Children with fetal alcohol spectrum disorders: problem behaviors and sensory processing.

OBJECTIVE: This study describes the sensory-processing and behavior profiles of a clinic-referred sample of children with fetal alcohol spectrum disorders (FASD) and examines the relationship between sensory processing and behavior. METHODS: Outcomes on the Short Sensory Profile (SSP) and Child Behavior Checklist (CBCL) for 44 children, ages 5 to 10 years, were assessed and compared using retrospective data analysis. RESULTS: A high proportion of the children demonstrated deficits in sensory processing and problem behaviors as measured by the SSP and the CBCL. Moreover, the correlation betwe…

Fetal alcohol spectrum disorders and their persisting sequelae in adult life.

INTRODUCTION: The present review describes the different variants of Fetal Alcohol Spectrum Disorders (FASD) as a consequence of maternal alcohol abuse during pregnancy. According to international findings, some 3000 to 4000 affected newborns may be expected annually in Germany. Detection of these individuals in clinical practice is insufficient. METHOD: Selective review of the literature and report of own findings based on longitudinal studies. RESULTS: Key features of FASD are short stature, microcephaly, various dysmorphic features, in severe cases amongst others consisting of congenital …

Genetic polymorphisms: impact on the risk of fetal alcohol spectrum disorders.

Clinical reports on monozygotic and dizygotic twins provided the initial evidence for the involvement of genetic factors in risk vulnerability for fetal alcohol spectrum disorders (FASD) including fetal alcohol syndrome (FAS). Research with selectively bred and inbred rodents, genetic crosses of these lines and strains, and embryo culture studies have further clarified the role of both maternal and fetal genetics in the development of FASD. Research to identify specific polymorphisms contributing to FASD is still at an early stage. To date, polymorphisms of only one of the genes for the alcoho…

Sexual dimorphism in lung transcriptomic adaptations in fetal alcohol spectrum disorders.

Current fetal alcohol spectrum disorders (FASD) studies primarily focus on alcohol's actions on the fetal brain although respiratory infections are a leading cause of morbidity/mortality in newborns. The limited studies examining the pulmonary adaptations in FASD demonstrate decreased surfactant protein A and alveolar macrophage phagocytosis, impaired differentiation, and increased risk of Group B streptococcal pneumonia with no study examining sexual dimorphism in adaptations. We hypothesized that developmental alcohol exposure in pregnancy will lead to sexually dimorphic fetal lung morpholog…

Fetal alcohol spectrum disorders prevention and clinical guidelines research-workshop report.

It is estimated that up to 1 in 20 people in the United States are affected by fetal alcohol spectrum disorders (FASD), an array of cognitive, emotional, physical and social disorders caused by exposure to alcohol during prenatal development. Common diagnoses encompassed within FASD include mood and behavioral disorders, memory and central nervous system deficits, attention-deficit/hyperactivity disorder (ADHD), slow growth and low body weight. While this condition affects a broad range of individuals and families, it is of particular concern in the military community, where cultural factors i…

Neuroinflammation in fetal alcohol spectrum disorders and related novel therapeutic approaches.

Fetal alcohol spectrum disorders (FASD) is an umbrella term to describe the neurological effects of prenatal alcohol exposure (PAE). It has been extensively characterized that PAE causes cell proliferation disruption, heterotopias, and malformations in various brain regions and there is increasing evidence that neuroinflammation is responsible for some of these neurotoxic effects. Despite evidence of its importance, neuroinflammation is not usually considered at diagnosis or treatment for FASD. Here, we discuss the literature regarding anti- inflammatory drugs and nutraceuticals, which hold pr…

Fetal Alcohol Spectrum Disorders: Awareness to Insight in Just 50 Years.

This article is part of a Festschrift commemorating the 50th anniversary of the National Institute on Alcohol Abuse and Alcoholism (NIAAA). Established in 1970, first as part of the National Institute of Mental Health and later as an independent institute of the National Institutes of Health, NIAAA today is the world's largest funding agency for alcohol research. In addition to its own intramural research program, NIAAA supports the entire spectrum of innovative basic, translational, and clinical research to advance the diagnosis, prevention, and treatment of alcohol use disorder and alcohol-r

Combining neuroanatomical features to support diagnosis of fetal alcohol spectrum disorders.

