Operations research to add postpartum family planning to maternal and neonatal health to improve birth spacing in Sylhet District, Bangladesh.

BACKGROUND: Short birth intervals are associated with increased risk of adverse maternal and neonatal health (MNH) outcomes. Improving postpartum contraceptive use is an important programmatic strategy to improve the health and well-being of women, newborns, and children. This article documents the intervention package and evaluation design of a study conducted in a rural district of Bangladesh to evaluate the effects of an integrated, community-based MNH and postpartum family planning program on contraceptive use and birth-interval lengths. INTERVENTION: The study integrated family planning

Planning Staff and Space Capacity Requirements during Wartime.

Determining staff and space requirements for military medical centers can be challenging. Changing patient populations change the caseload requirements. Deployment and assignment rotations change the experience and education of clinicians and support staff, thereby changing the caseload capacity of a facility. During wartime, planning becomes increasingly more complex. What will the patient mix and caseload volume be by location? What type of clinicians will be available and when? How many beds are needed at each facility to meet caseload demand and match clinician supply? As soon as these fac

A Short Message Service (SMS) increases postpartum care-seeking behavior and uptake of family planning of mothers in peri-urban public facilities in Kenya.

BACKGROUND: It is estimated that one third of maternal deaths in Kenya in 2014 could have been prevented by more timely care-seeking. Mobile health interventions are increasingly being recognized as tools for the delivery of health education and promotion. Many maternal deaths occur in the first few weeks after delivery and mothers who are given adequate care in the postpartum period have better health outcomes. Kiambu County, Kenya has a high level of literacy and phone ownership amongst mothers delivering in public hospitals and was chosen as a site for a postpartum short message service int

Engaging adults with severe mental illness in research: considerations and practical recommendations for meaningful patient and public involvement.

BACKGROUND: Patient and public involvement (PPI) in mental health research is essential but presents unique challenges, particularly when engaging adults with severe mental illness (SMI). This report focusses on the process, benefits, and challenges of involving adults with lived experience of SMI as active partners within a five-year research programme aimed at co-producing a new evidence- and theory-based intervention to support smoking behaviour change following mental health inpatient care. It summarises our PPI strategy, the lived experience insight provided, and practical recommendations

Beyond Risk Identification: Adherence to a Pediatric Suicide Prevention Pathway in Primary Care.

OBJECTIVE: To evaluate adherence to a structured suicide prevention pathway in pediatric primary care and characterize management practices following suicide risk assessment. METHODS: We conducted a retrospective cross-sectional study of well-child visits among adolescents aged 12-21 years at an urban academic pediatric clinic from 2022-2024. The pathway incorporated the Patient Health Questionnaire-9 Modified for Adolescents (PHQ-A) for suicide risk identification and the Columbia-Suicide Severity Rating Scale (C-SSRS) for risk assessment. Electronic health record (EHR) data assessed adheren

Behavior change techniques in a care partner-assisted oral health intervention for people with mild dementia: a secondary mixed-methods analysis.

BACKGROUND AND OBJECTIVES: Oral hygiene declines early in mild dementia (MD), yet few interventions specify which behavior change techniques (BCTs) drive improvement. We examined the use of BCTs in relation to oral health outcomes in a care-partner-assisted intervention for persons living with MD. RESEARCH DESIGN AND METHODS: Using a secondary convergent mixed-methods design, we analyzed the coaching arm of a three-arm randomized trial. Seventeen dyads (persons living with MD-care partners) completed four coaching sessions (68 coaching session transcripts). Seventeen BCTs were specified for u

Adolescent contraception: survey of attitudes and practice of health professionals.

