[Strategy for peroperative fluid therapy--balancing the central blood volume].

The strategy for preoperative volume treatment is not clear, partly because of the difficulty in monitoring the central blood volume (CBV). The cardiovascular responses to hypovolaemic shock are reviewed with emphasis on the often very low heart rate, which develops when the CBV is reduced by about 30%. A proposal has been put forward that normovolaemia is the CBV that does not limit cardiac output. The possibility of monitoring CBV by electrical impedance is also reviewed. It is concluded that the deviations reflect the CBV accurately in both experimental and clinical studies. A strategy that

Oral fluid therapy in paediatric burns (5-10%): an appraisal.

Fluid therapy by the oral route is the accepted method of treatment for smaller burns in children (less than 10%). [Settle JAD. Burns - the first five days. Essex: Smith and Nephew Pharmaceuticals Ltd, 1986.] A phone survey was carried out of all the hospitals in the United Kingdom that manage burns, to record their oral fluid therapy practices for burns (5-10% BSA) in paediatric patients. Included in the survey was an assessment of the uniformity of the contents of the fluids, their palatability and acceptance by patients and any side-effects from this form of treatment. There appears to be n…

[Gastroenteritis in children. Peroral rehydration, intravenous fluid therapy and nutrition].

In pediatrics, oral rehydration treatment is increasingly used in the management of gastroenteritis, and based on third world experiences, it is becoming used also in hospitals in western countries. In this paper, the advantages, limitations and practical use of oral rehydration treatment are discussed. Clinical examination of dehydrated children is described, and guidelines are given for intravenous fluid treatment. Furthermore, the article reviews modern research on the nutritional management of gastroenteritis. The incidence of secondary lactose intolerance has been dramatically reduced in

[Fluid therapy for superior mesenteric artery occlusion shock in rats].

The efficacy of three kinds of fluid such as isotonic sodium chloride (0.9% NaCl), hypertonic sodium chloride (7.5% NaCl) and iso-oncotic (3% in saline) albumin solutions to maintain the hemodynamics was investigated in rats with superior mesenteric artery occlusion (SMAO) shock. Three kinds of fluid were infused so as to maintain the mean arterial pressure within 70% of the control values. Animals in the control group without fluid therapy died within three hours because of shock. Three regimens could maintain the mean arterial blood pressure over three hours. The hypertonic crystalloid solut…

Unnecessary fluid therapy in patients with shock: Five classic fluid pitfalls and five ways to do it better.

Intravenous fluid therapy remains a cornerstone of shock management, yet is frequently misapplied. Unnecessary administration can lead to avoidable harm, including fluid accumulation, electrolyte disturbances, and delayed recovery. Despite good intentions, clinicians often fall into predictable patterns that drive overinfusion. Recognizing and correcting these habits is an important step toward safer and more effective fluid management, which may contribute to better patient outcomes. This narrative review highlights five recurring pitfalls in fluid management: confusion between resuscitation,…

Fluid Management for Critically Ill Patients: A Review of the Current State of Fluid Therapy in the Intensive Care Unit.

BACKGROUND: Intravenous fluids (IVF) are frequently utilized to restore intravascular volume in patients with distributive and hypovolemic shock. Although the benefits of the appropriate use of fluids in intensive care units (ICUs) and hospitals are well described, there is growing knowledge regarding the potential risks of volume overload and its impact on organ failure and mortality. To avoid volume overload and its associated complications, strategies to identify fluid responsiveness are developed and utilized more often among ICU patients. Apart from the amount of fluid utilized for resusc

Intraoperative hypotension during robotic-assisted radical prostatectomy: A randomised controlled trial comparing standard goal-directed fluid therapy with hypotension prediction index-guided goal-directed fluid therapy.

BACKGROUND: Robot-assisted radical prostatectomy (RARP) represents the leading application of robotic surgery in the treatment for prostate cancer with faster recovery and reduced invasiveness. Maintaining stable blood pressure during RARP is crucial to avoid complications. The approach used is goal-directed therapy (GDT); however, the Hypotension Prediction Index (HPI), a machine learning algorithm that analyses arterial waveforms, may provide additional benefits. OBJECTIVE: To evaluate the cumulative amount, frequency and duration of intraoperative hypotension episodes in patients undergoin

Goal-directed fluid therapy in autologous breast reconstruction results in less fluid and more vasopressor administration without outcome compromise.

OBJECTIVE: Aggressive or restricted perioperative fluid management has been shown to increase complications in patients undergoing microsurgery. Goal-directed fluid therapy (GDFT) aims to administer fluid, vasoactive agents, and inotropes according to each patient's hemodynamic indices. This study assesses GDFT impact on perioperative outcomes of autologous breast reconstruction (ABR) patients, as there remains a gap in management understanding. We hypothesize that GDFT will have lower fluid administration and equivocal outcomes compared to patients not on GDFT. METHODS: A single-center retro…

Fluid overload due to intravenous fluid therapy for vaso-occlusive crisis in sickle cell disease: incidence and risk factors.

Intravenous fluid therapy (IV-FT) is routinely used in the treatment of vaso-occlusive crises (VOCs), as dehydration possibly promotes and sustains erythrocyte sickling. Patients with sickle cell disease (SCD) are at risk of developing diastolic dysfunction and fluid overload due to IV-FT. However, data on the adverse effects of IV-FT for VOC is sparse. We aimed to evaluate the incidence and risk factors of fluid overload due to IV-FT in patients with SCD. Consecutive hospitalisations for VOC treated with IV-FT between September 2016 and September 2018 were retrospectively analysed. The median…

Revised Starling equation and the glycocalyx model of transvascular fluid exchange: an improved paradigm for prescribing intravenous fluid therapy.

I.V. fluid therapy does not result in the extracellular volume distribution expected from Starling's original model of semi-permeable capillaries subject to hydrostatic and oncotic pressure gradients within the extracellular fluid. Fluid therapy to support the circulation relies on applying a physiological paradigm that better explains clinical and research observations. The revised Starling equation based on recent research considers the contributions of the endothelial glycocalyx layer (EGL), the endothelial basement membrane, and the extracellular matrix. The characteristics of capillaries …

Are heat stroke patients fluid depleted? Importance of monitoring central venous pressure as a simple guideline for fluid therapy.

During pilgrimage season (Hajj) in Saudi Arabia 34 patients with heat stroke (HS) were centrally cannulated to assess their state of hydration and fluid requirement during cooling period. Central venous pressure (C.V.P.) measurements indicated that most victims of heat stroke had normal C.V.P. on arrival at heat stroke centres and may not be fluid depleted. Twenty-two patients (64.7%) had normal or above normal C.V.P. Twelve patients (35.3%) had zero or below zero C.V.P. Six patients (17.6%) had above 10 cmH2O (range 10-26 cmH2O) and could have developed acute congestive heat failure and pulmo

Fluid therapy in adults having non-cardiac surgery: A narrative review.

BACKGROUND: Adequate intraoperative fluid therapy is essential, as both uncorrected fluid loss and excessive fluid administration are associated with increased complications. However, current practice varies widely. In this narrative review, we examine current concepts of intraoperative fluid therapy in adults having non-cardiac surgery, focusing on fluid type, volume of fluid, and fluid administration strategy. RESULTS: Balanced crystalloids, compared to unbalanced crystalloids, more closely resemble the body's natural electrolyte composition. However, moderate intraoperative volumes of 0.9%…
