Dissociation of the anorexic effects of fenfluramine and amphetamine following intrahypothalamic injection.

Injections of amphetamine and fenfluramine directly into the hypothalamus produced changes in food intake dependent upon the site of injection. No significant modulation occurred following injections into the ventromedial nucleus. In the lateral hypothalamus both drugs gave rise to an overall depression of feeding behaviour but the time-course of action for the drugs differed and there was a high negative correlation between the action of the two drugs within individual animals. These findings provide further evidence for the belief that fenfluramine and amphetamine exert their anorexic action…

Pulmonary arterial hypertension associated with fenfluramine exposure: report of 109 cases.

The aim of the present study was to describe a large cohort of fenfluramine-associated pulmonary arterial hypertension (fen-PAH) and its possible prognostic markers. The records of all patients with a diagnosis of fen-PAH evaluated at the present authors' centre from 1986-2004 were retrospectively studied. Baseline clinical and haemodynamic data were collected, as well as survival times. The median duration of fenfluramine exposure was 6 months, with a median of 4.5 yrs between exposure and onset of symptoms. Nine (22.5%) out of 40 patients evaluated resulted positive for the presence of germl…

Fenfluramine-induced pulmonary vasoconstriction: role of serotonin receptors and potassium channels.

The anorexic agent fenfluramine considerably increases the risk of primary pulmonary hypertension. The mechanism of this effect is unknown. The appetite-reducing action of fenfluramine is mediated by its interaction with the metabolism of serotonin [5-hydroxytryptamine (5-HT)] in the brain. We tested the hypothesis that the pulmonary vasoconstrictive action of fenfluramine is at least in part mediated by 5-HT receptor activation. In addition, we sought to determine whether pharmacological reduction of voltage-gated potassium (K(V)) channel activity would potentiate the pulmonary vascular react…

Efficacy and tolerability of fenfluramine with concomitant potassium bromide in patients with Dravet syndrome.

OBJECTIVE: To assess the efficacy and tolerability of fenfluramine (FFA) with concomitant potassium bromide (BR) in patients with Dravet syndrome (DS). METHODS: This multicenter retrospective study, conducted within the German compassionate use program, analyzed BR doses and serum levels before and after FFA initiation, adverse events (AEs), seizure reduction, and symptoms changes using the Clinical Global Impression of Change (CGIC) scale. Timepoints were defined as T0 (baseline), T1 (FFA initiation), T2 and T3 (first and second BR level measurement after FFA initiation). RESULTS: Twenty-tw…

Efficacy of Low-Dose Fenfluramine on Adult Patients With Lennox-Gastaut Syndrome.

Purpose Fenfluramine has received approval for treating seizures in individuals with Lennox-Gastaut syndrome (LGS). Nonetheless, Phase III trials mainly focused on children, leaving limited information regarding its effectiveness and safety in adults. This case series aimed to elucidate these factors in adult patients. Methods This case series examined the clinical progression of adult patients diagnosed with LGS who received fenfluramine at our clinic following its approval in Japan in March 2024, extending through February 2025. Fenfluramine was administered to those experiencing drop attack…

Identification of fenfluramine adulteration in slimming foods using a highly sensitive immunoassay.

Fenfluramine, an appetite suppressant used for weight loss, can cause significant harm if overdosed. The unauthorized addition of fenfluramine to slimming foods has raised concerns. Currently, no rapid screening method is available for the quick detection of fenfluramine in the field. This study proposes six new haptens with varying spacer arm lengths and hydrophobicity, promising to elicit antibodies capable of being highly specific and strongly binding to fenfluramine. The study found that highly hydrophobic haptens with long spacer arms favored the generation of highly sensitive antibodies.…

The anorectic effect of fenfluramine is influenced by sex and stage of the estrous cycle in rats.

The controls of food intake differ in male and female rats. Daily food intake is typically greater in male rats, relative to female rats, and a decrease in food intake, coincident with the estrous stage of the ovarian reproductive cycle, is well documented in female rats. This estrous-related decrease in food intake has been attributed to a transient increase in the female rat's sensitivity to satiety signals generated during feeding bouts. Here, we investigated whether sex or stage of the estrous cycle modulate the satiety signal generated by fenfluramine, a potent serotonin (5-HT) releasing …

Effects on serotonin in rat hypothalamus of D-fenfluramine, aminorex, phentermine and fluoxetine.

Hypothalamic 5-HT (serotonin) regulates food intake, energy expenditure and bodyweight. Using in vivo microdialysis, we determined the effects of various anorectic drugs on hypothalamic extracellular 5-HT levels during the dark phase when rats predominantly feed. Phentermine and aminorex, which were originally considered to be catecholaminergic drugs, markedly increased 5-HT efflux in rat hypothalamus. Their actions were less profound than D-fenfluramine, but considerably greater than that of the selective 5-HT reuptake inhibitor, fluoxetine. This suggests that enhanced hypothalamic 5-HT funct…

Valvular abnormalities and cardiovascular status following exposure to dexfenfluramine or phentermine/fenfluramine.

CONTEXT: Fenfluramine and dexfenfluramine were voluntarily withdrawn from the market in September 1997 because of reports of an association with heart valve abnormalities. Studies have been limited by lack of comparison with untreated controls. OBJECTIVE: To evaluate cardiovascular status and the prevalence of valvular abnormalities, as assessed by clinical cardiovascular parameters and echocardiography, in patients treated for obesity with dexfenfluramine or phentermine/fenfluramine. DESIGN: Reader-blinded controlled study completed in February 1998. SETTING AND PARTICIPANTS: Twenty-five c…

Synergistic anorectic effect of dehydroepiandrosterone and d-Fenfluramine on the obese Zucker rat.

Fasted obese, female Zucker rats accustomed to eating a single high fat meal within 1 h a day were treated with intraperitoneal injections of dehydroepiandrosterone (DHEA) and dextrofenfluramine (d-fen), either individually or in combination. Caloric intake was measured over a 1-h period 2 h after drug administration, and results compared to that of vehicle-treated controls. At 50 mg/kg body weight, DHEA did not affect food intake. At doses of < or = 2 mg/kg d-fen did not affect food intake. Together, however, DHEA 50 mg/kg and d-fen < or = 2 mg/kg significantly decreased food intake. At doses…

d-fenfluramine's effects on normal ingestion assessed with taste reactivity measures.

The effects of d-fenfluramine on intake and on hedonic responses to taste stimuli in rats were investigated using a modified taste reactivity paradigm. Subjects (n = 15) were first trained to consume a 3% sucrose solution. They were then pretreated with d-fenfluramine (0.3-3.0 mg/kg, i.p.), and tested with access to either 3% sucrose, or a 0.01% quinine HCl solution. In the modified taste reactivity test, chronic oral cannulation was not used; instead, taste reactivity measures were scored during periods of noningestion in a voluntary intake test. d-Fenfluramine reliably reduced both sucrose a…
