Bilateral comminuted radial shaft fractures from a single gunshot: fixation with alternative techniques.

Acute bilateral radial shaft fractures are an unusual entity that has not been previously reported in the literature. Given its bilaterality, this rare clinical entity is best treated with stable internal fixation. Here we report the case of an 18-year- old right-hand-dominant man who sustained a low-caliber gunshot injury. He had been driving with both hands on the steering wheel when he was struck by a single bullet. The bullet caused displaced fractures of the left proximal radial shaft and the right distal radial shaft. Each fracture had extension outside the mid-diaphysis. The patient und

Multiple and comminuted mandibular fractures: treatment outlines in adverse medical conditions in Iraq.

This study was conducted to evaluate prospectively the simple and more reliable methods in the treatment of multiple and comminuted mandibular fractures in hospitals with limited facilities and a high influx rate of trauma patients such as ours in Baghdad. One hundred patients with multiple and comminuted mandibular fractures of different severities were included in this study. Fifty-four patients were with comminuted and 46 were with multiple mandibular fractures. Eighty-eight patients had isolated mandibular fractures, and 12 had other associated facial bones fractures. Seventy-two patients

Immediate plate fixation of highly comminuted femoral diaphyseal fractures in blunt polytrauma patients.

From January 1982 through December 1988, 150 patients with 153 Winquist Class III and IV comminuted diaphyseal femur fractures due to high energy blunt trauma were treated with immediate plate fixation. A total of 260 major general surgical systems were injured in 150 patients. Forty-nine patients did not have adequate preoperative spine radiographs due to positioning or time factors. Nineteen patients had spine fractures; nine were diagnosed post-femoral fixation. The average injury severity score (ISS) was 22.7. Three patients died (2%). Our institution predicted mortality with this ISS for

Radio-scapho-lunate partial wrist arthrodesis following comminuted fractures of the distal radius.

Painful radiocarpal arthritis following comminuted fractures of the distal radius may be treated either by total wrist fusion or by procedures which preserve movement. The authors have reviewed 15 patients with such fractures who have undergone radio-scapho-lunate partial arthrodesis. They report the results with an average follow-up of 23.8 months. Pain was abolished in 7 patients and resolved virtually completely in 4 cases. Restored grip strength averaged 49% of the contralateral side. There was considerable limitation of postoperative range of motion which was restricted to an oblique plan

Usefulness of multiple bioabsorbable Mg screws/K‑wires for comminuted radial head fractures.

PURPOSE: We hypothesized that multiple absorbable screws/K-wires would be effective for native head preservation in comminuted radial head fracture fixation (com-RHFs). METHODS: Seventeen patients who met the inclusion criteria between 2018 and 2020 were included. Radiologic findings indicating proper union and clinical outcomes such as the range of elbow motion, visual analog scale score, and Mayo Elbow Performance Score were assessed prospectively after surgery and at least three years of follow-up. RESULTS: The mean follow-up period was 4.6 years. Eleven, one, three, and two patients pres

Biomechanical design of a new percutaneous locked plate for comminuted proximal tibia fractures.

Comminuted proximal tibia fractures are an ongoing surgical challenge. This "proof of concept" study is the first step in designing a new percutaneous plate for this injury under toe-touch weight-bearing as prescribed after surgery. Finite element simulations generated design curves for overall stiffness, bone and implant stress, and interfragmentary motion using 3 fixations (no, 1, or 2 "kickstand" (KS) screws across the fracture gap) over a range of plate elastic moduli (EP = 5 to 200 GPa). Combining well-established optimization criteria to enhance callus formation (i.e. 0.2 mm ≤ axial inte

[TOTAL ELBOW REPLACEMENT AS AN ALTERNATIVE TO SEVERELY COMMINUTED FRACTURES AROUND THE ELBOW].

BACKGROUND: Intra-articular elbow fractures are considered to be one of the most complex injuries in orthopedic trauma. Some are too comminuted for open reduction and internal fixation. Recently, total elbow replacement (TER) has gained popularity for the treatment of comminuted elbow trauma when other treatment options are not possible. METHODS: Since 2007-2013 we treated 18 patients with TER due to comminuted distal humeral fractures. We used the Disabilities of the Arm, Shoulder and Hand scoring system (DASH) to evaluate the patient's satisfaction. In addition we evaluated the elbow range

Effectiveness of Minimally Invasive Plate Osteosynthesis (MIPO) on Comminuted Tibial or Femoral Fractures.

