Patho-anatomic findings in neck structures in asphyxiation due to hanging: a survey of 80 cases.

By examining 80 consecutive cases of death due to hanging, fractures of the thyroid cartilage or the hyoid bone or both, were noted in 45%. Fracture of the cricoid cartilage did not occur. No fracture occurred below the age of 25, and the frequency was slightly increasing with increasing age. There was a preponderance of fractures in males. The highest incidence of fractures was noted in typical hangings, while incomplete hanging to a very great extent was combined with congestion to the face, especially if the location of the ligature was atypical. Typical hanging in general did not produce c

Surgical management for isolated cricoid fracture causing arytenoid immobility.

Cricoid cartilage fractures usually occur concurrently with disorders of laryngeal function. In, particular, displaced cricoid lamina fractures can affect arytenoid movement. However, functional, recovery may require proper repositioning of the cricoid lamina, which is associated with a high rate of, complications. Here we present a case in which an isolated cricoid cartilage fracture with arytenoid, immobility due to displacement of the fracture in the cricoarytenoid joint space was successfully, treated. Our findings suggest that a combination of external approaches with temporary, cricothyr

A case of severe flail chest with several dislocated sterno-chondral fractures.

INTRODUCTION: Flail chest is diagnosed clinically by the presence of paradox movement of a segment of the thoracic wall during spontaneous breathing. Radiographic finding confirming a clinical flail chest are fractures of three or more consecutive ribs or costal cartilages in two or more places. Surgical stabilization is associated with a reduced length of hospital stay, time with mechanical ventilation and risk of respiratory complications. PRESENTATION OF CASE: A trauma patient had a Computed Tomography (CT) scan showing multiple costa fractures, sternal fracture, manubrium fracture, sterna

Ischemia leads to delayed union during fracture healing: a mouse model.

Vascular damage accompanying skeletal injury leads to an ischemic environment, and in clinical settings the extent of vascular damage is directly correlated with failure of skeletal repair. However, the exact mechanism(s) underlying ischemia-related defects in bone healing are not well understood. To better understand the mechanism and to facilitate development of novel interventions to treat ischemic fractures, a mouse model of long bone fracture healing in an ischemic environment was created. Ischemia was induced by femoral artery resection prior to tibia fracture. Fractures were left unstab

Expression of galanin and galanin receptor-1 in normal bone and during fracture repair in the rat.

The neuropeptide galanin (GAL) has recognized physiological actions in the nervous system and other tissues, but there is no documented evidence of GAL influencing normal or pathological bone metabolism. GAL expression, however, is upregulated in central and peripheral nerves following axotomy and is known to influence neural regeneration. Thus, severance of skeletal-associated nerves during fracture could similarly increase local GAL concentrations and thereby influence fracture healing. The initial aim of this study was therefore to identify the presence of GAL in normal bone and/or fracture

Calcified plate and flattening of the femoral condyle with spontaneous osteonecrosis.

Four cases with spontaneous osteonecrosis of the knee are reported to highlight mainly the features of the cartilage flap over the necrotic lesion. Case 1 and Case 2 showed late Stage 2 and Stage 3 spontaneous osteonecrosis in the medial femoral condyle, respectively, but featured a convex calcified plate with no flattening in the lateral view of the radiographic knee. Case 3 showed multiple small fractures in the subchondral necrotic bone of the cartilage flap. In Case 4, the cartilage flap became detached and was found in the synovium of the supra-patellar fossa; moreover, the necrotic bone

Trans-tragal incision for improved exposure of diacapitular and condylar neck fractures.

The authors present a modification of the preauricular approach that improves the exposure of the condyle whilst reducing diacapitular and condylar neck fractures. The incision is a combination of the hockey-stick and endaural incisions. Its inferior part runs within the ear on the posterior face of the tragus; the tragal cartilage is transected together with the retrotragal skin and included in the anterior skin flap. Between May 2009 and December 2010, 16 patients with diacapitular or condylar neck fractures were treated with this approach. All patients showed good occlusion postoperatively

[Magnetic resonance imaging of the locomotor apparatus].

A multiplanar approach and a good tissue differentiation characterize magnetic resonance imaging which have a great role in exploring numerous musculo-skeletal lesions; review of main indications and limits. MRI is useful in diagnosing certain undisplaced fractures, fatigue and insufficiency fractures, and consequences. Fibrous cartilage and hyalin cartilage lesions, tears of tendons, ligaments and muscles are well appreciated. MRI is also very good for an early and correct diagnosis of avascular necrosis, spinal degenerative lesions, spinal consequences of rheumatoid arthritis and ankylosing

[New options for management of posttraumatic articular cartilage defects].

Articular cartilage trauma, in particular due to its poor healing potential remains a complicated problem in both the adult and paediatric traumatology and orthopedics. In older patients, total endoprosthesis of the joint is a method of choice, however, in younger patients, the situation remains more complicated. In case of osteochondral lesions (arthrosis, chondral fractures. osteochoodrosis dissecns) the ideal management should result in complete recovery of the hyaline cartilage on the traumatized joint surface. Contemporary medicine uses some therapeutic procedures resulting in partial rec

The use of computed tomography in determining developmental changes, anomalies, and trauma of the thyroid cartilage.

