The surgical management of patients with biventricular hearts and complex double-outlet right ventricle with noncommitted ventricular septal defect, hypoplastic ventricles, common atrioventricular orifice, straddling tricuspid valve, and straddling mitral valves is controversial. OBJECTIVE: To clarify the association of the aortopulmonary collaterals (APCs)/venovenous collateral (VVCs) in patients with plastic bronchitis (PB) after the Fontan procedure.

Neoaortic root dilatation is a significant and inevitable late complication in patients after the Norwood operation. In this report, we describe a patient who had left pulmonary artery compression and severe neoaortic regurgitation from a severely dilated neoaortic root 11 years after the Norwood operation and subsequent Fontan procedure. We performed successful valve-sparing neoaortic root replacement in this patient.

BACKGROUND: A modification of the Fontan operation was recently applied, which includes anastomoses of the extracardiac conduit to the right pulmonary artery and inferior vena cava using simple clamping with no additional circulatory support, venous shunting, pulmonary artery preparation, or prior maintenance of azygos vein patency.
