Persistent hiccups following endobronchial ultrasound-guided transbronchial needle aspiration.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive diagnostic procedure widely used in modern pulmonary medicine. Major complications are rare and usually hemorrhagic or infectious, while neurological sequelae are exceptionally uncommon. We report a rare case of persistent hiccups following EBUS-TBNA in a middle-aged man evaluated for fever and necrotic mediastinal lymphadenopathy with a probable diagnosis of tuberculosis or invasive fungal diseases. Despite extensive investigations, including cross-sectional imaging and upper gastrointestinal …

COVID-19 Presenting With Intractable Hiccups: A Literature Review and a New Case.

Hiccups manifest as involuntary and repetitive diaphragm contractions, often involving the intercostal muscles. However, the precise underlying mechanism remains incompletely understood but typically benign. During the COVID-19 pandemic, the predominant clinical presentation featured fever, cough, and dyspnea. However, multiple atypical presentations are increasingly recognized as manifestations of COVID-19, including refractory hiccups. This article aims to examine the shared characteristics, distinctions, notable correlations, and prognosis among COVID-19 patients presenting with intractable…

Chylous ascites and persistent hiccups as complications after laparoscopic radical nephrectomy.

A patient in his mid-40s with a history of gastric ulcer and gastro-oesophageal reflux disease presented with 6 months of left flank discomfort. He was diagnosed with stage III (cT3aN1M0) papillary renal cell carcinoma and received neoadjuvant therapy for 3 months. Follow-up imaging showed partial response with residual carcinoma and metastatic lymphadenopathies. The patient subsequently underwent laparoscopic radical nephrectomy with lymph node dissection. Postoperatively, he developed chylous ascites and subsequent persistent hiccups as unusual complications. Although chylous ascites improve…

Eosinophilic esophagitis manifesting as intractable hiccups in an elderly patient: a case report.

BACKGROUND: Eosinophilic esophagitis is a condition characterized clinically by symptoms related to esophageal dysfunction and histologically by a marked eosinophilic infiltrate in the esophageal mucosa. The most typical symptoms of eosinophilic esophagitis include intermittent dysphagia with episodic food impaction and heartburn with propensity for young individuals. The relationship between hiccups and eosinophilic esophagitis is unclear but has been described. CASE REPORT: We report a case of a 93-year-old Middle Eastern male presenting for longstanding treatment-refractory hiccups. Imagin…

Characteristics of Posterior Cerebral Circulation in Patients With Intractable Hiccups: A Case Series.

INTRODUCTION: Certain vascular variations, including complete fetal-type posterior cerebral artery variants, may underlie the mechanisms contributing to intractable hiccups. METHODS: Brain magnetic resonance angiography was performed in 230 patients with intractable hiccups who visited our hiccup consultation clinic. Based on the proposed morphological classification system, vascular distribution patterns and frequencies of the Circle of Willis (CW) were compared with those in five previously reported healthy cohorts. RESULTS: Of the 230 patients, 215 (93.5%) had an incomplete CW type, which…

Hiccups with a Twist: A Rare and Surprising Cause Behind Those Involuntary Spasms.

Hiccups is a reflex consisting of a sudden spasmodic contraction of the diaphragm causing shaking of the inspiratory muscles of the chest and abdomen, followed by the sudden closure of the glottis.1 Persistent hiccups, i.e., lasting longer than 48 hours, usually signify an underlying organic cause and must be evaluated. Gastroesophageal disorders such as aerophagia, peptic ulcer disease, esophagitis, gastritis, and pancreatitis are the common causes of persistent hiccups. Distal esophageal spasm (DES) usually presents with dysphagia and chest pain.2 There have been no case reports of DES prese…

A Case of Persistent Hiccups Induced by an Enlarging Hepatic Cyst Underneath the Diaphragm.

Although hiccups are regularly self-limited and rarely life-threatening, prolonged hiccups are bothersome, which can significantly decrease the quality of life. Here, we report a case of persistent hiccups coexisting with an enlarging liver cyst situated just below the diaphragm in a patient with autosomal dominant polycystic kidney disease (ADPKD). No other underlying etiologies related to the symptoms were suspected. The cyst was difficult to drain surgically and the patient continued with symptomatic treatment. Although rare, hepatic cysts should be considered a potential cause of prolonged…

COVID-19 and Intractable Hiccups: An Apparently Harmless Yet Misleading Manifestation.

COVID-19 infection typically presents with various symptoms encompassing fever, cough, and shortness of breath, with occasional reports of less common manifestations such as dizziness and persistent hiccups. Herein, we present a distinctive case wherein refractory hiccups constituted the exclusive complaint, persisting despite standard therapeutic interventions and serving as a misleading sole presentation for COVID-19 infection that, unfortunately, was complicated by septic shock and patient demise. This case prompts contemplation regarding the potential impact of hiccups on the prognosis of …

Stellate Ganglion Block for Intractable Hiccups Secondary to a Motor Vehicle Collision.

