Early infections in patients undergoing bone marrow or blood stem cell transplantation--a 7 year single centre investigation of 409 cases.

Infections are a major cause of morbidity and mortality in patients undergoing high-dose therapy and subsequent autologous or allogeneic haemopoietic stem cell transplantation, despite the change from topical to systemic anti-infection prophylaxis and the introduction of growth factors and new antimicrobial drugs. We report our single centre experience with data from 409 patients treated at our unit from its opening in 1990 until May 1997. Three hundred and seventy-eight patients were transplanted for the first time, 12 patients were retransplanted or boosted and 19 patients were readmitted fo

Rapid identification of bacteria in blood cultures by using fluorescently labeled oligonucleotide probes.

The applicability of whole-cell hybridization for the identification of pathogenic bacteria in blood from septic patients was examined. Oligonucleotide probes, fluorescently labeled with fluorescein isothiocyanate, directed against the variable regions of the 16S rRNAs of the following bacterial species and/or genera were used: Streptococcus spp., Enterococcus faecalis, Staphylococcus aureus, coagulase-negative staphylococci (CoNS), Escherichia coli, Pseudomonas aeruginosa, and the Enterobacteriaceae family. A probe specific for the rRNAs of almost all bacteria and its complementary, reversed

The Causes of Postoperative Meningitis: The Comparison of Gram-Negative and Gram-Positive Pathogens.

AIM: To determine the microbiological etiology in critically ill neurosurgical patients with nosocomial meningitis (NM) and to show the impact of Gram-negative rods and the differences between patient characteristics and the clinical and prognostic measures in Gram-negative and Gram-positive meningitis. MATERIAL AND METHODS: In this prospective, single-center study, we reviewed all adult patients hospitalized during a 12-year period and identified pathogens isolated from post-neurosurgical cases of NM. Demographic, clinical, and treatment characteristics were noted from the medical records.

Value of routine anaerobic blood cultures for pediatric patients.

OBJECTIVE: Anaerobic bacteremia rarely occurs in children. Therefore we assessed the usefulness of routinely obtaining anaerobic blood cultures in our pediatric patients. STUDY DESIGN: Records of 9360 paired aerobic anaerobic blood culture bottles (Bactec NR660 System) containing blood specimens from pediatric inpatients and outpatients at Duke University Medical Center, Durham, N.C., were reviewed retrospectively. Yield and speed of detection were calculated for each bottle and compared for statistical significance by the McNemar test. RESULTS: A total of 723 clinically important microorgan

The association between penicillin allergy and surgical site infection after orthopedic surgeries: a retrospective cohort study.

BACKGROUND: Cephalosporins are used as first-line antimicrobial prophylaxis for orthopedics surgeries. However, alternative antibiotics are … common pathogen for SSI in our study cohort, while patients with PA had higher infection rate from gram-positive rods and gram-negative rods than non-PA group. CONCLUSION: Compared to patients without PA, patients with PA developed more SSI after orthopedic surgeries, especially deep SSI. The elevated infection rate could be secondary to the use of alternative pro …

The microbiology and outcome of sepsis in Victoria, Australia.

We analysed data from 33741 patients with ICD-10-AM-defined sepsis from an Australian hospital morbidity dataset to investigate the relationships between specific types of organisms, potential risk factors for infection, organ dysfunction, ICU utilization and hospital mortality. A total of 24% of patients received some of their care in an intensive care unit, and the overall hospital mortality rate was 18%. Gram-positive bacteria were isolated in 27% of cases and Gram-negative bacteria in 20%. Sepsis due to Staphylococcus aureus was associated with vascular and joint devices whereas Pseudomona

Septicaemia in an Austrian neonatal intensive care unit: a 7-year analysis.

The results of blood cultures and clinical data of 101 neonates with 110 episodes of septicaemia during a 7-y study period were reviewed. The overall incidence of culture-proven sepsis within the study period was 6.0 per 100 neonatal intensive care unit admissions and the mortality rate was 14%. Three groups of pathogens accounted for 70% of all isolates: coagulase-negative staphylococci (27%), aerobic Gram-negative rods (24%) and Enterococcus faecalis (19%). Group B streptococcus was the major pathogen of very early-onset septicaemia (within 24 h of birth), whereas late-onset infections were

[Gram-negative eubacteria isolated from the water, molluscs and algae of the Black Sea].