BACKGROUND: Comminuted fractures happen frequently due to traumas. Fixation without opening the fracture site, known as minimally invasive plate osteosynthesis (MIPO), has recently become prevalent. This study has been designed to assess the outcomes of this treatment for tibial and femoral comminuted fractures. METHODS: A total of 60 patients with comminuted femoral or tibial fractures were operated with MIPO method in this cross-sectional study at Alzahra university hospital in 2015. Eleven patients were excluded due to lack of adequate follow-ups. Patients'data including union time; infect

Treatment of comminuted olecranon fractures with olecranon plate and structural iliac crest graft.

Severely comminuted olecranon fractures are challenging injuries. Commonly used tension band wiring exerts excessive compressive forces causing olecranon shortening, joint incongruity and humeral trochlea subluxation. We report a retrospective study of 3 patients who underwent surgery for a severely comminuted olecranon fracture, with open reduction and fixation with a bridging rigid locking plate and intercalary tricortical structural iliac bone graft. Joint stability was restored allowing early mobilization and good functional outcome. Patients' mean age was 54 years. Mean follow-up was 23 m

Management of comminuted olecranon fractures with precut K-wires and tension band wiring.

Various methods are used to treat comminuted fractures of the olecranon. The preferred method of management of fractures of the olecranon is tension band wiring over K-wires. Often, it becomes difficult to fix small articular fragments with the 2 K-wires used for tension band wiring. Since 2005, we have used the missing K-wire technique to fix the small articular fragments.

Functional evaluation of intraarticular severely comminuted fractures of the calcaneus with gait analysis.

Twenty-one patients (23 feet) treated nonsurgically for severely comminuted intraarticular fractures of the calcaneus were evaluated prospectively with a clinical scoring scale and computerized gait analysis. All patients had Sanders type III and type IV fractures. The treatment protocol consisted of no closed reduction, immobilization in removable splint, physiotherapy after edema subsided, and weightbearing after 8 weeks. All patients had a minimum follow-up of 2 years (mean, 38 months). Clinical results were good in 2 patients, fair in 3 patients, and poor in 16 patients. Gait analysis show

Fixation stability of comminuted humeral shaft fractures: locked intramedullary nailing versus plate fixation.

BACKGROUND: This study compared the fixation stability of two treatments for humeral shaft fractures with segmental bone loss during cyclic, physiologic loading. METHODS: Six matched pairs of human humeri received either a 10-hole broad dynamic compression plate or a locked antegrade inserted humeral nail applied to a humeral diaphyseal osteotomy with a 1.5-cm gap defect. The bone-implant humeral constructs were axially loaded for 10,000 cycles at 250 N and 500 N, with measurements of gap displacement and calculation of construct stiffness. The specimens were then loaded to failure. RESULTS:

Non-operative treatment of comminuted fractures of the proximal humerus in elderly patients.

The purpose of this retrospective study was to assess the clinical and radiographical results of non-operative treatment of displaced multifragmental fractures of the proximal humerus with a minimum follow-up of 10 years. Fifteen patients (one man, 14 women) with 17 injured shoulders were examined. The Neer classification system was used to classify the fractures, and the functional outcome was assessed using the Constant score. At the 10 year review, the mean Constant score for the patients in the three-part fracture group was 59, and 47 in the four-part fracture group. The range of motion wa

Open reduction and internal fixation of comminuted, intraarticular fractures of the distal radius.

Comminuted intraarticular fractures of the distal radius present a difficult problem in management. It is well established that unreduced significant articular incongruency will result in early degenerative osteoarthritis. The technical demands of, and the results to be expected from, formal open reduction and internal fixation of these comminuted articular fractures have not been addressed in the current literature. From 1981 to 1986, 20 patients were treated by open reduction after failure of closed means. Seventeen patients were personally reviewed at an average follow-up of 3.25 years. The

Comminuted fractures of the radial head. The role of silicone-implant replacement arthroplasty.

Radial head resection is the accepted treatment of comminuted radial-head fractures in adults, but the results are not always satisfactory. This study examines the results of silicone replacement arthroplasty of the radial head in acute fractures and as a salvage procedure after failed resections of the radial head. The first group of patients was evaluated both before and after silicone replacement arthroplasty had been done as a salvage procedure following a previous radial head fracture. This group comprised twelve patients with an average follow-up of 3.8 years. Improvement was found in a

Fracture reduction and primary ankle arthrodesis: a reliable approach for severely comminuted tibial pilon fracture.

BACKGROUND: Posttraumatic arthritis and prolonged recovery are typical after a severely comminuted tibial pilon fracture, and ankle arthrodesis is a common salvage procedure. However, few reports discuss the option of immediate arthrodesis, which may be a potentially viable approach to accelerate overall recovery in patients with severe fracture patterns. QUESTIONS/PURPOSES: (1) How long does it take the fracture to heal and the arthrodesis to fuse when primary ankle arthrodesis is a component of initial fracture management? (2) How do these patients fare clinically in terms of modified Ameri
