Gout is often considered a disease of an excessive lifestyle, a 'malady of kings'. Today, more than 1% of the European and US populations are afflicted with gout, although ulceration over gout tophi remains uncommon. We describe four cases of ulceration associated with gout tophi to highlight the clinical presentation, complications and a management strategy.

A 34-year-old woman, 12 weeks pregnant, presented with an acute monoarthritis. Gout was proven by joint aspiration. The gout was multifactorial in origin. Only 2 cases of gout during pregnancy have been described previously.

A number of pharmacological agents can induce hyperuricaemia, and sometimes gout, usually by interfering with the renal tubular excretion of urate but also in some instances by increasing the formation of uric acid. We report here on 41 male patients with acute polyarticular gout seen in 3 years. Acute polyarticular gout continues to masquerade as other commoner rheumatological disorders such as septic arthritis, rheumatoid arthritis, degenerative joint disease, and even hemiparesis. Almost all of these patients had clues to the diagnosis of acute gout in their medical history. These clues included a past history of intermittent acute gout, prior attacks of polyarticular arthritis, previous hyperuricaemia, and/or obvious tophi. In 2013, much progress has occurred in gout research. Imaging continues to help elucidate aspects of pathophysiology and now suggests that healing of erosions could occur when urate levels are reduced dramatically. New genetic loci associated with hyperuricaemia have been identified and management strategies for prophylaxis of gout flares continue to evolve.