Microorganisms strains (384) have been isolated from water, algae and molluscs collected in the Karadag reserve and the village of Katsiveli (Black Sea shore of the Crimea); 81 of the above strains are gram-negative oxidase-positive rods which require sodium ions for their growth. A group of nonfermenting mobile bacteria deprived of arginine dihydrolase have been distinguished among the latter; as to their physiological and biochemical properties and sensitivity to some antibiotics these bacteria were referred to Alteromonas, Marinomonas, Pseudoalteromonas genera.

[Clinico-bacteriological studies on the etiology of bacterial prostatitis. I. The distribution of clinical isolates and bacteria-specific immunoglobulin in EPS].

The distribution of clinical isolates from patients with prostatitis and pathogenicity of the isolates were studied. A new method for measuring the bacteria-specific immunoglobulins in expressed prostatic secretion (EPS) was developed and used for the detection of local immuno-reaction against pathogenic bacteria in the cases with prostatitis. There were no cases with increased antibodies specific for gram-positive cocci (GPC), indicating that the pathogenecity of GPC in bacterial prostatitis was doubtful. On the other hand, specific antibodies against gram negative rods (GNR) were elevated in

Isolation of Corynebacterium aquaticum from spinal fluid of an infant with meningitis.

A 4-week-old female was hospitalized because of vomiting, irritability, and nuchal rigidity. A spinal fluid culture yielded Corynebacterium aquaticum. The diagnosis of C. aquaticum meningitis in this infant was supported by the following cerebrospinal fluid findings: Gram stain, elevated protein, hypoglycorrhachia, positive C-reactive protein, and polymorphonuclear leukocytosis. Antigen studies for common bacterial causes of meningitis were negative. C. aquaticum is a rare cause of human disease and may be initially confused with Listeria monocytogenes, which is a more common gram-positive, mo

Bacillus cereus-induced myonecrosis.

A patient with an incomplete amputation due to a crush injury to his arm developed a myonecrosis with Gram-positive rods noted on muscle and wound aspirates. The patient was treated for a probable Clostridium perfringens infection but culture results proved the organism to be Bacillus cereus. In light of the reported resistance of Bacillus cereus to penicillin, this case serves to emphasize the importance of expanded empiric coverage with high-dose penicillin and an aminoglycoside pending the return of culture and sensitivity results.

Microbial adhesion and biofilm formation on ureteral stents in vitro and in vivo.

Thirty ureteral stents, inserted for 5 to 128 days following extracorporeal shock wave lithotripsy, were examined for the presence of bacterial biofilms. Of these, 90% had adherent pathogens (44% mixed organisms) on the stents, 45% of which were present in low numbers (10(1)-10(2) per 1 cm3 section) and 55% were in small and large microcolony biofilms (> 2 x 10(2)-10(7)). The organisms were recovered from the stents even though urine culture was only positive in 27% of patients. Of the organisms isolated, 77% were Gram positive cocci, 15% Gram negative rods and 8% Candida. No blockage of the s

[Analysis of the microorganisms isolated from kidney recipients treated in the Department of Clinical Transplantology and General Surgery, Medical University in Bydgoszcz in 2001 year].

One of the most important reasons of complications after organ transplantation may be the infections. The aim of the present work was to analyse of microorganisms isolated from patients, which were the recipients for kidney transplantation in 2001 year. The diagnostic material contained 140 samples from 53 patients, 40 (22.2%) samples from Euro-Collins fluid used for kidney storage before the transplantation and 3 end-pieces of catheter. The positive cultures were found in 125 (69.4%) samples. Gram-positive bacteria constituted 58.4%, Gram-negative bacteria--34.2%, fungi--7.4%. 140 strains of

[Antibiotic resistance of anaerobic bacteria (author's transl)].

Reference data on the overall resistance patterns of anaerobic bacteria are needed since in individual clinical cases susceptibility testing is complicated by the long generation time and difficulties of test conditions. Benzylpenicillin and tetracyclines are the drugs of choice against Clostridium perfringens. Newer cephalosporins are also active, whereas other Clostridium species have been found resistant. Beta-lactam antibiotics show high activity against Peptococcaceae; resistance to clindamycin should be noted, however. The good in vitro activity of tetracyclines, clindamycin and erythrom
