Three Dimensional Visualization of Human Cardiac Conduction Tissue in Whole Heart Specimens by High-Resolution Phase-Contrast CT Imaging Using Synchrotron Radiation.

BACKGROUND: The feasibility of synchrotron radiation-based phase-contrast computed tomography (PCCT) for visualization of the atrioventricular (AV) conduction axis in human whole heart specimens was tested using four postmortem structurally normal newborn hearts obtained at autopsy. METHODS: A PCCT imaging system at the beamline BL20B2 in a SPring-8 synchrotron radiation facility was used. The PCCT imaging of the conduction system was performed with "virtual" slicing of the three-dimensional reconstructed images. For histological verification, specimens were cut into planes similar to the PCC

Body Surface Electrocardiographic Mapping for Non-invasive Identification of Arrhythmic Sources.

The authors describe a novel three-dimensional, 252-lead electrocardiography (ECG) and computed tomography (CT)-based non-invasive cardiac imaging and mapping modality. This technique images potentials, electrograms and activation sequences (isochrones) on the epicardial surface of the heart. This tool has been investigated in the normal cardiac electrophysiology and various tachyarrhythmic, conduction and anomalous depo-repolarisation disorders. The clinical application of this system includes a wide range of electrical disorders like atrial arrhythmias (premature atrial beat, atrial tachycar

Complete heart block during central venous catheter placement in a patient with pre-existing left bundle branch block.

Heart block and arrhythmia are complications of pulmonary artery and cardiac catheterization. Injury to the conducting system of the heart often involves the right bundle causing right bundle branch block (RBBB). If patients already have left bundle branch block (LBBB), complete heart block (CHB) may result. After trauma, impairment of the right bundle is usually transient with recovery in hours, but complete heart block can lead to symptoms requiring invasive treatment. Similar complications are rare with insertion of central venous catheters, as they should not enter the heart. Injury to the

Post-Transcatheter Aortic Valve Implantation QRS Interval Alternans: What Is the Mechanism?

Alternating bundle branch block is a significant conduction abnormality observed after transcatheter aortic valve replacement. This electrocardiographic finding indicates progressive conduction system disease, often on a background of preexisting conduction disturbances (eg, right bundle branch block). However, the presence of alternating fascicular block is very rare. The presence of alternating fascicular blocks suggests an increased risk for complete heart block, frequently necessitating permanent pacemaker implantation to prevent life-threatening arrhythmias.

Analytical Insights into Ephaptic Coupling and Its Effect on Conduction Velocity.

Cardiovascular disease continues to be the leading cause of death in the United States. A major contributing factor is cardiac arrhythmia, which results from irregular electrical activity in the heart. On a tissue level, cardiac conduction involves the spread of action potentials (AP) across the heart, enabling coordinated contraction of the myocardium. On a cellular level, the transmission of signals between cells is facilitated by low-resistance pathways formed by gap junctions (GJs). Recent experimental studies have sparked discussion on whether GJs play a dominant role in cell communicatio

Cell and gene therapy for arrhythmias: Repair of cardiac conduction damage.

Action potentials generated in the sinoatrial node (SAN) dominate the rhythm and rate of a healthy human heart. Subsequently, these action potentials propagate to the whole heart via its conduction system. Abnormalities of impulse generation and/or propagation in a heart can cause arrhythmias. For example, SAN dysfunction or conduction block of the atrioventricular node can lead to serious bradycardia which is currently treated with an implanted electronic pacemaker. On the other hand, conduction damage may cause reentrant tachyarrhythmias which are primarily treated pharmacologically or by me

A Conduction-Sparing Atrial Patch Placement Technique During Atrioventricular Septal Defect Repair.

BackgroundSurgical repair of atrioventricular septal defect (AVSD) carries a persistent risk of postoperative complete heart block due to the close proximity of the atrioventricular conduction system to the inferior atrial septum during atrial patch placement. We evaluated a modified atrial patch placement technique designed to minimize conduction system injury during AVSD repair.MethodsAll patients undergoing AVSD repair between January 2019 and December 2024 were retrospectively reviewed. Patients were stratified according to the atrial patch placement technique. We evaluated a standard appr

Prolongation of the cardiac QTc interval in Turner syndrome.

