Periprosthetic femoral fractures associated with contralateral hip disease.

Femoral osteolysis associated with contralateral hip degenerative changes is a risk factor for ipsilateral periprosthetic femoral fracture. We report 5 comminuted proximal shaft fractures around loose femoral implants occurring in patients with both symptomatic femoral lysis and a painful hip on the other side. Our evolving strategy involves timely revision surgery once this pattern is recognized. Once fracture has occurred, emergent revision requires extensive medical evaluation, availability of long-stem revision implants, and appropriate instrumentation and allografts for fracture fixation.

Mortality of patients following hip fractures in Barbados.

A retrospective study of all patients admitted with hip fractures to the Queen Elizabeth Hospital in Barbados from January 1996 to December 1998 was conducted. There were 110 patients who had sustained either a femoral neck or peri-trochanteric fracture. The mean age was 80.7 years and 71.8% of the patients were women. Mortality at six months in 110 patients was 32.7% and 41.2% at 12 months in 102 patients. Forty-nine patients had open reduction and internal fixation of their fractures, forty-five patients were treated with Austin Moore prostheses and one patient had excisional arthroplasty. P…

Osteoporotic pertrochanteric hip fractures: management and current controversies.

The treatment of intertrochanteric hip fractures has evolved over the past 80 years because of a better understanding of fracture anatomy, application of biomechanical principles, and novel technologic advances. Surgical treatment of intertrochanteric hip fractures is the current standard of care, with short-term goals of fracture stabilization and early patient mobilization and the long-term objective of restoring patients to their previous level of independence and function. Treatment for stable intertrochanteric hip fractures includes use of percutaneous intramedullary devices and open redu…

Displaced intracapsular hip fractures: hemiarthroplasty or total arthroplasty?

The role of total hip arthroplasty for the treatment of displaced intracapsular fractures of the proximal femur in active patients is controversial. Some authors have shown that such patients, when treated with a bipolar or unipolar hemiarthroplasty, are at increased risk of having acetabular erosion develop that might require later revision to a total hip replacement. In fact, the results of some authors were not substantially different from those reported for elective total hip arthroplasty and were better than results reported for hemiarthroplasty. However, other authors have strongly recom

Contralateral hip fractures - can predisposing factors be determined?

A case control study was carried out in the Orthopaedic Department of Bradford Royal Infirmary in an attempt to see if certain medical conditions, which can affect balance and stability, are more common in those who sustain a second proximal femoral fracture. Medical conditions included in the study were: late effects of cerebro-vascular accident, blindness, syncope and collapse, alcoholism, Alzheimer's disease, epilepsy, Parkinsonism, ischaemic heart disease and senile dementia. The study group comprised 53 patients admitted to hospital between 1992 and 1998 with two separate proximal femoral

Risk factors for osteoporotic hip fractures in Mexicans.

BACKGROUND: Osteoporosis (OP) and its fractures are a major problem due to their impact in morbidity and mortality in the elderly. Although … er. Controls were healthy subjects who were in hospital waiting rooms accompanying patients without hip fractures. A questionnaire covering known possible risk factors for osteoporotic hip fracture was administered by the same evaluator to all subjects. Pelvic roentgenograms, anterior-posterior view, were obtained in both cases and controls to perform the Singh …

Hip fractures: a worldwide problem today and tomorrow.

Hip fractures lead to excess deaths and substantial disability. Most are related both to falls and to osteoporosis, which affects one in four post-menopausal white women, but a lesser number of men or women of other races. Consequently, about half of the 1.66 million hip fractures worldwide in 1990 occurred in Europe and North America. Even within these regions, however, there is substantial variation in hip fracture incidence rates, which suggests the existence of important environmental factors that could be manipulated to reduce hip fracture occurrence. This is important because in the Unit…

[Malunited juvenile fractures in pelvic and hip area].

Fractures of the hip and pelvis are rare in the growing skeleton. Conversely, they are complicated by morphological and functional abnormalities in a particularly high percentage of cases. We present a review of posttraumatic structural abnormalities and their consequences. In addition to the common presentations, less well-known abnormalities and their possible treatments are discussed.

[Femur shaft fractures in artificial hip joint replacement].

In a total of 1980 patients who underwent total hip replacement 0.8% suffered from intra- and postoperative fractures of the femoral shaft. These spontaneous fractures were preconditioned by damage of the cortical layer due to rheumatoid arthritis, aseptic and septic looseening of the prosthesis as well as local trauma. In most cases stability could be restored by plate osteosynthesis, sometimes in combination with change of the prosthesis.

Traumatic hip dislocation with ipsilateral femoral shaft fractures.

A traumatic dislocation of the hip joint associated with an ipsilateral femoral shaft fracture is a rare injury resulting from severe trauma. Initially the dislocated hip joint is often unrecognized and consequently reduction is delayed. Early diagnosis and reduction of the hip joint improves the functional prognosis. The maximum functional recovery is possible only with greater awareness of this combined injury. Examination of the hip joints by clinical roentgenography in the critically injured patient, especially those with head injuries, or physical signs masked by extremity swelling, shoul

Does early surgery within 48 hours improve clinical outcomes in elderly hip fractures? A matched cohort study of 1776 hip fractures.

PURPOSE: The aim of this study was to determine if a delay to surgery of more than 48 h was associated with poorer functional outcomes and increased 1-year mortality rates for elderly hip fractures. METHODS: A retrospective review of surgically treated elderly (≥ 60 years old) hip fracture patients in a single institution was conducted. Patients were divided into 2 groups depending on hours from admission to surgery: Group 1 ( ≤ 48 h) and Group 2 (> 48 h); these groups were 1:1 matched for the initial Modified Barthel's Index (MBI) and Charlson Comorbidity Index (CCI). RESULTS: 2562 patients…

Fracture Toughness: Bridging the Gap Between Hip Fracture and Fracture Risk Assessment.

PURPOSE OF REVIEW: This review surveys recent literature related to cortical bone fracture mechanics and its application towards understanding bone fragility and hip fractures. RECENT FINDINGS: Current clinical tools for hip fracture risk assessment have been shown to be insensitive in some cases of elevated fracture risk leading to the question of what other factors account for fracture risk. The emergence of cortical bone fracture mechanics has thrown light on other factors at the tissue level that are important to bone fracture resistance and therefore assessment of fracture risk. Recent c…

The hidden fracture: a retrospective study of fragility vertebral fracture in patients with hip fracture.

UNLABELLED: Vertebral fragility fractures (VFFs) are underdiagnosed despite increasing future fracture risk. In 307 hip fragility fracture p … ce of pain, VFFs are linked to increased morbidity and a higher risk of future fractures, including hip fractures. The purpose of this study was to determine the prevalence, location, type, and severity of previous VFFs in patients presenting with hip fragility fractures. Hip osteoarthritis as a predictor of the fracture pattern in proximal femur fractures.

INTRODUCTION: Several authors have suggested a correlation between the fracture patterns of proximal femur fractures and the degree of hip osteoarthritis (HOA), but the current evidence to support this are insufficient. The aim of our study was to demonstrate whether there is an association between the grade of HOA and fracture pattern observed, in patients presenting with a fragility fracture of the proximal femur. MATERIALS AND METHODS: We contacted a retrospective review of all patients presenting to our institution with fragility fractures involving the proximal femur, between March 2012
