The clinical, pathologic and cytologic findings were correlated in 86 cases of metastatic germ-cell tumors. Although the cytologic features of malignant germ-cell tumors are sufficiently characteristic to make specific cytologic diagnosis possible, the diagnostic accuracy can be augmented with cytochemical stains. It was found that due to recent advances in therapy, cytologic detection of metastases does not necessarily indicate a fatal outcome.

Twenty-eight patients with primary malignant germ cell tumors (GCT) of the mediastinum were treated at the University of California at Los Angeles and The Johns Hopkins Hospital in the past 30 years. Of 11 patients with pure seminomas, nine (82%) are free of disease from 6 months to 15 years following therapy. Fifty-five per cent of patients presenting with testicular tumours in 1973--78 had teratomata. A history of undescended testis was only found in patients with seminoma. The right testis was the more frequent site of the primary. Radiation doses of 3400 cGy (rad) in 20 fractions for seminoma and 4500 cGy in 25 fractions for teratoma were well tolerated and sterilised microscopic disease in the majority of patients. Autopsy findings in 20 cases of clinical extragonadal germ-cell tumors arising in the anterior mediastinum are presented. The histologic types were: 2 choriocarcinomas, 3 seminomas, 3 malignant teratomas, 3 embryonal carcinomas, and 9 mixed germ-cell tumors. Germ-cell malignant tumours occurred in a man and his son. The father, who had a teratoma of the right testicle removed 24 years ago, is presently alive and well. The son, who had a choriocarcinoma presenting as an abdominal mass, possibly originating in the testicle, died within 7 months of the diagnosis with metastases in the lungs, liver and retroperitoneum. This report documents the third such case of germ-cell neoplasms occurring in father and son.

INTRODUCTION: Pediatric intracranial germ cell tumors (IC-GCTs) are frequently associated with pubertal dysfunctions, either at diagnosis or during follow-up. This study aims to evaluate the prevalence and types of pubertal disorders in a cohort of pediatric patients with IC-GCTs.
METHODS: We collected clinical, radiological, histopathological, and hormonal data. Pubertal dysfunctions were classified as: (1) pseudo-precocious puberty (PPP, gonadotrophin-independent); (2) central precocious puberty (CPP, gonadotrophin-dependent); (3) hypogonadotrophic hypogonadism (HH).

It consists of distended tubules that may have either intratubular seminoma or intratubular embryonal carcinoma cells. BACKGROUND: Malignant ovarian germ cell tumours typically require multimodal therapy including surgery and systemic platinum-based chemotherapy. Most patients are cured, with survival rates exceeding 95%.
CASE: This report describes an unusual case of ovarian germ cell tumour (GCT) recurring 15 years after surgery and manifesting as metastatic disease to the liver, lung, and retroperitoneal lymph nodes.
CONCLUSION: Thymic hyperplasia was a confounding finding in this case, and it should be considered in the differential diagnosis of a mediastinal mass in heavily treated patients with GCT.

In approximately 25% of patients with testicular germ cell tumor (GCT), the cancer metastasizes to lymph nodes and distant organs. The initial lymphatic drainage site of GCTs is the retroperitoneum.
