Women's costs for accessing comprehensive sexual and reproductive health services: findings from an observational study in Johannesburg, South Africa.

BACKGROUND: Evaluating progress towards the Sustainable Development Goal of universal access to sexual and reproductive (SRH) services requires an understanding of the health needs of individuals and what constitutes access to services. We explored women's costs of accessing SRH services in Johannesburg, South Africa and contextualized costs based on estimates of household income. METHODS: We conducted an observational study of women aged 18-49 at a public HIV treatment site and two public primary health care facilities from June 2015 to August 2016. Interviews assessed women's SRH needs (for

Supporting children and adolescents who have experienced sexual abuse to access services: Community health workers' experiences in Kenya.

UNLABELLED: Child sexual abuse (CSA) is a global health problem with significant health consequences. The World Health Organization recommends immediate and long-term treatment for all survivors. However, in low- and middle-income countries, less than 10 % of sexually abused children seek health services. Community health workers (CHWs) can potentially increase uptake of services, but, the risks and benefits of services provided by CHWs are poorly understood. METHODS: Through in-depth interviews, we examined the experiences of CHWs providing services to children in Kenya. Sixteen CHWs were pu

Young Adults' Access to Insurance Through Parents: Relationship to Receipt of Reproductive Health Services and Chlamydia Testing, 2007-2014.

PURPOSE: Adolescents' concerns about confidential service receipt have been linked to avoidance of sexual and reproductive healthcare. Healthcare system changes allowing young adults to remain on a parent's health insurance plan up to age 26 may have extended these concerns to young adults. This study examines: (1) The association between the relationship of young women to primary health plan policy holder (parent or self) on receipt of reproductive health services and chlamydia screening. (2)Changes, over time, in the proportion of young women who are parentally- versus self-insured. METHODS

What high-risk women are telling us about access to primary and reproductive health care and HIV prevention services.

Focus group discussions on barriers to health care and attitudes toward family planning, reproductive health services, and condom use were conducted with 63 women at high risk for HIV due to their own injection drug use, sex with injection drug users, sex industry work, or a history of multiple sexually transmitted diseases. Barriers identified include the high cost of health care, perceived poor quality of care and experiences of discrimination and stigmatization, geographic accessibility, fear of legal/social services punitive actions, misperceptions about the efficacy of birth control metho

Bridging Formal Health Services and Community Care for Improved Access to Culturally Appropriate End-of-Life Support for Migrant Communities.

The research aimed to understand the different cultural needs relating to death and dying of migrant populations in Western Sydney, Australia, and how end-of-life care can be provided in culturally safe ways which can, in turn, inform the policies that shape palliative care and bereavement service delivery. The mixed methods design for this study used an online quantitative survey using the Death Literacy Index, key informant qualitative interviews, culture-based community focus groups, and individual Photovoice interviews with carers. In all, 266 participants across the three largest migrant

A mixed-methods evaluation of outreach service provision by the "Strengthening Migrant Access to Reproductive Health in Thailand" Initiative, 2020-2024.

INTRODUCTION: Despite considerable progress, pregnancy-related health outcomes are still below Sustainable Development Goal targets for many low-to-middle-income countries. This study evaluated the Strengthening Migrant Access to Reproductive Health in Thailand (SMARH-T) Initiative that included an outreach service provision (2020-2024) to address upstream determinants of prenatal care and a family planning service provision for undocumented migrant women and newborns along the Thailand-Myanmar border. METHODS: This study employed a sequential explanatory mixed-methods design with a quantitat

Interventions to improve access to sexual and reproductive health services for adolescents living with HIV in sub-Saharan Africa: a scoping review.

OBJECTIVE: Adolescents living with HIV (ALHIV) face barriers to accessing sexual and reproductive health (SRH) services, negatively affecting their health and quality of life. These challenges are particularly pronounced in sub-Saharan Africa (SSA), where HIV prevalence, unintended pregnancies and child marriage remain high. This review aimed to map interventions designed to improve SRH access for ALHIV in SSA, applying Levesque's framework to identify facilitators and barriers from supply-side and demand-side perspectives. DESIGN: Review followed the Preferred Reporting Items for Systematic

Experiences of Care from Mental Health Services among Partners of Women Accessing Support for Postpartum Psychosis: a Qualitative Study.