Recognition of injury to the hyoid bone and thyroid and cricoid cartilages is intrinsic to post-mortem examination. Due to its increasing brittleness with age the thyroid cartilage is particularly susceptible to injury following neck trauma, although there is inconsistency in the patterns of injury reported. In this study computed tomography scans of the head and neck of 431 deceased persons (235 males and 196 females) between the ages of 1 day and 100 years (mean age 35.93 ± 24.15), and including 25 victims of hangings, were examined to reveal the pattern of age-related change and the types o

Effect of osmolarity and displacement rate on cartilage microfracture clusters failure into two regimes.

Articular cartilage is a poroviscoelastic (PVE) material with remarkable resistance to fracture and fatigue failure. Cartilage failure mechanisms and material properties that govern failure are incompletely understood. Because cartilage is partially comprised of negatively charged glycosaminoglycans, altering solvent osmolarity can influence PVE relaxations. Therefore, this study aims to use osmolarity as a tool to provide additional data to interpret the role of PVE relaxations and identify cartilage failure regimes. Cartilage fracture was induced using a 100 μm radius spheroconical indenter

Transient gamma-secretase inhibition accelerates and enhances fracture repair likely via Notch signaling modulation.

Approximately 10% of skeletal fractures result in healing complications and non-union, while most fractures repair with appropriate stabilization and without pharmacologic intervention. It is the latter injuries that cannot be underestimated as the expenses associated with their treatment and subsequent lost productivity are predicted to increase to over $74 billion by 2015. During fracture repair, local mesenchymal stem/progenitor cells (MSCs) differentiate to form new cartilage and bone, reminiscent of events during skeletal development. We previously demonstrated that permanent loss of gamm

The science of pinning the neck of the femur.

Experiments show that the cancellum of some femoral heads and of the trochanteric region is too weak to support a nail. A simple classification of traumatic femoral neck fractures is presented. These are either vertical or, less commonly, transversely disposed. Either type may be undisplaced, impacted, or displaced. For adequate fixation of undisplaced vertical and all displaced fractures a nail plate is essential. For undisplaced transverse fractures and impacted vertical fractures two screws usually suffice. Impacted transverse fractures require no active treatment. Hammering nails into the

[Discussion on internal fixation of Hoffa fractures].

Hoffa fracture is an unstable intra-articular fracture with significant redisplacement tendency. It is easy to be missed diagnosis when accompanied by distal intercondylar or supracondylar fracture of femur. CT scan is the gold standard for the diagnosis of Hoffa fracture. The treatment principles are anatomic reduction of the articular surface, reliable internal fixation, and early functional activity. At present, the main treatment is arthroscopic screw fixation. During screw fixation, the tail cap of screw should be buried, resulting in non-healing iatrogenic injury of articular cartilage.

Management of an open acetabular fracture in a skeletally immature patient.

BACKGROUND: Open acetabular fractures in children are rare, but potentially devastating injuries. Secondary to the low incidence, there is an apparent lack of reports on appropriate management strategies for open pediatric acetabular fractures in the literature. METHODS: Description of a case study. RESULTS: A 3 years and ten months-old girl was ejected as a passenger from an all terrain vehicle. She sustained a displaced, grade IIIA open left anterior column acetabular fracture. The injury was treated by extending the open wound to a formal first window of the ilioinguinal approach. After s

Subchondral bone failure in overload arthrosis: a scanning electron microscopic study in horses.

Mechanical overload leads to a common arthrosis in the metacarpal condyle of the fetlock joint of racehorses. This is usually asymptomatic but severe forms can cause lameness. Subchondral bone failure is often present and the predictability of the site provided an opportunity to study of the progression of bone failure from microcracks to actual collapse of subchondral bone. Twenty-five fetlock condyles from racehorses with various stages of disease were selected. Stages ranged from mild through severe subchondral bone sclerosis, to the collapse of bone and indentation or loss of cartilage kno

Insufficient Mechanical Loading Downregulates Piezo1 in Chondrocytes and Impairs Fracture Healing Through ApoE-Induced Senescence.

Insufficient mechanical loading impairs fracture healing; however, the underlying mechanisms remain unclear. Increasing evidence indicates that Piezo1 plays an important role in fracture healing, although the effect of Piezo1 on the endochondral ossification of chondrocytes has been overlooked. This study reports that mechanical unloading down-regulates the expression of Piezo1 in chondrocytes and leads to fracture nonunion. Single-cell sequencing of calluses revealed that specific deletion of Piezo1 in chondrocytes upregulated the expression of apolipoprotein E (ApoE) in hypertrophic chondroc

Pseudomass lesions due to occult trauma of the larynx.

Fracture of the laryngeal skeleton is best diagnosed by computed tomography. Deformity of the laryngeal cartilage may cause signs and symptoms that suggest neoplastic submucosal mass. Symptoms may not develop until years after the original injury. A series is described in which the patients demonstrate the characteristic signs, symptoms, and computed tomographic appearance of old, often forgotten, fractures of the thyroid cartilage. The diagnosis is best made by computed axial tomography of the neck.

Proteomics analysis of hip articular cartilage identifies differentially expressed proteins associated with osteonecrosis of the femoral head.

OBJECTIVE: The cartilage degeneration that accompanies subchondral bone necrosis plays an important role in the development of osteonecrosis of femoral head (ONFH). To better understand the molecular basis of cartilage degradation in ONFH, we compared the proteomic profiles of ONFH cartilage with that of fracture control. DESIGN: Hip cartilage samples were collected from 16 ONFH patients and 16 matched controls with femoral neck fracture. Proteomics analysis was conducted using tandem mass tag-based quantitation technique. Gene ontology (GO) analysis, KEGG pathway and protein-protein interact
