The "Braids Lady" of Arezzo: a case of rheumatoid arthritis in a 16th century mummy.

OBJECTIVE: To diagnose a probable case of rheumatoid arthritis in a mummified female body from the 16th century and to backdate the first clinical diagnosis, entering the diatribe regarding the ancientness of the disease. METHODS: Image techniques such as normal X-ray, X-ray by mammography, total body CT and high resolution CT were used. Microscopic examination by stereomicroscopy was performed. Samples of tissue were submitted to histology. These data and the review of past literary references, of artistic representations and of paleopathological cases provided an interesting contribution to

Re-enablement of the neurologically impaired hand--2: surgical correction. report of a surgical workshop held at Green Pastures Hospital and Rehabilitation Centre, November 2004, Pokhara, Nepal.

The last half-century has seen tremendous advances in the surgical treatment of neuropathic conditions of the hand. Many of these procedures give clear benefit to patients and have been established by clinical practice and well-formulated assessment. However, much remains to be discovered, especially with respect to which procedures should be used on which hands, and the impact of various procedures on the ability to carry out activities of daily living. Increased communication between the 'West', where funding is available for well designed trials and where reference textbooks are usually pub

Biomechanics of balance in the hand.

The mechanical balance of a hand has nothing to do with having equally strong muscles on opposite sides of each joint. It has to do with the balance of torque at a sequence of joints while the hand is exerting force on external objects that need to be controlled by the hand. The balance may be calculated or seem to be satisfactory when, for example, the tension in finger flexors is sufficient to hold the fingers flexed around an object that is being moved against external force. During such activity balance is not achieved until other muscles such as wrist extensors exert enough tension to hol

[Reconstruction of the thumb and radial side of palm--a report of 6 cases].

Since 1985, on the basis of anatomic study of dorsal pedal artery, thumb and radial side of palm reconstruction have been successfully performed in one stage by using opposite composite dorsal pedal flap including the second toe, the second metatarsal and the first toe web. Because they were all distributed by dorsal pedal artery and great saphenous vein, dissecting these tissues was not difficult. Good function and cosmetic appearance were achieved following reconstruction.

[2-stage operative treatment of Dupuytren contracture].

The incidence of Sudeck-like complications after surgery for Dupuytren's contracture is discussed. In a series of 209 operations for Dupuytren's contracture some complications were observed in 26 cases. In 12 patients these complications could not be precisely explained. In the author's opinion they may illustrate the creation of a nerve-irritating source within the operative field. In order to avoid these complications a two-stage operation was proposed, especially for severe contractures in elderly patients. A preliminary fasciotomy relieves most of symptoms of the disease and may also serve

Wrist deformity, bother and function following wrist fracture in the elderly.

OBJECTIVE: Wrist deformity in older people is common following treatment for a wrist fracture, particularly after non-surgical treatment. A cohort of older wrist fracture patients were surveyed by telephone regarding perceived deformity, bother with deformity and patient-reported wrist function. The objectives were to: (1) determine whether older patients with wrist fractures perceived a deformity of their wrist and if they were bothered by it; (2) test if there were associations between deformity and treatment-type and between deformity and function; (3) test for associations between bother a

Haemophilic arthropathy in the joints of the hands and feet.

Radiographs of the hands and feet of 72 haemophilic patients were reviewed for peripheral joint involvement. Fifty patients or 69% had changes in the small joints of their hands and/or feet with a total of 160 abnormal joints. In the hands the metacarpo-phalangeal joints were predominantly involved (42 of 50 joints), and in the feet, the metatarso-phalangeal joints (68 of 110 abnormal joints), as well as the posterior subtalar joint (36 of 110 joints). Röntgen abnormalities were characterized by irregularity and/or flattening of the articular cortices. The involvement of the small peripheral j

Hand reconstruction.

To conclude, one must apologize for having introduced so many subjects and left so many loose ends. Hand reconstruction poses a multitude of problems which are presently in an evolutionary phase. In discussing the notion of hand surgery specialization one draws upon orthopaedic, plastic and microsurgical techniques. But is it not precisely this adaption of diverse techniques to one organ which constitutes a speciality? Rapid progress in hand surgery was achieved the moment surgeons confined the major part of their activities to the treatment of this organ. But let us repeat that progress impli
