Transplantation for type 1 diabetes mellitus. Whole organ or islets?

Two types of transplants are offered to patients with complicated insulin dependent diabetes mellitus: a) whole pancreas transplantation, b) pancreatic islet transplantation. A total of 29000 whole pancreas transplantations and 1500 islet transplantations have been performed worldwide until today. Patient survival for whole pancreas recipients is 85% five years after transplantation, whereas very few islet studies focus on patient survival. Graft survival for whole pancreas recipients is 90%, 70% and 45%, at one, five and ten years after transplantation respectively. On the other hand, only 44

[PRETAGREF study: Prevalence of tobacco use and factors associated with smoking cessation in kidney transplant recipients].

INTRODUCTION: Tobacco use increases the risk of mortality, cancers and cardiovascular diseases in transplanted patients. Transplanted patients are encouraged to quit tobacco use before and after renal transplantation. This study was carried out to evaluate the prevalence of tobacco consumption in transplanted patients in one French region. This survey was also conducted to identify factors associated with failure in smoking cessation. MATERIALS AND METHOD: A questionnaire was sent by mail to transplanted patients followed in our center between the 1/01/95 and the 31/12/10. A second mail was s

Toe-to-finger combined with free flap transfer for primary one-stage post-traumatic reconstruction of the complex fingerless hand.

Toe-to-finger transplantation is intimately related to the development of microsurgical free tissue transplantation, which is one of the most important advances in the history of reconstructive surgery. It is also generally acknowledged that a mangled hand with massive tissue loss and amputation of multiple digits presents a challenge for plastic and reconstructive surgeons. In this retrospective study we reviewed 11 cases of primary one-stage post-traumatic reconstruction of complex fingerless hands using a combination of toe-to-finger and free tissue transplantation performed in Shanghai Jia

Lymphedema after upper limb transplantation: scintigraphic study in 3 patients.

Lymphatic vasculature is known to spontaneously reconnect after hand replantation. Nonetheless, lymphatic outflow has not been specifically studied in hand transplantation.Lymphedema was studied clinically and scintigraphically in 3 bilateral upper limb transplants performed in Valencia, Spain, since 2006. Case 1 was a radiocarpal level, case 2 midforearm and proximal forearm, and case 3 was a transhumeral transplantation. Follow-up was 5, 4, and 3 years, respectively. Clinically, in case 1, there was a left-sided moderate lymphedema, case 2 was normal, and a right-sided moderate lymphedema wa

Bilateral Proximal Forearm Transplantation: Case Report at 7 Years.

BACKGROUND: Although return of function has been reported in patients undergoing proximal forearm transplantations (PFTs), reports of long-term function are limited. In this study, we evaluated the clinical progress and function 7 years postoperatively in a patient who underwent bilateral PFT. CASE PRESENTATION: A 58-year-old man underwent bilateral PFT in May 2012. Transplantation involved all of the flexor and extensor muscles of the forearm. Neurorrhaphies of the median, ulnar, and radial nerves were epineural and 7 cm proximal to the elbow. Immunosuppressive maintenance medications during

Perioperative ischemic injury and fibrotic degeneration of muscle in a forearm allograft: functional follow-up at 32 months post transplantation.

Allografts of the forearm are still uncommon in the field of composite tissue allograft transplantation. In November 2007, a right-hand allograft and a left-hand/full-length forearm allograft were transplanted in a 30-year-old man who lost both hands and the vision in his left eye due to an explosion. The patient underwent alemtuzumab and steroid induction therapy. Tacrolimus, mycophenolate mofetil, and low doses of steroids were given to prevent rejection. The allografts were rejected 3 times, but these episodes were controlled successfully. The immunosuppressive regimen was switched from tac

[Liver transplantation: deciding between need and donor allocation].

Liver transplantation represents an established therapeutic option for advanced liver diseases. The spectrum of indications encompasses infectious, vascular, immunological and toxic diseases leading to cirrhosis, in addition to genetic, metabolic, developmental and selected neoplastic diseases. On the one hand the timing of liver transplantation is determined by the disease specific course until decompensation and the disease manifestation involving bile ducts or hepatocytes. On the other hand it represents gene therapy of diseases affecting the liver, or entities where the genetic defect lies

The contributions of microvascular surgery to emergency hand surgery.

Microvascular surgery has revolutionized the field of emergency hand surgery in the areas of replantation and microvascular transplantation. Amputated parts from as small as a distal phalanx to the entire extremity can now be successfully replanted and unreplanted, or missing parts can be restored and reconstructed with immediate microvascular transplants (MVTs). MVTs can be used to cover exposed vital structures and replace missing complex parts. Part or whole digits can be reconstructed, with up to 4 toe transplants. Joints, bones, and muscles can be replaced with osteocutaneous and neuromyo

Vascularized Composite Hand Allograft Procurement and Preparation for Distal and Proximal Forearm Allotransplantation: A Stepwise Approach.

