Trial Forge Guidance 3: randomised trials and how to recruit and retain individuals from ethnic minority groups-practical guidance to support better practice.

Randomised trials, especially those intended to directly inform clinical practice and policy, should be designed to reflect all those who could benefit from the intervention under test should it prove effective. This does not always happen. The UK National Institute for Health and Care Research (NIHR) INCLUDE project identified many groups in the UK that are under-served by trials, including ethnic minorities. This guidance document presents four key recommendations for designing and running trials that include the ethnic groups needed by the trial. These are (1) ensure eligibility criteria and

Mitral Valve Leaflet Modification: A Working Group Position Statement on Best Practices and Step-by-Step Guide.

Left ventricular outflow tract (LVOT) obstruction is a potentially lethal complication of transcatheter mitral valve replacement (TMVR). The risk for LVOT obstruction can reliably be identified on preprocedural cardiac computed tomography. Intentional LAMPOON (laceration of the anterior mitral leaflet to prevent LVOT obstruction) and related techniques effectively prevent TMVR-induced LVOT obstruction, thereby expanding the treatable patient population. However, perceived technical complexity, the absence of purpose-built devices, and limited training opportunities have hindered the widespread

Aortic Valve Leaflet Modification: A Working Group Position Statement on Best Practices and Step-by-Step Guide.

Coronary artery obstruction is a potential life-threatening complication of transcatheter aortic valve replacement (TAVR) in all settings including native, valve-in-valve, and redo-TAVR. Risk of obstruction can be identified on preprocedural computed tomography. BASILICA (bioprosthetic or native aortic scallop intentional laceration to prevent iatrogenic coronary artery obstruction) effectively prevents TAVR-induced coronary obstruction. Perceived technical complexity, limited availability of purpose-built devices, and scarce training opportunities have hindered the adoption of BASILICA and re

A survey on the institutional practices of the use of isavuconazole in onco-hematological children, on behalf of the Infectious Working Group of AIEOP.

Isavuconazole is a broad-spectrum antifungal agent approved for the treatment of invasive fungal infections (IFIs) in adults and, recently, in pediatric patients aged 1 year and older. Use in the pediatric setting remains limited, but available data indicate generally favorable safety and efficacy. This survey describes the institutional practices of use of isavuconazole across centers belonging to the Italian Association of Pediatric Hematology and Oncology (AIEOP). A 25-item web-based questionnaire, approved by the scientific board of the Infectious Working Group, was distributed to AIEOP ce

Barriers and enablers of elderly care specialists towards deprescribing practices in the nursing homes: a focus group study.

INTRODUCTION: Polypharmacy increases as patients age. It is associated with a decline in quality of life and an increased risk of potentially inappropriate medications (PIMs). These are highly prevalent among long-term care residents in European countries. The potential benefits of deprescribing, as the process of withdrawing PIM’s are widespread, including improvements in both health and quality of life for patients. An overview of the barriers and enablers of deprescribing is still lacking in national research or policy discussions involving a larger group of Elderly Care specialist (ECS), w

Current practices of pain assessment and analgesic use in laboratory mice: A 2022 FELASA Working Group survey.

Assessing and alleviating pain in animals involved in research is critically important. However, the effective implementation of pain management depends on the knowledge and attitudes of the personnel involved. Following a Federation of European Laboratory Animal Science Associations 'Pain in Mice' working group initiative, a questionnaire to survey current practices concerning analgesic use in laboratory mice was distributed to several professional groups in the field of laboratory animal science. Besides demographic data, attitudes to pain and analgesia and sources of information and advice

Evaluation and management of insomnia in the clinical practice in Italy: a 2023 update from the Insomnia Expert Consensus Group.

BACKGROUND: Insomnia is the most reported sleep disorder in industrialized countries, affecting, in the chronic form, around 10% of the European population. In Italy, such a percentage seems to be even higher. Although insomnia can be an independent disorder, it is frequently described as comorbid condition and may precipitate, exacerbate, or prolong a broad range of physical and mental disorders. Evaluating and targeting insomnia in the Italian clinical practice should be a priority. METHODS: The present expert opinions and recommendations represent an update from 2020 and insights from Inso

Securing lethal means for suicide: a focus group study exploring perceptions and barriers among practicing veterinarians.

OBJECTIVE: Veterinarians are at higher risk for suicide than the general population, and 1 reason for this may be veterinarians' access to and knowledge of pentobarbital-a common suicide method in this population. One possible approach to reducing suicide risk is means safety. This study examined the acceptability and feasibility of means safety protocols in the veterinary workplace. SAMPLE: 43 veterinarians from a mix of specialty areas aged 26 to 53 years, currently practicing in the United States, recruited via social media and listservs. METHODS: Participants completed a 60- to 90-minute

A practical guide to optimizing the benefits of post-stroke spasticity interventions with botulinum toxin A: An international group consensus.

This consensus paper is derived from a meeting of an international group of 19 neurological rehabilitation specialists with a combined experience of more than 250 years (range 4-25 years; mean 14.1 years) in treating post-stroke spasticity with botulinum toxin A. The group undertook critical assessments of some recurring practical challenges, not yet addressed in guidelines, through an exten-sive literature search. They then discussed the results in the light of their individual clinical experience and developed consensus statements to present to the wider community who treat such patients. Th

Adapted group-based dialectical behaviour therapy for binge eating in a practicing clinic: clinical outcomes and attrition.

Research evidence has been accumulating for the efficacy of dialectical behaviour therapy (DBT) for binge eating. However, support for its effectiveness and transportability beyond efficacy trials is lacking. The current study evaluated the feasibility of group-based DBT for binge eating within the context of an operating community clinic. Women ages 24-49 (M = 39.60, SD = 9.53) with either subthreshold and full-threshold binge eating disorder or bulimia nervosa formed the group and comprised the sample (n = 5 treatment completers). Positive outcomes included significant improvement in both bi

Laboratory practices for prenatal Group B streptococcal screening and reporting--Connecticut, Georgia, and Minnesota, 1997-1998.

Group B Streptococcus (GBS) is a leading cause of neonatal sepsis in the United States. CDC, in collaboration with the American College of Obstetricians and Gynecologists and the American Academy of Pediatrics, recommends that laboratories adopt optimal screening practices to identify GBS and to promptly report test results so that GBS-colonized pregnant women can receive antibiotics during labor. To assess GBS screening practices in clinical laboratories, state health departments surveyed laboratories in Connecticut, Georgia, and Minnesota, participants in the Emerging Infections Program.