In Nigeria, there is a persistent reluctance to provide adolescents with contraceptives and this is based largely on the premise that the culture does not support pre-marital sexual activity. However, several studies have revealed that a large number of adolescents are experiencing early sexual debut, some as early as 13 years. Furthermore, mortality resulting from termination of unwanted pregnancies is on the increase among Nigerian adolescents. The aim of this study is to assess the attitudes and practice of health workers in Ibadan regarding adolescent contraception. A structured questionna

Urban-rural differences in caregivers' willingness to support adolescent girls' contraceptive use in Ghana: a comparative study.

Adolescent pregnancy remains a critical public health challenge in Ghana, accounting for a significant proportion of unintended pregnancies, unsafe abortions, and adverse health outcomes. This study explored urban-rural differences in caregivers' willingness to support adolescent girls' contraceptive use, focusing on Awutu Senya East (urban) and Jaman North (rural) districts. A cross-sectional comparative design was employed, involving 366 caregivers. Data were collected using structured surveys and analyzed through descriptive, comparative, and modified Poisson regression techniques. Findings

Challenges to access and provision of palliative care for people who are homeless: a systematic review of qualitative research.

BACKGROUND: People who are homeless or vulnerably housed are a marginalized group who often experience high rates of morbidity and die young as a result of complex problems. Access to health care and support can be challenging, with access to palliative care even more so. This review presents a synthesis of published qualitative research exploring from the perspective of homeless people and those working to support them, current challenges to palliative care access and provision, in addition to suggestions for what may improve palliative care for this population. METHODS: Systematic review of

Contraceptive security, information flow, and local adaptations: family planning Morocco.

BACKGROUND: Many developing countries increasingly recognize and acknowledge family planning as a critical part of socio-economic development. However, with few health dollars to go around, countries tend to provide essential drugs for curative care, rather than for family planning products. Donors have historically provided free contraceptives for family planning services. Whether products are donated or purchased by the country, a successful family planning program depends on an uninterrupted supply of products, beginning with the manufacturer and ending with the customer. Navigating spatial barriers in healthcare: health equity and end-of-life care infrastructure in South Korea and China.

INTRODUCTION: East Asia is experiencing an unprecedented demographic transformation, with rapidly aging populations creating an urgent public health demand for accessible hospice and palliative care facilities. However, infrastructural planning regulations may implicitly marginalize these essential health services as stigmatized facilities, creating regulatory and institutional conditions that can constrain community-adjacent access to end-of-life care. METHODS: This study conducts a structured comparative legal and health policy analysis of facility-siting frameworks in South Korea and China

Supporting the carers of people with dementia.

Dementia is a progressive neurological syndrome. Because of its complexity and longevity, dementia can have a negative effect on the health of informal carers, including spouses, family members and friends. Carers of people with dementia are at increased risk of depression and some long-term conditions; however, there are interventions that nurses can implement to support them. This article reviews the effect that caring has on carers of people with dementia, while exploring how nurses can provide optimal support through care planning, signposting to external services and understanding the imp

Improving support and planning ahead for older people with learning disabilities and family carers: a mixed-methods study.

BACKGROUND: People with learning disabilities are living longer. Despite government policy to encourage people to lead supported lives in their community, family carers often maintain support due to dissatisfaction with services. This can lead to people moving from the family home in a crisis. OBJECTIVES: (1) Find out what is known about health needs and resources for older people with learning disabilities (aged ≥ 40 years); (2) identify exemplars of good services for older people with learning disabilities; (3) explore service exemplars through ethnographic case studies; (4) evaluate suppor

How Does an Unplanned Pregnancy Affect Maternal Stress and Depression From Pregnancy to 12 Months Postpartum? A Longitudinal Study.

BACKGROUND: Research studies have shown an increased prevalence of mental health issues in women with unplanned pregnancies. However, many of these studies use cross-sectional methods. This particular study aimed to examine perceptions of stress and depression in women with unplanned pregnancies compared with those with planned pregnancies. The study tracked these perceptions, from the early stages of pregnancy to 12 months postpartum. METHODS: In this quasi-experimental study, pregnant women attending health centers completed a baseline questionnaire around the fifth month of pregnancy. This
