Percutaneous endoscopic gastrostomy: indications, technique, complications and management.

Percutaneous endoscopic gastrostomy (PEG) is the preferred route of feeding and nutritional support in patients with a functional gastrointestinal system who require long-term enteral nutrition. Besides its well-known advantages over parenteral nutrition, PEG offers superior access to the gastrointestinal system over surgical methods. Considering that nowadays PEG tube placement is one of the most common endoscopic procedures performed worldwide, knowing its indications and contraindications is of paramount importance in current medicine. PEG tubes are sometimes placed inappropriately in patie…

Percutaneous radiological gastrostomy: single-puncture double-anchor technique.

PURPOSE: We evaluated technical success, safety and effectiveness of percutaneous radiological gastrostomy (PRG) with a modified technique: single puncture and double anchor. MATERIALS AND METHODS: From January 2008 to June 2011, 163 patients underwent PRG with a single-puncture double-anchor technique. The stomach was punctured with a 17-gauge Chiba needle, and gastropexy was performed by placing two anchors in the gastric lumen. Finally, a 12-F Wills-Oglesby percutaneous gastrostomy catheter was inserted. Technical success and complications at 30 days were evaluated on the basis of imaging …

Colonic injury following percutaneous endoscopic-guided gastrostomy insertion.

Percutaneous endoscopic gastrostomy (PEG) is a common practice usually offered to patients who are unable to tolerate or swallow oral feed and require long-term nutrition. We present a case of early pneumoperitoneum after a PEG placement due to colonic perforation. The patient was severely malnourished and had a medical history of brain injury, cerebrovascular accident cerebrovascular accident (CVA) and bilateral below knee amputations from a bomb blast 13 years ago. The PEG tube was placed under sedation. On the first postoperative day, the patient had a subtle pneumoperitoneum that was consi…

Cone-beam computed tomography-guided percutaneous radiologic gastrostomy.

The purpose of this study was to investigate the feasibility of a flat-detector C-arm-guided radiographic technique (cone-beam computed tomography [CBCT]) for percutaneous radiologic gastrostomy (PRG) insertion. Eighteen patients (13 men and 5 women; mean age 62 years) in whom percutaneous endoscopic gastrostomy (PEG) had failed underwent CBCT-guided PRG insertion. PEG failure or unsuitability was caused by upper gastrointestinal tract obstruction in all cases. Indications for gastrostomy were esophageal and head and neck malignancies, respectively. Before the PRG procedure, initial C-arm CBCT…

Percutaneous endoscopic gastrostomy (PEG) - An useful 'surgical' measure.

Eight patients were studied to evaluate the efficacy of a surgeonled percutaneous endoscopic gastrostomy (PEG). Three patients underwent PEG at the time of elective surgery (carcinoma larynx-2, carcinoma tonsil-1), two underwent placements during emergency surgery for neck trauma (blunt injury-1, penetrating injury-1) and three for palliation (recurrent tongue carcinoma-1, recurrent epilaryngeal carcinoma-1, metastatic neck nodes-1). For patients undergoing intraoperative PEG, the operative time was prolonged by an average of 12 minutes. There were no major procedural or feedingrelated complic…

One-trocar laparoscopy-aided gastrostomy in handicapped children.

BACKGROUND/PURPOSE: Percutaneous endoscopic gastrostomy has become a common technique to provide nutritional support to handicapped children with swallowing disorders; however, this technique is sometimes associated with serious complications. We report a novel method of 1-trocar laparoscopy-aided gastrostomy with special reference to its influence on gastroesophageal reflux (GER). METHODS: The subjects consisted of 22 profoundly handicapped children, aged from 1 to 14 years (median, 7 years). Twenty-four-hour esophageal pH monitoring was conducted preoperatively in all subjects and performed…

Peritonitis following percutaneous gastrostomy in children: management guidelines.

BACKGROUND: To establish the incidence, timing and outcome of peritonitis following percutaneous gastrostomy (PEG) insertion in children. METHODS: Patients developing peritonitis after PEG insertion during a 5-year period (1990-95) were identified. Variables analysed included clinical presentation, management, operative findings and outcome. RESULTS: One hundred and twenty paediatric patients received 130 PEG in the 5-year period. Eight children developed peritonitis: 4 within 24 h of PEG insertion and 4 following routine PEG tube change (3-18 months later). All four patients developing earl…

How to avoid complications during percutaneous endoscopic gastrostomy.

