PURPOSE: Awake flexible bronchoscope-guided intubation is challenging in patients with extremely limited mouth opening (when there is inadequate space for an oropharyngeal airway), especially when nasal access is unavailable. Alternatives include awake front of neck access, which is an invasive procedure and not suitable for elective surgery. We present a novel technique to facilitate flexible bronchoscope-guided oral intubation in these patients.

Approaches for noninvasive bone quality assessment are of great clinical need, particularly in individuals that require close monitoring of disease progression. X-ray measurements are standard approaches to assess bone quality; however, they have several disadvantages. Here, a nonionizing approach for noninvasive assessment of the second metacarpal bone based on near infrared (NIR) spectroscopy was investigated. Transcutaneous bone signal detection was experimentally confirmed with cadaveric hand data, and Monte Carlo modeling further indicated that 50% of the measured signals arise from bone.

Congenital bilateral vocal cord paralysis (CBVCP) is a rare but potentially life-threatening condition and awareness of the condition is necessary to ensure early diagnosis and treatment. This case describes a 25-month-old boy suffering from CBVCP. The main symptoms at birth were inspiratory stridor combined with a normal voice and feeding problems. The difficulties in achieving the right diagnosis are demonstrated, and the treatment so far, including tracheotomy and a feeding tube, is outlined. The importance of fibre optic laryngoscopy in both diagnosis and control is stressed.

Fiberoptic-compatible oral airways (FCOAs) combine the simplicity and benefits that traditional oral airways provide, with the advantage of mechanically guiding fiberoptic intubation. This review examines and compares the salient properties of these devices. Of note, the clinician should pay particular attention to the location and depth of the channel. FCOAs, with an anterior channel, may be advantageous for use with difficult intubations arising from an excessively anterior-oriented glottis, whereas a channel with excessive depth may hinder the localization of a glottis which is off-midline.

Airway management is a major concern in Robin sequence, where micrognathia, glossoptosis and a cleft of the soft palate exist. The management of an infant with a potentially difficult airway is discussed where tracheal intubation was aided by the use of a 2.2-mm fibrescope mounted with a 3-mm endotracheal tube passed through a size 1 laryngeal mask airway.

We conducted a randomized, controlled trial to compare fiberoptic phototherapy with conventional phototherapy in healthy jaundiced newborns with birth weights greater than 2500 g. A chromium-sensitized, and thulium and holmium-doped YAG laser (THC:YAG laser) was used to create bilateral limbal sclerostomies in six Dutch pigmented rabbits. A study of 374 patients with ulcer disease, chronic gastroduodenitis and esophagitis allowed to distinguish three clinico-endoscopic syndromes (erosive-ulcerative, irritated pylorus, regurgitating syndromes). Therapeutic endoscopic manipulations were carried out: transesophageal block of the vagal nerve, pricking of the ulcer with trimecaine, solkoseryl, oxyferriscorbone, trental and laser treatment. Obliterative otosclerosis has been a challenge since the advent of stapes surgery. "Drill-out" procedures have had a generally poorer prognosis than conventional stapes surgery because of excessive bleeding, acoustic trauma from the burr, and reclosure of the oval window by otosclerosis. The trumpet maneuver, a technique to inflate the hypopharynx with air, allows visualization of the pyriform sinuses and retrocricoid area during both fiberoptic examination and CT scanning. The technique is useful in delineating both normal and abnormal anatomy, and is especially useful in the evaluation of patients with hypopharyngeal symptoms in whom there is a low suspicion of anatomic disease. The methods, endoscopic photographs, and CT scan images are presented.
