Anatomical basis of approaches to foramen magnum and lower clival meningiomas: comparison of retrosigmoid and transcondylar approaches.

OBJECT: Meningiomas of the lower clivus and foramen magnum are among the most challenging of all neurosurgical lesions. Debate continues regarding the most appropriate approach to this eloquent anatomical region. This anatomical study was undertaken to measure and compare the area of surgical exposures of the lower clivus achieved using the retrosigmoid and the extreme-lateral transcondylar (ELT) approaches. METHODS: Thirteen embalmed cadaveric heads were dissected bilaterally via the retrosigmoid approach on one side and the ELT approach on the other. The circumference of the area of exposur…

Recurrence of a neurenteric cyst with malignant transformation in the foramen magnum after total resection. Case report.

A 53-year-old man presented with recurrence of a neurenteric cyst with malignant transformation in the foramen magnum 3.5 years after total resection of the original tumor had been reported. For 2 years following the initial surgery, the patient had been in good condition, but then underwent ventriculoperitoneal shunt placement for intracranial hypertension. At the time there was no evidence of recurrence of the tumor on magnetic resonance (MR) images. One and one-half years later, he presented with headache and anorexia. A massive recurrent tumor was identified on MR images. The tumor was sev…

[A case of a young infant with Chiari II malformation treated by only bony foramen magnum decompression].

A case of a young infant with Chiari II malformation treated by only foramen magnum decompression was reported. The patient was a 3-month-old infant with a one-month history of stridor and respiratory distress. The myelomeningocele was surgically repaired. At the age of two weeks, a ventriculoperitoneal shunt had been inserted. On admission, MRI demonstrated Chiari II malformation. The shunt mechanism was functioning. Suboccipital craniectomy and upper cervical laminectomy from C1 to C5 level was performed. After the removal of the dural band at the foramen magnum level, good pulsation of the …

How I do it - The "drum skin" duraplasty technique after foramen magnum decompression for Chiari malformations.

BACKGROUND: Chiari malformations are a spectrum of posterior cranial fossa anomalies characterized by herniation of the cerebellar tonsils through the foramen magnum. Surgery is the treatment of choice for selected patients with good postoperative outcomes. METHODS: We describe foramen magnum decompression (FMD) with dural opening and a "drum skin" duraplasty technique. CONCLUSIONS: In our experience, FMD with "drum skin" duraplasty is a safe and effective procedure, reducing the risk of CSF leakage and arachnoidal adhesions and yielding better long-term clinical and radiological outcomes th…

Morphometric Analysis of Posterior Fossa and Foramen Magnum among Pediatric Age Group 6 to 16 Years.

Background Configuration and size of the foramen magnum and posterior cranial fossa plays an important role in the pathophysiology of various disorders like Chiari malformations, basilar invagination etc. Thus, a fundamental knowledge of normal anatomy of this region is important to the clinician for diagnosis and treatment. However, we couldn't find any anatomical study related to the concerned topic among the pediatric population aged 6 to 16 years in Nepal to the best of our knowledge. Objective To attain the baseline results (volume of bony part of posterior cranial fossa and the surface a…

How I do it. Foramen magnum meningiomas. Surgical tenets for a unique pathology of a transition zone.

BACKGROUND: Foramen magnum meningiomas (FMMs) represent significant neurosurgical challenge because of their intimate relationship with highly functional and vital neurovascular structures. METHOD: We discuss and illustrate technical nuances to resect type 3 FMMs with anterolateral dural attachment, causing compression and displacement of the medulla oblongata, accessory nerve and upper cervical spinal cord, with encasement of vertebral artery, through far lateral approach. CONCLUSION: Proper extradural preparation, early cerebrospinal fluid drainage, sectioning of dentate ligament, working …

Foramen Magnum Stenosis in Achondroplasia: Imaging-Based Surgical Indications, Synchondrosis Fusion, and the Role of Ventriculomegaly.

INTRODUCTION: Foramen magnum stenosis (FMS) and ventriculomegaly are frequent in achondroplasia due to cranial base hypoplasia. While FMS can cause spinal cord compression, the optimal treatment strategy remains debated. We reviewed our single-center experience and explored factors influencing surgical decisions. METHODS: We retrospectively analyzed 18 children with achondroplasia evaluated between 2008 and 2024. MRI was used to assess FMS by anterior-posterior diameter and cerebrospinal fluid (CSF) area at the foramen magnum level. Ventriculomegaly was defined as an Evans Index ≥0.30. CT was…

IDH-mutant Astrocytoma Arising in the Brainstem with Symptom Improvement by Foramen Magnum Decompression: A Case Report.