Percutaneous endoscopic gastrostomy (PEG) has advantages over open gastrostomy, and is an accepted technique for children. However, a number of technical problems may be encountered during insertion. This report identifies those problems and proposes precautions that can be taken to reduce their likelihood. Recognized major complications include esophageal injury, colonic perforation, wound infection, gastric erosion by the gastrostomy tube, and later symptomatic gastroesophageal reflux requiring correction by fundoplication.

Percutaneous endoscopic gastrostomy: the Heidelberg Repatriation Hospital experience.

Percutaneous endoscopic gastrostomy (PEG) was performed on 32 patients (mean age 75 years) who were dysphagic but enteral alimentation was possible. Seventeen patients were recovering from a stroke; the interval between the onset of stroke and PEG averaged 44 days. The procedure was successful and well tolerated by 16 of these 17 patients. Ten patients (31%) still had a functioning PEG, a median of 30 weeks after placement. Seven patients whose swallowing recovered had their tubes removed an average of 3 months after their insertion. Fifteen patients (47%) subsequently died from their underlyi…

Fluoroscopically guided percutaneous gastrostomy: is gastric fixation necessary?

In this animal study the authors evaluated the safety of inserting gastrostomy catheters percutaneously without the aid of gastric fixation. In a laboratory model (excised dog stomachs) there was not leakage around gastrostomy tubes of different sizes (nos. 8 to 18 French), despite maximum gastric distension and intragastric pressures of up to 100 mm Hg. When the tubes were removed, the risk of leakage from the gastrostomy increased in proportion to the size of the gastric defect. Percutaneous gastrostomy catheters (no. 14 French) were then placed in five live dogs without gastric fixation and…

Percutaneous endoscopic gastrostomy feeding tube in neuroscience patients.

Despite a few associated complications, the PEG is considered an excellent feeding tube for long-term enteral nutritional support, especially in the patient with neurologic dysfunction. The tube can be easily removed if the patient is able to resume oral intake and no longer needs supplemental nutrition. This tube can be easily converted to a gastro-jejunal tube if it is later determined a patient needs to be fed distal to the pylorus. Neuroscience nurses should be aware of this feeding tube, its indications, method of insertion and care in order to better provide adequate and safe nutritional

Fatal and disastrous complications following percutaneous endoscopic gastrostomy.

Percutaneous endoscopic gastrostomy (PEG) has become the preferred method of nutritional support in virtually every patient in whom this procedure is technically feasible because of its apparent technical facility, cost containment, and bedside insertion. PEG can, however, be associated with serious complications and death. This is a report of three patients who developed life-threatening abdominal wall abscesses and four patients who died after PEG insertion. The patients ranged in age from 30 to 80 years, four female and three male. Complicating medical conditions included cirrhosis, diabete…

Internal cross-piece recovery after percutaneous endoscopic gastrostomy.

To retrieve the internal cross-piece during removal of a Ponsky-Gauderer PEG, we have sutured the cross-piece to the end of the gastrostomy catheter prior to its insertion endoscopically. As the tube is withdrawn, the internal cross-piece slides off the catheter and remains in the stomach. The cross-piece is then removed by pulling on the suture connecting it to the gastrostomy tube. This simple and safe modification eliminates any risk of gastrointestinal obstruction secondary to a retained cross-piece.

Gastrostomy Sequence With Ventriculoperitoneal Shunting-Does It Matter?

BACKGROUND: Neurological injury requiring ventriculoperitoneal shunt (VPS) placement often necessitates gastrostomy for nutritional support. The sequence of these procedures is debated over concerns for shunt infection and displacement requiring revisional surgery as a consequence of gastrostomy. OBJECTIVE: To determine the optimal sequence of VPS shunt and gastrostomy tube placement in adults. METHODS: In an all-payer database, adult patients undergoing gastrostomy and VPS placement were identified within 15 days between January 2010 and October 2021. Patients were categorized as receiving …

Complications in patients with neurological impairment after gastrostomy.

BACKGROUND: Neurological impairment (NI) is responsible for most conditions that require a permanent gastrostomy tube. The present study assessed the occurrence of short- and long-term complications after video-assisted gastrostomy (VAG) in patients with NI. METHODS: The incidence of short- (<6 months) and long-term (over 2 years) complications of VAG were analyzed in a retrospective study. The differences between the incidence of the complications of VAG according to the age at surgery (≤15 years vs. ≥16 years) were also evaluated. The short- and long-term complications observed were granula…