Postpartum psychosis (PP) affects one to two per 1,000 women and requires urgent intervention. Whilst PP primarily impacts women, it can place significant strain on partners, who often play a pivotal role throughout postnatal care and recovery. Little is known about how mental health services engage with and support partners during a PP episode. Therefore, this study aims to explore partners' experiences of mental health services to better understand their experiences and needs and inform service provision. Semi-structured interviews were conducted with 12 partners of women who received suppor

The impact of area deprivation on access to and utilization of health services in the last year of life: A retrospective population-based cohort study.

BACKGROUND: The healthcare needs of people living in areas of high deprivation are complicated by the cumulative effect of the sociodemographic factors known to impact on health outcomes, such as income, housing and education. Of note, for people living in more deprived areas, life expectancy is shorter and the onset of chronic disease and multimorbidity occurs much earlier. While the relationship between area deprivation and access to palliative care is becoming more widely researched, the vast majority of studies to date have focused on referrals to specialist palliative care services. This

Financial Assistance Value for Equitable Access to Specialty Medications for Adults With Multiple Sclerosis Using Health System Pharmacy Services.

BACKGROUND: Contributing to the total economic burden of multiple sclerosis (MS) in the United States, the estimated direct medical costs of MS in 2019 were $63.3 billion, with medications accounting for over half of these expenses.1 MS disease-modifying therapy (DMT) costs can limit access, causing patients not to adhere to the medication regimen, which may lead to relapse, progression, disability, hospitalizations, and increased health care costs. Integrated health system specialty pharmacies (HSSPs) provide assistance to patients to alleviate the financial burden of specialty medications. M

Greater need but reduced access: a population study of planned and elective surgery rates in adult mental health service users.

AIMS: Timely access to surgery is an essential part of healthcare. People living with mental health (MH) conditions may have higher rates of chronic illness requiring surgical care but also face barriers to care. There is limited evidence about whether unequal surgical access contributes to health inequalities in this group. METHODS: We examined 1.22 million surgical procedures in public and private hospitals in New South Wales (NSW), Australia, in 2019. In a cross-sectional study of 76,320 MH service users aged 18 and over, surgical procedure rates per 1,000 population were compared to rates

Subsidized Versus Unsubsidized Senior Housing Communities in PA: A Window on Variation in Health, Function, and Access to Services in Old Age.

Introduction: Independent Living residences for older adults can be divided into two categories and require better definitions for research purposes; the purpose of this manuscript is to provide those definitions and explore variation in provided services and resident characteristics: (a) Subsidized age-based housing (55+) (Department of Housing and Urban Development (HUD) housing units for low-income adults), and (b) non-subsidized age-based housing. Methods: Residents in the two settings were compared: 37 subsidized locations (p = 289 residents) and 19 non-subsidized (p = 208). Aging support

11 years of regular access to subsidized veterinary services is associated with improved dog health and welfare in remote northern communities.

Access to veterinary services can have positive impacts on animal health and welfare, and on human mental and physical health and well-being; however, many communities worldwide lack access to such services. At their request, the 5 communities of the Sahtu Settlement Area, Northwest Territories, Canada, have received annual access to preventive veterinary services through the University of Calgary's Northern Community Health Rotation since 2008. To determine the reach of the program, we conducted a dog census in 2017. We then conducted a chart review of 11 years of dog medical records from 200

How hospital practitioners can support sofa surfing adolescents to access community mental health services: An English perspective.

Informal homelessness or, as Centrepoint describe, 'the hidden homeless' includes young people who may sleep on their friends' or extended family's couches or floor. They estimate that 103,000 people aged 16-24 in the United Kingdom presented to their Local Authority in 2017-2018 as being or at risk of being homeless. A proportion of young people who experience homelessness rely on their own resources rather than approaching their Local Authority for support. There are a number of barriers that make it harder for any homeless person to access services to enhance their emotional well-being. If

Strategies to improve access to and utilization of health care services and adherence to antiretroviral therapy among HIV-infected drug users.

We review five innovative strategies to improve access, utilization, and adherence for HIV-infected drug users and suggest areas that need further attention. In addition, we highlight two innovative programs. The first increases access and utilization through integrated HIV and opioid addiction treatment with buprenorphine in a community health center, and the second incorporates adherence counseling for antiretroviral therapy in methadone programs. Preliminary evaluations demonstrated that these strategies may improve both HIV and opioid addiction outcomes and may be appropriate for wider dis
