[Open intercarpal palmar dislocation fracture with capitate fracture--stabilization by external fixation].

A case of open carpal dislocation fracture not previously described is reported on. Reduction and stabilization of the dislocation were successfully performed using external fixation without additional Kirschner wires. Despite extensive trauma, hand function is now excellent, and there are no signs of residual carpal instability.

Fractures and Dislocations of the Tarsal Navicular.

Fractures of the tarsal navicular are commonly the result of trauma or chronic overload. Because of its complex anatomy and blood supply, the tarsal navicular is susceptible to osteonecrosis, and injury to this bone can lead to posttraumatic arthrosis of the surrounding joints. Diagnosis of the injury, especially in patients with stress fractures, can require a high index of suspicion and the use of advanced imaging. The treatment of stress fracture is controversial and ranges from immobilization in a non-weight-bearing cast or boot to internal fixation with or without bone grafting. Traumatic

Complications of dislocated calcaneal fractures.

BACKGROUND: Calcaneal fractures are the most common of all tarsal fractures, with the majority being the result of high energy injuries causing comminution of bone and lesions of the posterior articular surface. If the bone fragments are left dislocated or non-operative reduction is attempted, the outcome is poor, often with chronic disability. On the other hand, operative treatment does not always lead to a good result and there is an inherent risk of devastating complications. MATERIAL AND METHODS: During a 3-year period (2002-2005) the authors treated operatively (by open reduction and int

Bilateral Galeazzi fracture-dislocations.

We present the case of a patient with bilateral Galeazzi fracture-dislocations with an irreducible distal radioulnar joint (DRUJ) on one side. Current treatments, such as anatomic reduction and stable internal fixation of the radius and anatomic and stable reduction of the DRUJ followed by plaster immobilization in supination, have produced results much better than those associated with historical conservative treatments. The most important part of successful treatment is maintaining intraoperative control of reduction quality, DRUJ stability, and, if necessary, open reduction of the DRUJ. Her

Fractures and dislocations of the cervicothoracic junction.

Presented is a retrospective review of case notes and all available imaging studies in seven patients with acute fractures-dislocations of the cervicothoracic junction. Imaging studies included radiographs (five cases), computed tomography (six cases), and magnetic resonance imaging (seven cases). The study group consisted of five men and two women with mean age at presentation of 43.6 years (range 25-69 years). Four patients had been in road traffic accidents, whereas three patients had had falls. Three patients sustained complete neurologic deficits with no recovery, whereas the remaining fo

Fractures and dislocations of the carpal bones.

The most common wrist fractures in athletes are fractures of the scaphoid and fractures of the hook of the hamate. Accurate diagnosis and conservative treatment are extremely important. A physician should not compromise principles of treatment for a rapid return of the athlete to competition, because all too often this will jeopardize eventual recovery and his or her future career. It is far better to be conservative, to insist upon complete recovery before permitting competition, especially in high-school and college students, and to be more concerned with the athlete's potential achievements

Patellar dislocation and osteochondral fractures.

Follow-up study of nine patients with patellar dislocation in combination with osteochondral fractures. Three of these patients with special problems in relation to knee pathology are discussed in detail. This group of knee injuries is often overlooked because of frequent spontaneous reposition. Etiology, diagnosis and treatment are discussed. Arthroscopy is recommended to visualize chondral defects. Results are presented in combination with follow-up. Four patients still complain of instability of the knee after surgery. Reconstruction is the treatment of choice. The kind of fixation depends

Open radiocarpal fracture-dislocations.

Ten open radiocarpal fracture-dislocations in nine patients were treated by wound debridement, open reduction, and maintenance of reduction by casting supplemented with internal or external fixation. Eight patients had associated fractures or dislocations of the ipsilateral extremity: elbow fracture or dislocation (three patients), shoulder fracture (one patient), fractured humerus (two patients), and phalangeal fractures (three patients). In seven of ten cases the median and/or ulnar nerve was severely contused and required immediate decompression. Six patients were followed at least 15 month

Subtleties of Lisfranc fracture-dislocations.

Tarsometatarsal (Lisfranc) fracture-dislocations can be very difficult to recognize radiographically. The evaluation of the Lisfranc joint can be simplified by meticulously studying the alignment of the metatarsal bases with their corresponding tarsal bones. The usefulness of this observation has been confirmed by: anatomic descriptions, biomechanical analysis, and clinical foot radiographs. Subtle abnormalities identified at the Lisfranc joint using this observation included: metatarsal subluxations identified only on a single projection, associated tarsal subluxations and dislocations, irred

Fractures and dislocations of the ankle joint.

The classification of fractures of the ankle joint is based on the mechanism of injury, which provides the clue to successful reduction. The physiological movements of the ankle and foot are defined, and the pathological movements associated with injury are described.

Arthroscopic reduction and percutaneous fixation of perilunate dislocations and fracture-dislocations.

PURPOSE: The purpose of this study was to review clinical and radiographic outcomes of perilunate dislocations and fracture-dislocations treated with arthroscopic reduction and percutaneous fixation. METHODS: Twenty patients who had an acute dorsal perilunate dislocation or fracture-dislocation were treated with an arthroscopic technique at a median interval of 3.9 days from the time of injury. They were retrospectively reviewed at a mean follow-up of 31.2 months (range, 18 to 61 months). Range of motion and grip strength were measured. Radiographic evaluations included time to scaphoid union

[Perilunate dislocations and fracture-dislocations of the wrist, a review of 14 cases].

OBJECTIVES: We report a retrospective series of 14 dislocations or perilunate fracture-dislocations. The results of our series are compared with the data of the literature and we discuss epidemiology, types of lesions, surgical treatment, complications and prognosis of this pathology. METHODS: The series included seven pure dislocations and seven fracture-dislocations including three trans-scapho-lunate forms (including one Fenton's syndrome). The displacement of all these lesions was posterior. The mean age was 35 years. Sixty-four percent were manual workers. All 14 patients had undergone s

[Modern ideas on the treatment of dislocations and fracture-dislocations in the elbow joint].

The authors describe the causes and surgical methods of treatment of fresh and long-standing dislocations and fracture-dislocations in the elbow joint. The indications to using different operative methods and technical-surgical interventions are described in details as well as long-term results of treatment by the literature data. The current methods and the authors' methods of prophylactics and treatment of postoperative complications are analyzed. The methods of rehabilitation of such patients are also described.

Lisfranc dislocations: fractures and/or dislocations through the tarso-metatarsal joints.

Because of the anatomical configuration of the tarso-metatarsal joints with their strong ligamentous connections, pure dislocations in this region are rare. We describe our experience of 24 cases of Lisfranc dislocation. Classification is according to Wilson. In the long term, functional and radiological results appear to depend on the accuracy of reduction. For good anatomical results, immediate closed or, if necessary, open reduction and stabilization by percutaneous Kirschner wires is required. Because of the severity of soft tissue damage, decompression with dorsal skin incisions should al

Incidence of peroneal tendon dislocation with talus fracture.

BACKGROUND: Traumatic peroneal tendon dislocation (PTD) is known to occur with pilon and calcaneus fractures, however, literature describing PTD in concurrence with injury to the talus remains limited. METHODS: This was a retrospective review of adult patients with operatively treated talus fractures treated at a level I academic trauma center between 2007 and 2021. Charts, radiographs, and advanced imaging, when available, were reviewed for patient demographics, injury characteristics, and complications. RESULTS: 126 patients with 128 talus fractures were included. Incidence of PTD after ta