Upper limb amputations represent a real medical and surgical challenge. The ideal treatment should restore function, sensation, and body image. At present, neither traditional reconstructions nor prostheses meet all these criteria. However, vascularized composite allografts offer a unique option for restoring form and function satisfactorily despite harmful immunosuppression. Ideally, donor tissue is procured in excess to repair without tension. The donor upper limb is procured through a fish-mouth incision at the mid-arm level. Then, the brachial artery and vein, median, ulnar, and radial ner

[New tendencies in hand surgery].

The author summarizes the new therapeutic tendencies in hand surgery at the past one and a half decade. He discusses the development of hand surgery, as an independent field, in a form of a short historical summary, then he reviews in detail new therapeutic methods considered important such as rehabilitation procedures after tendon injuries, present position of complete hand transplantation, new operations of regeneration of the injured skin and repair of nerve damages, as well as the conservative therapeutic options of Dupuytren's disease. Finally he outlines the modified, new operative proce

[Second toe transplantation in loss of a long finger. Indications--planning--technique].

Transplantation of the second toe is a routinely employed method in reconstructive hand surgery. Most often it is used for thumb or midhand amputations. Following partial amputations of digits distal to the MP-joints, toe transplantations are less frequently employed. However, function as well as cosmesis of the hand after partial amputation of digits can considerably be improved by toe transplantation. The length of the reconstructed finger is a most important aspect of operative planning which has influence on operative technique as well as functional and aesthetic results. A smooth junction

[CARDIAC TRANSPLANTATION: REVIEW OF MANAGEMENT ISSUES AND CURRENT CHALLENGES].

Despite advances in pharmacological treatment of chronic heart failure (HF) as well as utilization of mechanical assist devices, long-term morbidity and mortality of HF patients remain high and many of these patients progress to end-stage HF. Heart transplantation has become the treatment of choice for selected patients with end-stage HF. However, challenges still exist. While the number of heart transplant candidates is increasing, the number of donors remains constant and low. Moreover, transplant candidates become older with more complex co-morbidities are at higher risk for complications s

Targeted stem cell transplantation strategies in ALS.

Amyotrophic lateral sclerosis (ALS) is a devastating neurodegenerative disorder that results in paralysis and ultimately death due to respiratory failure. Transplantation of neural precursor cells (NPCs) derived from the central nervous system is a promising therapeutic strategy for treatment of brain and spinal cord disorders such as ALS. ALS is a particularly challenging disease for designing relevant therapies, and presently no effective treatment exists. Despite such daunting challenges, a number of the potential benefits of NPC transplantation coincide with the neuropathological obstacles

[Fifty years of plastic surgery in the Netherlands. VII. Hand surgery].

The first and second world wars were responsible for a turbulent development of surgery of the hand. The sixties and seventies were marked by the emergence of microsurgery, growing of the knowledge of functional anatomy and use of vascularized skin flaps and joint prostheses. These days hand surgery is mainly concentrated in 'hand centres', with co-operation of a plastic surgeon, physical and occupational therapists and rehabilitation specialists. In the future much can be expected from the genetic and metabolic manipulation of congenital malformations and acquired deformities.

Risk factors for reexploration in toe-to-hand transfer: a multivariate analysis of 363 cases.

BACKGROUND: Reexploration after toe-to-hand transfer increases the risk of failure. In this study, the authors aimed to identify risk factors for reexploration after toe-to-hand transfer. METHODS: The medical records of 363 patients who underwent single toe-to-hand transfer between 2000 and 2010 were reviewed retrospectively. Patient demographics, clinical data, and operative data were collected, and multivariate analysis was performed. RESULTS: Of these 363 patients, 299 toes (82.4 percent) had primary success in transplantation. Sixty-four toes required reexploration, of which seven toes f

[Peromelia].

Peromelia is a rare but mutilating congenital deformity representing failure of formation of parts of the upper limb. The deformity is presented unilaterally in the vast majority of the cases. Usually, these children are totally healthy beside from the deformity and are not seriously handicapped by the deformity. Correction of the deformity by hand- or forearm transplantation is not yet possible. Treatment consists of support with prosthesis in case of missing upper- or forearm and construction of fingers by toe-to-hand transplantation in case of missing fingers and hand. Treatment is not mand

[Microsurgical correction of the congenital adactylous hand].

The correction of different kinds of congenital malformations of the hand by means of microsurgical autotransplantation of toes onto the hand was performed to 17 patients. The main purpose of the operation was to restore the hand function, to improve the esthetic appearance of the hand which must be solved in parallel, but is not an indication to operation. Tactics of operation for transplantation of toes onto the hand must be different for different congenital defects and dependent on the function of the 1st finger. Of great importance is the question whether the operation will give an additi
