manifestation of osteonecrosis of the femoral head.

We describe two cases of pathological fracture of the femoral neck occurring as the first manifestation of osteonecrosis of the femoral head (ONFH). No abnormal findings suggestive of ONFH were identified on the radiographs for either of the patients, and the fractures occurred like spontaneous fractures without any trauma or unusually increased activity. The patients' medical history, age, and good bone quality suggested ONFH as a possible underlying cause of the fractures. If we had not suspected ONFH as a predisposing condition, these minimally displaced fractures might have been fixed inte

Treatment of a femoral neck fracture combined with ipsilateral femoral head and intertrochanteric fractures: A case report.

BACKGROUND: This article presents a rare case of a complex hip fracture involving the ipsilateral femoral neck, trochanter, and femoral head, that was accompanied by hip dislocation. Currently, there is no established standard treatment method for this specific type of fracture. Therefore, it is crucial to comprehensively consider factors such as patient age, fracture type, and degree of displacement to achieve a successful outcome. CASE SUMMARY: A 38-year-old man sustained a comminuted fracture of his right hip as a result of a car accident. The injuries included a fracture of the femoral he

Transepiphyseal fracture of the femoral neck with dislocation of the femoral head and fracture of the posterior column of the acetabulum in a child.

We describe a 15-year-old boy with a posterior dislocation of the hip, fracture of the posterior column of the acetabulum and separation of the femoral capital epiphysis. To our knowledge no previous case in a child has been reported. Such high-energy injuries are extremely rare, and a poor outcome is expected. We advocate early referral to a specialised tertiary centre, and the use of a modification of Delbet's classification to reflect the complexity and displacement which may occur with this injury.

Effects of Cerclage Cabling on Preventing Periprosthetic Femoral Fractures When Using Cementless Stems for Unstable Femoral Neck Fractures.

BACKGROUND: Periprosthetic femoral fractures (PFFs) are serious complications in hip arthroplasty for femoral neck fractures. The rates of intraoperative (iPFFs) and postoperative PFFs (pPFFs) are higher in cementless stem cases than in cemented cases. This study aimed to investigate the effects of cerclage cabling on PFF prevention in cementless arthroplasty for femoral neck fractures. METHODS: This retrospective study included 329 consecutive patients who underwent hip arthroplasty using a cementless stem for femoral neck fractures. A total of 159 and 170 patients were in the non-cabling an…

Surgical treatment and prognosis of acetabular fractures associated with ipsilateral femoral neck fractures.

Combined ipsilateral acetabular and femoral neck fractures are the result of high-energy trauma. Satisfactory treatment for this injury pattern remains a challenge, since traditional open reduction and internal fixation (ORIF) is always accompanied by a high prevalence of posttraumatic arthritis and avascular necrosis of the femoral head. Eight of 502 acetabular fractures from 1990 to 2008 were diagnosed with combined ipsilateral femoral neck fracture, in which 5 patients' fractures were associated with hip dislocation. These patients were injured from falls, traffic accidents, or crushing acc…

Finite element analysis of femoral neck system in the treatment of Pauwels type III femoral neck fracture.

The optimal treatment strategy for femoral neck fractures remained controversial, especially the Pauwels type III femoral neck fracture of young patients was a challenge. Femoral neck system (FNS) was a newly developed internal fixation for treating femoral neck fracture and this study aimed to compare the biomechanical advantages and disadvantages between FNS and 3 cannulated configuration screws (CCS) with or without an additional medial buttress plate (MBP). In this study, Pauwels type III femoral neck fracture model with an angle of 70° was constructed and 3 different fixation models, FNS,…