Anatomic anomalies of the cardiovascular system occur in approximately 50% of individuals with Turner syndrome (TS), with the specific genetic cause(s) for the heart defects still unknown. Because congenital heart disease may be associated with conduction system abnormalities, we compared electrocardiograms (ECGs) in 100 women with TS and 100 age-matched female controls. Women with TS were significantly more likely to demonstrate left posterior fascicular block (p < 0.005), accelerated AV conduction (p < 0.006), and T wave abnormalities (p < 0.006). The PR interval was significantly shorter (1

Conduction disturbances after transcatheter aortic valve implantation.

Conduction disturbances and permanent pacemaker implantation remain the most common complications after transcatheter aortic valve implantation. The strongest predictors of conduction abnormalities and subsequent permanent pacemaker implantation after transcatheter aortic valve implantation include pre-existing right bundle branch block, a short membranous interventricular septum, deep transcatheter heart valve implantation, and valve type. Importantly, both new permanent pacemaker implantation and new left bundle branch block after transcatheter aortic valve implantation are associated with i

Electrophysiologic delineation of the specialized atrioventricular conduction system in two patients with corrected transposition of the great arteries in situs inversus (I,D,D).

Electrophysiologic delineation of the atrioventricular conduction system at surgery is described in two patients with corrected transpostion of the great arteries in situs inversus. Intra-atrial electrograms were recorded in one patient from sites immediately adjacent to the coronary sinus located in the left-sided right atrium. The intraventricular portion of the atrioventricular conduction system was identified in both patients along the posterior and inferior margin of the ventricular septal defect, in contrast to the superior and anterior location found in corrected transposition of the gr

Atrioventricular Conduction: Physiology and Autonomic Influences.

Atrioventricular (AV) nodal conduction is decremental and very prone to alterations in autonomic tone. Conduction through the His-Purkinje system (HPS) is via fast channel tissue and typically not that dependent on autonomic perturbations. Applying these principles, when the sinus rate is stable and then heart block suddenly occurs preceded by even a subtle slowing of heart rate, it typically is caused by increased vagal tone, and block occurs in the AV node. Heart block with activity strongly suggests block in the HPS. Enhanced sympathetic tone and reduced vagal tone can facilitate induction

Native Mitral Valve Endocarditis Complicated by Intermittent Heart Block With Spontaneous Recovery: A Case Report.

Isolated native mitral valve endocarditis without aortic involvement is a rare but serious cause of complete heart block. In contrast, aortic valve endocarditis is a well-recognized cause of life-threatening conduction abnormalities due to its close anatomical relationship with the conduction system. We report the case of a 68-year-old female patient with Streptococcus viridans bacteremia, presenting with native mitral valve endocarditis complicated by complete heart block, with imaging and clinical findings confirming sparing of the aortic valve.

Wide complex tachycardia in a patient with non-ischemic cardiomyopathy: What is the diagnosis?

A 55-year-old male presented with acute heart failure and incessant wide complex tachycardia resembling an outflow tract ventricular tachycardia. Meticulous analysis of the electrocardiograms established the diagnosis of pre-excitation with prolonged atrio-ventricular (A-V) conduction over a decrementally conducting accessory A-V pathway. "Linking" between the accessory A-V pathway and normal A-V conduction system resulted in sustained maximal pre-excitation as well as periodic transition to normal A-V conduction without appreciable change in heart rate. Successful radiofrequency ablation of t

Epistatic rescue of Nkx2.5 adult cardiac conduction disease phenotypes by prospero-related homeobox protein 1 and HDAC3.

RATIONALE: Nkx2.5 is one of the most widely studied cardiac-specific transcription factors, conserved from flies to man, with multiple essential roles in both the developing and adult heart. Specific dominant mutations in NKX2.5 have been identified in adult congenital heart disease patients presenting with conduction system anomalies and recent genome-wide association studies implicate the NKX2.5 locus, as causative for lethal arrhythmias ("sudden cardiac death") that occur at a frequency in the population of 1 in 1000 per annum worldwide. Haploinsufficiency for Nkx2.5 in the mouse phenocopie
