Somatostatin-containing cells in the extrahepatic biliary tract of humans.

We investigated the presence of endocrine cells in the human extrahepatic biliary system using the avidin-biotin-peroxidase complex method. Twenty gallbladders obtained during operations for cholelithiasis, large portions of three common bile ducts obtained during Whipple's procedure for carcinoma of the head of the pancreas, and 155 endoscopic transpapillary biopsy specimens taken during control endoscopy from patients who had had a previous endoscopic sphincterotomy were examined. Somatostatin-containing cells were found in the glandular epithelium of the ampulla of Vater and the bile ducts.

[Anthropometric and topographic characteristics of the gallbladder in cholecystitis].

Anthropometric studies were carried out upon 228 patients with cholecystitis, and the findings were compared to those obtained in the control group. The location of the liver anterior margin, projection of the gall-bladder fundus, the upper point of the gall-bladder, the site of the fusion of the cystic duct and the hepaticocholedochus versus the spine and the costal arch margin were studied in 174 patients. On the grounds of these studies a classification embracing 9 variants of the gall-bladder position has been suggested together with some variants of oblique incision in the right subcostal

Treatment of congenital dilatation of the choledochus- reconstruction of the bile duct after resection of the dilated choledochus.

Reconstruction of the way of bile flow was attempted in 21 cases with the resection of the dilated bile duct, and the post-operative courses were observed over a long period. It was found that ascending cholangitis is seldom caused by the hepaticoduodenostomy. It is important, when hepaticoduodenostomy is performed, that the sound section of the wall of the bile duct without stenosis in the hepatic side should be selected. Reconstruction of the way of the bile flow to prevent ascending cholangitis by leading normal bile flow into the duodenum should be devised.

Nonanastomotic hepaticogastrostomy for palliative decompression.

A simple, low risk technique has been used in two patients with high malignant obstruction of the common hepatic bile duct for the creation of a biliary enteric fistula.

Hepatobiliary scanning with 99mTc-pyridoxylidene glutamate. A retrospective study investigating the criteria for differentiation between intrahepatic and extrahepatic obstruction.

In 70 99mTc-pyridoxylidene glutamate (PG) studies with verified diagnoses, the following scan patterns were found. (1) Normal: within 30 min of PG injection the scan reveals the liver, hepatic ducts, common bile duct, gallbladder and flow to the intestine; after 2 h the liver had a higher concentration of activity than the hepatic ducts or the common bile duct. (2) Complete extrahepatic obstruction: no hepatic excretion to the intestine is observed 18-24 h after PG injection, nor is activity observed in the hepatic ducts, common bile duct and gallbladder. (3) Incomplete extrahepatic obstructio

Anatomic variations of the right hepatic duct: results and surgical implications from a cadaveric study.

Purpose. Thorough understanding of biliary anatomy is required when performing surgical interventions in the hepatobiliary system. This study describes the anatomical variations of right bile ducts in terms of branching and drainage patterns, and determines their frequency. Methods. We studied 73 samples of cadaveric material, focusing on the relationship of the right anterior and posterior segmental branches, the way they form the right hepatic duct, and the main variations of their drainage pattern. Results. The anatomy of the right hepatic duct was typical in 65.75% of samples. Ectopic drai

Mirizzi Syndrome in a patient with an accessory hepatic duct.

Mirizzi syndrome (MS) represents an uncommon clinical condition, being characterized by a narrowing of the common hepatic duct or its erosion by stones impacted in the cystic duct or gallbladder Hartman's pouch. Very uncommonly, MS can be reported in patients with contemporaneous bile duct anomalies. The case is reported of a 76-year-old Caucasian woman with a MS with a cholecystobiliary fistula and a contemporaneous aberrant biliary duct for the right posterior segments.Due to the presence of an anatomical abnormality, an open approach was decided: also during surgery, it was impossible to cl

Atypical cyst of the right ductus hepaticus.

Cystic dilatation is a rare disease of extra- or intra-hepatic bile ducts. According to the literature it is an disorder accompanying other diseases rather than an independent diagnosis. In the case of smaller dimensions it is often found by coincidence because of its asymptomatic course of development. Mostly it manifests itself through abdominal pain or icterus. The authors show a case report of 14-months old patient with the finding of a cyst in the right ductus hepaticus with extraordinary dimensions (Fig. 2, Ref. 11). Full Text (Free, PDF) www.bmj.sk

[The Mirizzi syndrome--from the first description until today].

Pablo Luis Mirizzi was the first to describe an obstructive jaundice caused by compression of the common hepatic duct by the stone in the cystic duct or the neck of the gallbladder in 1948. McSherry et al in 1982. described a new type of Mirizzi's syndrome calling it type II. Csendes et al in 1989. gave a new classification in four types. According to it, type II of Mirizzi's syndrome was devided in three types depending on the size of the destruction of the common hepatic duct. We previously described a subtype of Mirizzi's syndrome in which besides very wide cholecystohepatic fistula, a comb