Intractable repetitive hiccups are a rare prolongation of the common physiologic reflex arc. If left untreated, chronic hiccups can decrease a patient's quality of life. Many nonpharmacologic, pharmacologic, and interventional treatment modalities have emerged. A 53-year-old male with a past medical history of a motor vehicle collision (MVC) two years earlier presented to a pain clinic with hiccups lasting several months. The patient was experiencing weight loss, lack of sleep, mood changes, and aspiration pneumonia requiring hospitalization secondary to the hiccups. Vagal and respiratory mane…

Olanzapine for cisplatin-induced hiccups: observations from a 338-patient study.

BACKGROUND: Several case reports suggest that olanzapine palliates hiccups. To our knowledge, however, no larger scale studies have confirmed that olanzapine prevents or palliatives hiccups. Hence, the current study sought to substantiate the conclusions from these earlier case reports. METHODS: This multi-site single institution study focused on cisplatin-treated cancer patients because this chemotherapy agent is associated with hiccups and because olanzapine is often used as an antiemetic with this agent. Relevant data were extracted from medical records. Hiccup incidence shortly after chem…

Esophageal Plasmacytoma Revealed by Persistent Hiccups: A Case Report and Literature Review.

INTRODUCTION: Solitary extramedullary plasmacytoma (SEP) is a rare neoplasm that is derived from monoclonal proliferation of plasma cells in the soft tissues or organs arising outside the bone marrow. It is present in about 3% of all plasma cell tumors and originates mainly from the upper respiratory tract and nasopharynx. Involvement of the esophagus is exceptionally seen in cases of SEP. Case Presentation. We report a novel case of a 74-year-old male patient attended with a 6-month history of hiccupping further associated with dysphasia and weight loss all caused by esophageal plasmacytoma. …

Orofacial Dyskinesia and Intractable Hiccups in a Patient with Varicella-zoster Virus Encephalomyelitis.

A 73-year-old Japanese man with diabetic complications presented with involuntary lip movements and long-lasting hiccups after developing zoster rash. Magnetic resonance imaging revealed lesions involving the medial temporal lobe and C1 level of the spinal cord. Varicella-zoster virus (VZV) encephalomyelitis was diagnosed. We considered attributing the orofacial dyskinesia, a very rare symptom of VZV central nervous system (CNS) complications, to the temporal lobe lesion. Although the culprit lesion for the hiccups was unclear, further examinations may have clarified this issue. As immunocompr…

Intractable Hiccups in a Young Male: Is it a Tell-Tale Sign of Pseudocyst of Pancreas?

Although encountered routinely in the outpatient department, hiccups or singultus are mostly neglected by the clinicians owing to its benign presentation and self-limiting nature. However, an innocent-looking symptom such as a hiccup can be a warning sign of serious underlying medical conditions and hence should be addressed seriously. Hiccups can seriously alter the quality of life and induce anxiety when they become intractable. We present an atypical case of a 30-year-old male who presented with intractable hiccups for four months and upon investigations revealed to be a case of chronic pan…

Clinical characteristics of intractable or persistent hiccups and nausea associated with herpes zoster.

AIM: Intractable or persistent hiccups and nausea (IHN) are rarely associated with herpes zoster (HZ-IHN). We aimed to identify the clinical characteristics of HZ-IHN by comparing them with those of neuromyelitis optica spectrum disorder associated with IHN (NMOSD-IHN). METHODS: We collected 8 patients with HZ-IHN and 12 patients with NMOSD-IHN diagnosed between 2002 and 2020 from medical databases. Medical records including clinical information, laboratory data on serum anti-aquaporin 4 (AQP4) antibodies, serological or cerebrospinal fluid findings for the varicella zoster virus, medullary M…

Intractable or persistent hiccups treated with extracranial acupuncture: Two case reports.

RATIONALE: Hiccups are a common clinical symptom, and persistent hiccups and intractable hiccups severely impair the individual's quality of life. To date, there has been no effective treatment specifically for hiccups. Herein, we report 2 cases with intractable or persistent hiccups that were successfully treated with extracranial acupuncture. PATIENT CONCERNS: The first case is a 46-year-old woman who presented with a 7-year history of intractable hiccups that had worsened over the past 3 years. She also complained of chest tightness, dyspnea, palpitations, dreaminess, dysphoria, intoleranc…

Persistent Hiccups as the Only Presenting Symptom of ST Elevation Myocardial Infarction.

Clinical manifestations of acute myocardial infarction can be more than just chest pain. Patients can present with dyspnea, fatigue, heart burn, diaphoresis, syncope, and abdominal pain to name a few. Our patient was a 74-year-old male with a past medical history of type 2 diabetes mellitus, hypertension, hyperlipidemia, and COPD due to chronic tobacco use, who presented with persistent hiccups for 4 days and no other complaints. Coincidently, he was found to have a diabetic foot ulcer with sepsis and acute kidney injury and hence was admitted to the hospital. A routine 12-lead EKG was done, a…
