How to try this: detecting delirium.

For patients and their loved ones, delirium can be a frightening experience. A fluctuating mental status is important to identify because it often signals a need for additional treatment. The Confusion Assessment Method (CAM) diagnostic algorithm enables nurses to assess for delirium by identifying the four features of the disorder that distinguish it from other forms of cognitive impairment. It can be completed in five minutes and is easily incorporated into ongoing assessments of hospitalized patients. (This screening tool is included in the series Try This: Best Practices in Nursing Care to

Epidemiology and Prevention of Healthcare-Associated Infections in Geriatric Patients: A Narrative Review.

Demographic studies show that life expectancy is increasing in developed countries; increased longevity has also increased the share of the older population with often concomitant chronic conditions. An ageing population and increased comorbidities lead to more complex pharmacological therapies (polypharmacy). The particular picture provided by chronic conditions and polypharmacy can lead to longer hospital stays and a greater need for healthcare. Elderly patients are identified as being in the high-risk group for the development of healthcare-associated infections (HAIs) due to the age-relate

[Dementia disorders, behavior problems and the care of clients in geriatric day-care compared to residents in homes for the elderly: a cross-sectional study in eight communities in Baden].

AIM: The study aims to compare clients using institutions of geriatric day-care to residents in homes for the elderly with regard to functional impairment, dementia disorders, behavior problems and the care situation. METHODS: A cross-sectional study of 17 geriatric day-care facilities in eight towns and cities in Baden examined the data for all 257 clients who received care on a given reference date. These clients were compared to a reference population drawn from all residents (N = 1,387) of 15 randomly selected residential and nursing homes in the city of Mannheim, whereby identical assess

Correlates of probable alcohol abuse among women working in nursing homes.

AIMS: The purpose of this nationwide study was to assess the prevalence of probable alcohol abuse (PAA) among women working in geriatric care, and to study its demographics, medical and work-related correlates. METHODS: The employees of geriatric nursing homes and geriatric hospital wards in Iceland with 10 patients or more were invited to participate in this cross-sectional study. The response rate was 80% (n=1515), with 96% being women (n=1432). Men were consequently omitted from the study. Questions were included on demographics, psychosocial factors, workplace environment, health behavior

[Components of geriatric assessment and therapeutic adherence in elderly patients with acute myocardial infarction].

BACKGROUND AND OBJECTIVE: Poor therapeutic adherence after acute myocardial infarction (AMI) can lead to early serious complications. Information on the impact of geriatric assessment on adherence is scarce. The objective of this study was to analyze, in older patients with AMI, the impact of geriatric assessment on therapeutic adherence 12 months after admission. MATERIALS AND METHODS: A previous study randomized patients aged >75 years who had presented an AMI to a nursing health education program versus conventional management, evaluating the impact of this intervention on therapeutic adhe

Orthogeriatric care-outcome of different fragility fractures.

INTRODUCTION: Fragility fractures (FF) are associated with increased morbidity and mortality and reflect a dramatic turning point in the life of older adults. The scientific discourse is dominated by proximal femoral fractures, but FF affect multiple parts of the body and often precede hip fractures. Orthogeriatric co-management has multiple shown to improve patient's outcome. We hypothesize that all geriatric patients with FF benefit from orthogeriatric co-management. MATERIALS AND METHODS: We retrospectively evaluated all patients over 70 years with FF (hip joint, periprosthetic, spine, pel

Implementing Assessment of Cognitive Function and Frailty Into Primary Care: Data From Frailty and Alzheimer disease prevention into Primary care (FAP) Study Pilot.

BACKGROUND: Aging can be affected by frailty and chronic diseases causing physical, cognitive, sensory, and functional decline evolving gradually to disability. The assessment of older patients is carried out in some geriatric day hospitals (GDHFs). However, it seems difficult to assess all patients in these GDHFs. In this context, a care model, which uses a specialist nurse trained in primary care and geriatric assessment, has been developed. In this article, we describe the organization, details of the evaluation, and provide the main characteristics of the first 200 patients assessed over a

Dysphagia and aspiration pneumonia in older adults.