AIM: To identify easily accessible neuroanatomical abnormalities useful for diagnosing fetal alcohol spectrum disorders (FASD) in fetal alcohol syndrome (FAS) but more importantly for the probabilistic diagnosis of non-syndromic forms (NS-FASD). METHOD: We retrospectively collected monocentric data from 52 individuals with FAS, 37 with NS-FASD, and 94 paired typically developing individuals (6-20 years, 99 males, 84 females). On brain T1-weighted magnetic resonance imaging, we measured brain size, corpus callosum length and thicknesses, vermis height, then evaluated vermis foliation (Likert s…

Psychopharmacological Treatments in Children with Fetal Alcohol Spectrum Disorders: A Review.

Psychiatric symptoms in children with Fetal Alcohol Spectrum Disorders (FASD) present with high prevalence and morbidity, often across symptom domains, e.g. ADHD-like symptoms, emotional dysregulation and sleep problems. Polypharmacy is often used, but no empirically-based guidelines exist regarding optimal treatment for these children. Moreover, stimulant use in these children is controversial as their responsiveness may be different due to altered neural circuitry associated with prenatal alcohol exposure. The objective of this review is to give an overview of existing data on pharmacologica…

Neuroinflammatory contribution of microglia and astrocytes in fetal alcohol spectrum disorders.

Ethanol exposure to the fetus during pregnancy can result in fetal alcohol spectrum disorders (FASD). These disorders vary in severity, can affect multiple organ systems, and can lead to lifelong disabilities. Damage to the central nervous system (CNS) is common in FASD, and can result in altered behavior and cognition. The incidence of FASD is alarmingly high, resulting in significant personal and societal costs. There are no cures for FASD. Alcohol can directly alter the function of neurons in the developing CNS. In addition, ethanol can alter the function of CNS glial cells including microg…

Exploring the complexity of intellectual disability in fetal alcohol spectrum disorders.

Brain development in mammals is long lasting. It begins early during embryonic growth and is finalized in early adulthood. This progression represents a delicate choreography of molecular, cellular, and physiological processes initiated and directed by the fetal genotype in close interaction with environment. Not surprisingly, most aberrations in brain functioning including intellectual disability (ID) are attributed to either gene(s), or environment or the interaction of the two. The ensuing complexity has made the assessment of this choreography, ever challenging. A model to assess this comp

Source monitoring in children with and without fetal alcohol spectrum disorders.

OBJECTIVES: Deficits in memory are well-documented in children with fetal alcohol spectrum disorders (FASD); however, one aspect of memory not yet studied in children with FASD is source monitoring. This study examined overall source monitoring ability and performance profiles of children with FASD compared to controls. METHOD: Participants included 19 children with FASD and 38 typically developing children (aged 6-12 years). Children were presented with auditory word lists and were required to recall the source of words for reality, external, and internal source monitoring tasks. RESULTS: C…

Further development of a neurobehavioral profile of fetal alcohol spectrum disorders.

BACKGROUND: Heavy prenatal alcohol exposure (AE) results in a broad array of neurobehavioral deficits. Recent research has focused on identification of a neurobehavioral profile or profiles that will improve the identification of children affected by AE. This study aimed to build on our preliminary neurobehavioral profile to improve classification accuracy and test the specificity of the resulting profile in an alternate clinical group. METHODS: A standardized neuropsychological test battery was administered to 3 groups of children: subjects with AE (n = 209), typically developing controls (C

Behavioral interventions for children and adolescents with fetal alcohol spectrum disorders.

Exposure to alcohol in utero is considered to be a leading cause of developmental disabilities of known causation. The most severe consequence of such exposure, fetal alcohol syndrome (FAS), is characterized by a distinct constellation of facial anomalies, growth retardation, and central nervous system dysfunction. Both animal and human studies, however, suggest that there may be considerable variability in the manifestations of in utero alcohol exposure across individuals, and, consequently, the term fetal alcohol spectrum disorders (FASD) has come into usage to reflect the entire continuum o…

Preventing fetal alcohol spectrum disorders: the role of protection motivation theory.

This article examines health communication campaigns aimed at preventing alcohol consumption among women who are pregnant or attempting to become pregnant. Relevant communication materials were gathered and a qualitative review was conducted. A majority of the campaigns followed the tenets of protection motivation theory by focusing on the threat variables of severity and vulnerability, as well as emphasizing response efficacy. Few campaigns focused on costs or self-efficacy. Future fetal alcohol spectrum disorders prevention initiatives should attempt to reduce perceived costs, as well as inc…
