[Sodium fluoride in the therapy of osteoporosis].

Every form of osteoporosis--except the physiological involution osteoporosis--requires treatment. Osteoporosis is no disease by itself but a result or a concomitant phenomenon of various diseases. Therefore the primary disease has to be treated also. Only for the so-called idiopathic osteoporosis hitherto no cause is known. Today sodium fluoride is the only substance with an influence upon the bone which may induce re-ossification and re-stabilization. In the described series of investigations this compound, with which treatment by gradual dosage is possible, has proved its effectiveness. Oste

Plasma fluoride and enamel fluorosis.

It is postulated that tissue fluid F concentrations are the primary determinants of flouride effects on bones and developing teeth and that these concentrations are dependent on, or mirrored by, blood plasma F. It has earlier been shown that the plasma F levels are dependent on the dietary F supply as well as on skeletal F concentration. Fasting and post-ingestion or postinjection plasma F levels have been determined in rats on F doses that cause different degrees of enamel fluorosis. The results indicate that temporary peak values rather than elevated fasting values are responsible for the oc

Fluoride enamel stability and remineralization.

Dental caries remains as one of the most prevalent noncommunicable diseases even with advances in preventive dental care. This literature-based research examines how fluoride exposure influences enamel stability and caries formation. We hypothesized that fluoride reduces caries incidence by promoting remineralization and reinforcing the crystal lattice structure. A comprehensive search was done through PubMed, ScienceDirect, Karger, and Figshare to select peer reviewed studies that evaluated fluoride and enamel interactions. Quantitative data were extracted from open access in vitro and in viv

Fluoride in Human Health and Nutrition.

Fluorine is in the form of fluorides ubiquitous in nature and as such an inevitable part of our environment. In small amounts, it is known to have beneficial effects on dental health. On the other hand, excessive chronic intakes can result in adverse effects including the development of dental fluorosis in children and/or skeletal fluorosis in both children and adults. The adequate intake has been set, based on empirical observations, at 0.05 mg/day/kg body weight however, the threshold between beneficial and harmful effects is narrow. Despite numerous studies, knowledge on the fluoride toxici

Systemic versus topical fluoride.

The actual mechanism of fluoride action is still a subject of debate. A dogma has existed for many decades, that fluoride has to be ingested and acts mainly pre-eruptively. However, recent studies concerning the systemic effect of fluoride supplementation concluded that the caries-preventive effect of fluoride is almost exclusively posteruptive. Moreover, epidemiologists have cast doubt on the validity of the 'old' studies dealing with fluoride use. The concept of the posteruptive fluoride effect is supported by in vitro and in situ investigations demonstrating that the mode of action of fluor

The effect of fluoride on the developing tooth.

This review aims to outline the effects of fluoride on the biological processes involved in the formation of tooth tissues, particularly dental enamel. Attention has been focused on mechanisms which, if compromised, could give rise to dental fluorosis. The literature is extensive and often confusing but a much clearer picture is emerging based on recent more detailed knowledge of odontogenesis. Opacity, characteristic of fluorotic enamel, results from incomplete apatite crystal growth. How this occurs is suggested by other changes brought about by fluoride. Matrix proteins, associated with the

Anabolic effects of fluoride on bone.

Fluoride exerts a biphasic action at the level of osteoblasts, on bone mineral, on bone structure and function, and in the treatment of osteoporosis. At low circulating concentrations, skeletal uptake of fluoride is limited and the effects are beneficial. At higher concentrations and greater skeletal uptake, fluoride may cause the formation of abnormally mineralized bone of impaired quality. A new treatment program entailing intermittent slow release sodium fluoride (SR-NaF) with continuous calcium citrate may capture desirable qualities of fluoride without toxic effects, and be therapeuticall

[Benefits and risks of fluoride supplements].

Fluoride is an essential trace element. It is incorporated in calcified tissues. At doses lower than 1.5 mg/day, fluoride has a prophylactic action against dental caries by absorption on the enamel surface. At doses ranging from 15 to 25 mg/day, it is used as a therapeutic agent in adults, in the treatment of post-menopausal osteoporosis with vertebral crush fractures. At high doses, fluoride may induce skeletal fluorosis leading in children to pseudorachitism. In children, fluoride supplement has been recommended for several years, as a prophylactic agent against dental caries. It may be give

Dental tissue effects of fluoride.

It is now well-established that a linear relationship exists between fluoride dose and enamel fluorosis in human populations. With increasing severity, the subsurface enamel all along the tooth becomes increasingly porous (hypomineralized), and the lesion extends toward the inner enamel. In dentin, hypomineralization results in an enhancement of the incremental lines. After eruption, the more severe forms are subject to extensive mechanical breakdown of the surface. The continuum of fluoride-induced changes can best be classified by the TF index, which reflects, on an ordinal scale, the histop

Kinetics of fluoride in the oral fluids.

The normal concentration of fluoride in saliva is about 1 mumol/L, which is somewhat less than that in plasma, and the salivary concentration is relatively independent of flow rate. Even this low concentration appears to be significant in terms of maintaining the integrity of tooth mineral. After fluoride consumption, the level in plasma peaks within less than an hour, and this produces a corresponding increase in salivary levels, which achieve baseline values usually within a few hours. At low concentrations (less than 4 mmol/L) in oral fluids, fluoride undergoes only slight reaction with too

Fluoride and bone: an unusual hypothesis.

Given the total body weight, the weight of the skeleton and the proportion of minerals in the skeleton, a simple formula can be used to estimate the accumulation of fluoride in the skeleton over periods of years. It would appear that daily intakes of fluoride considered beneficial to developing teeth may, if ingested throughout adult life, lead to skeletal fluorosis of varying degrees in a significant proportion of the population. To date, the accumulation of fluoride in the skeleton seems to have been ignored as a possible aetiological factor in certain metabolic bone disorders now common in

Intestinal absorption of fluoride preparations.

Gastric intestinal absorption of five different preparations of sodium fluoride was measured by means of ten hour urinary excretion. Best absorption was by nonenteric coated sodium fluoride. All three enteric coated preparations showed poorer absorption, with marked individual variations. Florical, the currently available preparation, showed better absorption than any enteric coated tablets and only slightly less than sodium fluoride alone. For active cochlear otospongiosis two capsules three times a day should be prescribed.

The reaction of stannous fluoride and hydroxyapatite.

Systematic investigation of the chemical reaction of a slurry of SnF2, Ca10(OH)2(PO4)6, and H2O shows that products other than CaF2 are determined by the relative concentrations of the reactants. In addition to Sn2OHPO4, and Sn3F3PO4, a previously unreported compound, Ca(SnF3)2, has been observed as a product of the reaction.