PURPOSE: To review the risks, diagnosis, management, and prevention of aspiration pneumonia in adults over age 65, and discuss the major role of dysphagia as a risk factor in this population. DATA SOURCES: A comprehensive search of the literature using PubMed (MedLine) and CINAHL. CONCLUSIONS: Aspiration pneumonia is a common diagnosis in older adults and is associated with significant morbidity and mortality. Oropharyngeal and esophageal dysphagia, often related to stroke, dementia, poor oral hygiene, or multiple chronic illnesses, increases the risk of aspiration. Nurse practitioners (NPs)

What is the impact of professional nursing on patients' outcomes globally? An overview of research evidence.

BACKGROUND: Nursing is an integral part of all healthcare services, and has the potential of having a wide and enduring impact on health outcomes for a global ageing population. Over time nurses have developed new roles and assumed greater responsibilities. It is increasingly important to demonstrate the safety and overall impact of nurses' practice through research, to support the case for greater investment and development of nursing services around the world. OBJECTIVE: To provide an overview of existing research evidence on the impact of nursing on patient outcomes, identify gaps in evide

Delirium in Nursing Home Residents: A Narrative Review.

Delirium is an important component of the geriatric syndromes and has been recognized to negatively influence the prognosis of older people in hospital and in a post-acute setting. About 2-5% of older people world-wide live in nursing homes and are characterized by functional impairment, cognitive decline, dementia, comorbidities, and polypharmacotherapy, all factors which influence the development of delirium. However, in this setting, delirium remains often understudied. Therefore, in this narrative review, we aimed to describe the latest evidence regarding delirium screening tools, epidemio

Acute pain management in the older person.

The proportion of older people in society is growing steadily. This particular group is more likely to be admitted to hospital and a number of papers have highlighted the inadequate or inappropriate assessment and management of their pain, particularly in the perioperative period. Pain relief is possible for the majority of people. This paper aims to give an overview of the assessment and management of pain in older people.

Age or Frailty: What Predicts Outcomes in Geriatric Patients With Rib Fractures?

INTRODUCTION: Age and frailty are associated with poor outcomes in geriatric patients with rib fractures. To date, no studies have assessed the relative and independent association of each factor using a large national dataset. We therefore sought to determine whether age, frailty, or their combined consideration most effectively predicts outcomes in geriatric patients with rib fractures. METHODS: The 2017-2023 TQIP database was queried for all geriatric patients (age ≥65 y) who sustained at least one rib fracture. A multivariate analysis with propensity score matching was performed to examin

Associations of Various Health-Ratings with Geriatric Giants, Mortality and Life Satisfaction in Older People.

Self-rated health is routinely used in research and practise among general populations. Older people, however, seem to change their health perceptions. To accurately understand these changed perceptions we therefore need to study the correlates of older people's self-ratings. We examined self-rated, nurse-rated and physician-rated health's association with common disabilities in older people (the geriatric giants), mortality hazard and life satisfaction. For this, we used an age-representative population of 501 participant aged 85 from a middle-sized city in the Netherlands: the Leiden 85-plus

[The human dignity of elderly people admitted to geriatric wards].

A testament to longevity, old age is a time of great vulnerability. An ageing and ailing body often weakens the immune system and leaves a person susceptible to illness. Ageing therefore presents a challenge for the medical profession, which addresses it through a specialised branch of medicine: geriatrics. In this field of medicine for older people, which focuses on health and quality of life, the medical profession has both positive and negative legal obligations aimed at protecting dignity.

Cultural Humility, Nursing Students, and Aging Adults: A Descriptive Phenomenological Study.

This study explored how engagement with older adults promotes the development of cultural humility in prelicensure nurses. Using a descriptive phenomenological approach informed by Mezirow's transformative learning theory and Kolb's experiential learning theory, nine nursing students were interviewed following their volunteer experience at an international senior athletic competition. Four themes emerged: reframing aging, patient-centered connection, dismantling power dynamics, and shifting from medicalization to holistic care. Findings suggest that meaningful interactions with older adults fo

Methadone maintenance: its future in skilled nursing facilities.

The number of geriatric residents in skilled nursing facilities (SNF) receiving methadone maintenance is likely to increase as the drug abusing population ages. Currently, facilities specializing in the care of human immunodeficiency virus (HIV) diseases are predominantly affected, but over the next decade this will change. Residents on methadone have unique needs based on their drug history and current treatment demands and will present new challenges for the affected institutions. There are no current publications or data in the literature exploring the use of methadone for the aging nursing
