PURPOSE: The literature remains pessimistic about surgery for finger stiffness in patients with complex regional pain syndrome (CRPS), citing the risks of exacerbation and limited motion gains. This study reports the outcomes of joint-release procedures and details the surgical technique.
METHODS: All patients with CRPS and contracted finger joints who presented with unyielding stiffness and consented to surgery were included. Patients underwent a minimally invasive release of the metacarpophalangeal and/or proximal interphalangeal (PIP) joints. By protocol, all patients also underwent carpal…

Lateral band snapping syndrome can be a debilitating condition involving the proximal interphalangeal (PIP) joint of the fingers. Although rare, it is thought to occur due to disruption of the finger extensor mechanism allowing inappropriate subluxation of the lateral bands over the PIP joint, leading to mechanical and painful symptoms. Here, we describe a woman in her early 40s with a painful, snapping PIP joint secondary to a work-related injury. On examination, there was radial lateral band subluxation with joint motion. The dorsal aspect of the small finger was explored, and the radial later

PURPOSE: To determine whether preservation of blood supply to the index metacarpophalangeal joint decreases the rate of physeal arrest.
METHODS: A retrospective review of 41 pollicized digits in 35 patients with 2-year minimum radiographic follow-up was conducted at a single institution. Other complications evaluated included nonunion at the pollicized digit base and clinical instability at the new carpometacarpal joint. Findings were compared to historical controls, which were performed by our group prior to routine identification and sparing of the metacarpophalangeal joint blood supply. No

Five patients with chronic instability of digital joints presented with instability and functional disability. Two patients had ulnar collateral ligament damage of the thumb metacarpophalangeal joint and another had chronic multidirectional instability due to radial collateral ligament, dorsal capsule and palmar plate laxity of the metacarpophalangeal joint of the thumb. The fourth patient had a lax radial collateral ligament and palmar plate of the proximal interphalangeal joint of the little finger and the fifth had chronic laxity of the ulnar collateral ligament of the interphalangeal joint

A rare complex isolated metacarpophalangeal joint dislocation of the middle finger is described, and the pathology is reviewed. The dorsal longitudinal incision used in this case provided excellent exposure for the reduction and helped avoid injury to the digital nerves on the palmar side of the injury. Early mobilization with extension blocking is advocated to maximize joint range of motion postoperatively.

Boutonniere deformity results from damage to the central band and structures stabilizing the finger's extensor apparatus, leading to flexion of the proximal interphalangeal (PIP) joint and hyperextension of the distal interphalangeal (DIP) joint. Untreated or delayed diagnosis of boutonniere deformity results in progression of both deformity and degenerative changes in the PIP joint. In advanced, chronic deformities (Burton IV), surgical treatment includes arthrodesis or PIP joint replacement. In the presented case, a 29-year-old right-handed man presented with a persistent boutonniere deformi

Disorders affecting the metacarpophalangeal joints of the long fingers are relatively uncommon and often under-recognized compared to those involving the thumb. This review provides a comprehensive and structured overview of a broad spectrum of pathologies involving these joints, including fractures, ligament sprains, joint instabilities and dislocations, sagittal band injuries, boxer's knuckle, lesions of the deep transverse metacarpal ligament, muscle disorders, trigger finger, mechanical locking, arthropathies, avascular necrosis of the metacarpal head (Dieterich's disease), and tumors. PURPOSE: In a cadaveric model, a comparison was made of the strength of a suture method for collateral ligaments (the N method) with that of simple sutures using suture anchors for the repair of collateral ligaments in the proximal interphalangeal joint.
METHODS: We obtained 72 fingers from 18 upper limbs of fresh-frozen cadavers and compared the left and right sides of the same specimens. In experiment 1, we examined the rupture strength and rupture sites of intact collateral ligaments in 24 fingers. In experiment 2, we compared the rupture strength and failure modes of the N method (three l

Synovial chondromatosis is a rare condition that is also known as Reichel syndrome. It is a disorder that affects the monoarticular joints. The proliferation and metaplasia of the synovial cartilage are its defining characteristics. Many tiny, intra-articular osseocartilaginous loose bodies are formed as a result of this proliferation. They are fed by the synovial fluid once they break off from the synovial surface and enter the joint cavity, where they develop into calcification and ossification. If left untreated, it usually causes the afflicted joint to malfunction severely. Large joints su

Tumoral pseudogout is a rare clinical form of calcium pyrophosphate dihydrate crystal deposition disease. Tumoral pseudogout can mimic other diseases such as chondroid tumor, tophaceous gout, or tumoral calcinosis. Its radiological features have been presented in some case reports, but no specific radiographic features have been identified. Here, we report an unusual case of recurrent tumoral pseudogout involving the proximal interphalangeal joint of the right long finger. This case presents with progressive radiological findings of the disease with an enlarging and recurrent calcified mass an

The authors report a case of traumatic dislocation of both interphalangeal joints of the ring finger in a 29-year-old male patient, an injury sustained while playing soccer football. Reduction was achieved by closed manipulation, followed by splinting in the "intrinsic plus" position. The joints were stable but had not yet fully recovered their mobility at six months follow-up. The authors survey the literature reports on this rare injury.

A patient is reported with an extremely unusual clinical presentation comprising painful subungual glomus tumors, hemangioma-like skin lesions of the trunk, and nodular lesions of the finger joints. Prominent arteriovenous shunting also was present. The etiology of all these abnormalities was thought to be extensive proliferation of glomus cells.

Complex dislocation of the index metacarpo-phalangeal joint almost always requires surgical intervention. Controversy exists as to the most suitable surgical approach to reduction: palmar or dorsal. We reviewed four cases and carried out dissections in eight cadaver hands to compare the surgical approaches. The interposed volar plate was found to be the most important obstacle to reduction. Both approaches were successful in obtaining reduction. The dorsal approach was simple and safe, but necessitated longitudinal division of the volar plate and may carry a theoretical risk of late instabilit

A unique case of an isolated, closed, complex metacarpophalangeal (MP) joint dislocation of the long finger is reported. The deep transverse metacarpal ligaments play an integral role in the stability of the metacarpophalangeal (MP) joints because of their firm bilateral attachments to the palmar plate. The partial rupture of this ligament during the course of trauma prevents closed reduction. Essential correction consists of incision of the remaining fibers of the ligamentous connection to the palmar plate.