Diffusely infiltrative midline gliomas are known to have a poor prognosis. The standard treatment for typical diffuse midline glioma in the pons is local radiotherapy as surgical resection is inappropriate. This case reports a brainstem glioma in which stereotactic biopsy and foramen magnum decompression were concomitantly performed to confirm the diagnosis and improve symptoms. A 23-year-old woman was referred to our department with a chief complaint of headache for six months. Magnetic resonance imaging (MRI) showed diffuse T2 hyperintense swelling of the brainstem with the pons as the main …

Far-Lateral Approach for Foramen Magnum Meningioma: An Anatomical Study with Special Reference to Bulbopontine Junction.

Intracranial meningiomas are sometimes located anteriorly to the foramen magnum and can cause disabling long tract symptoms. The far-lateral approach has been developed to provide an extensive view over the bulbopontine junction and the surrounding lower cranial nerves and upper spinal nerves with a good control on the vertebral artery, allowing the safe resection of such tumors. It is the report of a case with anatomical study before and after the removal of the meningioma. The use of the far-lateral approach allowed us to (1) control the vertebral artery in its V3 (Atlantic extradural) and V…

Spinal Accessory Nerve Meningioma at the Foramen Magnum with Medullar Compression: A Case Report and Literature Review.

BACKGROUND: Meningiomas that arise from the cranial nerve are rare. We present a case with an intradural extramedullary tumor at the foramen magnum originating from the spinal accessory nerve. CASE DESCRIPTION: The patient was a 69-year-old woman with dizziness and pain in the bilateral shoulder for 2 years. Neurologic examination revealed spinal accessory nerve palsy (difficult in raising the shoulder, deficit of 3/5) on the left side without further deficits. Magnetic resonance imaging showed medullar compression because of a left intradural extramedullary foramen magnum lesion dorsolateral…

Lower third clivus and foramen magnum intradural tumor removal: The plea for a simple posterolateral approach.

BACKGROUND: Anterior or anterolateral lesions of the lower third clivus and/or foramen magnum require a surgical exposure that provides a clear visualization of both pathological and normal anatomy without retraction of neural or vascular structures. The posterolateral approach permits access to the anterolateral intradural aspect of the brainstem. The aim of this study was to stress that there is no need for vertebral artery transposition, occipital condyle drilling, occipitocervical fixation or trans-oro-pharyngeal access to remove these lesions. METHODS: All five consecutive patients treat…

Morphometric comparison of foramen magnum in non-syndromic craniosynostosis patients with or without Chiari I malformation.

BACKGROUND: In order to compare the morphometry of foramen magnum (FM) in a matched-pair study, in children with non-syndromic craniosynostosis with and without Chiari I malformation (CMI), both brain magnetic resonance (MRI) and multidetector computed tomography (MDCT) images were utilized. METHODS: Brain MR images were retrieved from the Helsinki University Hospital Picture Archiving and Communications System to identify patients with CMI during 1.1.2004 to 31.3.2009. Age-, gender-, and craniosynostosis-matched controls were retrieved from the same cohort. Morphometric analysis of FM was ca…

Dural arteriovenous fistula at the foramen magnum presenting with subarachnoid hemorrhage: case reports and literature review.

BACKGROUND AND PURPOSE: Spinal dural arteriovenous fistulas (DAVFs) may arise at any level from the foramen magnum to the sacrum. Only a few case series of DAVFs at the foramen magnum have been reported, especially with patients presenting with subarachnoid hemorrhage (SAH). We performed a retrospective study of four such cases and summarize experiences in the diagnosis and surgical treatment of a DAVF at the foramen magnum. METHODS: Four male patients, aged from 35 to 51 years, were admitted with severe headache. The cranial computerized tomography scans of all four patients showed SAH, with…

Neonatal neurological disorders involving the brainstem: neurosonographic approaches through the squamous suture and the foramen magnum.

Brainstem damage which often indicates a critical condition is usually underestimated by trans-anterior-fontanel neurosonography (NS) owing to the far-field limitations. Instead, NS alternately scanning through the squamous suture of the temporal bones and the foramen magnum could provide a better visualization of the brainstem structures. The NS characteristics of brainstem lesions caused by various neonatal neurological disorders, such as hypoxic-ischemic encephalopathy (HIE), metabolic encephalopathy, birth trauma and bacterial meningoencephalitis, can be depicted at the acute stage. An ech…
