Endoscopically guided frontal sinus beclomethasone instillation for refractory frontal sinus/recess mucosal edema and polyposis.

Despite its metabolic complications, systemic corticosteroid therapy remains a mainstay in the treatment of refractory polyposis after endoscopic frontal sinusotomy. Furthermore, topical nasal corticosteroids often fail, presumably due to the relatively small dosage actually absorbed by the polyps. In order to minimize steroid complications while increasing the locally absorbed dose, beclomethasone (approximately 1 cc, 84 mcg/100 microliters) was instilled under endoscopic guidance directly into the frontal sinus in 31 instances in 16 patients with postoperative frontal recess/sinus polyposis …

Management of Frontal Sinus Fractures.

Fractures of the frontal sinus occur from extreme forces and are often associated with other injuries. Management of frontal sinus fractures is variable and dependent on involvement of the anterior table, posterior table, and frontal outflow tract. Severe complications can developed from poorly managed fractures, such as meningitis, mucocele, mucopyocele, and brain abscess. Surgeons should be aware of appropriate management and surgical techniques for addressing frontal sinus fractures. Herein, we review the presentation and management of frontal sinus fractures, including conservative, endosc…

Endoscopy-Assisted Frontal Sinus Obliteration.

Comminuted fractures of the anterior table of the frontal sinus involving frontonasal ducts are traditionally treated with sinus obliteration. This technique entails precise mucosa removal which requires unimpeded visualization of the bony recesses. To achieve adequate access to the mucosa, temporary removal of a large part of the frontal bone may often be necessary. This in turn increases fracture comminution requiring placement of additional hardware associated with the risk for bony malalignment, infection, plate palpability, and visibility, especially in thin-skin individuals. Endoscopy, o…

Management of Frontal Sinus Tumors.

The most common primary tumors of the frontal sinus are osteomas and inverted papillomas, although a variety of other tumors involving this space have been reported. With the advent of new surgical techniques and instrumentation, an endoscopic approach to this region has become feasible. The preoperative assessment and decision making must take into account the complexity of frontal sinus anatomy, tumor type, tumor location, and associated attachments. These procedures allow adequate visualization, tumor removal, and postoperative monitoring, and preserve fairly normal sinus function. Open tec…

Management of frontal sinus fractures.

The goals in the treatment of frontal sinus injuries are to provide an esthetic outcome, restore function, and prevent complications. However, there is no consensus as to how to best achieve these goals. Unfortunately, the questions that Stanley proposed in 1989 still lack definitive answers more than 19 years later: (1) Which fractures, if left untreated, will lead to an immediate or delayed complication? and (2) What is the appropriate surgical procedure if treatment of the fracture is deemed necessary? This article discusses the controversies in the surgical treatment of such fractures and …

Frontal sinus fractures: current concepts.

Frontal sinus injuries may range from isolated anterior table fractures resulting in a simple aesthetic deformity to complex fractures involving the frontal recess, orbits, skull base, and intracranial contents. The risk of long-term morbidity can be significant. Optimal treatment strategies for the management of frontal sinus fractures remain controversial. However, it is critical to have a thorough understanding of frontal sinus anatomy as well as the current treatment strategies used to manage these injuries. A thorough physical exam and thin-cut, multiplanar (axial, coronal, and sagittal) …

[Frontal sinus pathology and epilepsy].

OBJECTIVES: Because of its location and the fragility of its physiology, the frontal sinus is the first of the facial sinuses to cause complications. In this context, orbital sepsis, cranial vault osteitis, meningitis, cerebral abscess, longitudinal sinus thrombophlebitis can occur. A more uncommon consequence of frontal sinusitis is isolated epilepsy. METHODS: We report two cases of patients admitted in our department after a generalised epilepsy seizure with, on the CT-scan, an opacity of the frontal sinus with a posterior wall lysis. RESULTS: We operated on quickly both patients after the…

Frontal sinus cementifying ossifying fibroma.

Cementifying ossifying fibroma is a fibro-osseous lesion which produces cementum. Cementifying ossifying fibroma is a mesodermal, non-odontogenic tumor of ectopic multipotential periodontal membrane blast cells. Although, it is a benign bone tumor, it has an aggressive destructive behavior in patients under 20 years of age with high tendency for recurrence. Cementifying ossifying fibroma can be recognized microscopically by the presence of cementicles. This is a case of frontal sinus cementifying ossifying fibroma presenting with proptosis in a 39-year-old male.

Image-guided frontal sinus surgery.

Most patients with inflammatory frontal sinus disease that is un-responsive to medical therapy can be surgically managed with conventional endoscopic techniques; image-guidance technology is not usually necessary. However, in more advanced cases or for revision surgery, a surgical navigation system can be extremely beneficial in preoperative planning and intraoperative localization of complex frontal sinus outflow anatomy. For patients who have failed endoscopic approaches, external procedures, such as frontal sinus obliteration, can also be enhanced through the application of image guidance. N…

Management of frontal sinus fractures.

PURPOSE OF REVIEW: This review summarizes the contemporary management of frontal sinus fractures. RECENT FINDINGS: Recent additions to the armamentarium of the facial trauma surgeon include bioresorbable plates and screws, and attempts to use endoscopic techniques when feasible. SUMMARY: The basic principles of frontal sinus fracture management remain the same. New approaches and new instruments help make repair easier, safer, and more cosmetically acceptable.

Revision endoscopic frontal sinus surgery.

PURPOSE OF REVIEW: Recurrent or persistent frontal sinus disease after endoscopic sinus surgery poses formidable challenges owing to the frontal sinus outflow tract's complex variable anatomy, close proximity to vital structures, and predilection for scarring and stenosis. Frontal sinus obliteration used to be the preferred technique of most sinus surgeons for addressing difficult frontal sinus disease; however, several effective endoscopic techniques exist as excellent alternative procedures for addressing this difficult clinical problem. RECENT FINDINGS: Several endoscopic techniques have b…
