The evacuation of two rural hospitals following a tsunami warning: What happened next.

This paper describes the lessons gained and actions taken following the evacuation of two small rural hospitals following a tsunami warning. These evacuations were unique in that the normal protocol to transfer within the health system was not feasible due to their geographical isolation. The after action review process provided valuable insight to all stakeholders, who entered into discussions to ensure procedures, supplies, equipment and arrangements would be put in place to support the safe and effective evacuation of patients in the event of a future emergency. A key component of this proc

From Compliance to Competence: Accelerating Readiness to Change in Hospital CBRNE Preparedness.

Hospital preparedness for CBRNE events remains an Achilles' heel-not because plans do not exist, but because the gap between documented protocols and executable competence is vast. This editorial describes a transformative workshop for a hospital disaster team, operationalizing their Emergency Operations Plan through high-fidelity simulation, iterative practice, and structured hot wash after-action reviews. Anchored in the Transtheoretical Model, our assessment captured not merely knowledge retention but the team's readiness to change-their shift from passive compliance to active ownership. Po

Initial response to the 2024 Noto earthquake by the university hospital closest to the disaster area.

Major earthquakes have occurred frequently in Japan throughout history, and the 2024 Noto earthquake is no exception. However, such natural disasters differ in some respects, and specific problems related to these events have also become clear. Kanazawa Medical University Hospital, which was the closest university hospital to the disaster area of the 2024 Noto earthquake, played a crucial role in serving the local community in the wake of the earthquake. The first method used to identify disaster-related patients is to tag them when they are examined by a physician. After confirming that a pat

Lessons Learned From Helene: The Role of a Rural Community Hospital in Disaster Response After a Major Hurricane.

Hurricane Helene struck Western North Carolina in September 2024, leaving an entire region without power, clean water, or internet connectivity for more than a week. A small, rural, community hospital and its emergency department, which normally sees 84 patients per day, suddenly became the sole source of medical care for an estimated 200,000 residents and experienced a 70% surge in patient volume. Although structural damage to the hospital was limited, the complete loss of digital connectivity rendered electronic medical records, laboratory information systems, radiology, pharmacy, and routin

Emergency management as a complex adaptive system.

This paper explores the application of insights from the study of Complex Adaptive Systems (CAS) to emergency management in search of an adaptive approach to hazard management capable of functioning with increased effectiveness in dynamic, uncertain and unclear environments. This approach demands a shift in perception towards seeing emergency management at the local, tribal, state and national level as a CAS. To function successfully as a CAS, emergency management must move past its current linear hazard-based approach to a hazard-agnostic consequence-based systems approach. This must be predi

Maldistributed health care services: restructuring the current regulatory system.

The American health care delivery system currently suffers from a variety of problems; among the most intractable of these is a maldistribution of health care services. This Note focuses on two aspects of this problem: unnecessary hospital beds, and medically underserved populations. The Note also discusses the related issues of hospital cost inflation and inefficient use of limited resources. It then examines the current statutory remedies for these problems, and subjects their effectiveness to a two-tiered test. The Note concludes that the existing mechanisms, while partially effective, ulti

Creating and Maintaining a "Climate-Smart" Emergency Department: A Scoping Review of Current Progress and Future Potential.

INTRODUCTION: Climate change represents one of the most significant global health threats, with emergency departments (ED) serving as frontline responders to climate-related health emergencies. While EDs are major contributors to healthcare's environmental footprint and critical responders for climate disasters, no comprehensive review has examined sustainability and climate-resilience initiatives specifically implemented in ED settings. METHODS: We conducted a scoping review examining literature on sustainable and climate-resilient measures in EDs. Comprehensive searches of PubMed, Scopus, a

Preparedness planning and response to a mass-casualty incident: A case study of Sunnybrook Health Sciences Centre.

This case study describes a response to a real mass-casualty incident from the perspective of Sunnybrook Health Sciences Centre, a major trauma centre in Toronto, on a day when hospital occupancy levels were at a peak. To help evaluate the lessons learned from the incident response, the article also outlines the hospital's preparedness planning and the readiness assessment it conducted six months prior to the incident. Lessons learned from the readiness assessment are closely linked to those of the real incident, highlighting the importance of readiness assessments and that routine exercises a

Developing a hospital emergency preparedness program.

The emergency preparedness program is not a static entity that only needs to be taken off the shelf and looked at once or twice a year. The various components of the program require that it be a daily part of the hospital's routine. Only by becoming a part of the everyday operation of the hospital can the full benefits of an emergency preparedness program be realized in an actual disaster. A part of the recovery activities after a hospital experiences an actual disaster should be to examine all phases of the emergency preparedness program. The program should be viewed as if it were being desig

Airport emergency management: The art of strict rules and flexible minds.

For businesses large and small, the world over, maintaining operational continuity is an economic imperative. This paper suggests that airports provide a good case study from which to learn, due to their emergency-oriented nature; their nonstop tango with disruptions of many origins; their experience in moving from crisis to normal operations as quickly as possible; and because of their multifaceted functions and stakeholders. With this in mind, the article illustrates how the various challenges faced by airport operators align with those faced by businesses more generally. The paper argues th

Bioterrorism preparedness.

The Connecticut Department of Public Health (DPH) entered into a cooperative agreement with the Centers for Disease Control and Prevention (CDC) to establish public health preparedness and a response plan for bioterrorism. With funds from the CDC and an additional grant from the Health Resources and Services Administration (HRSA), the DPH designated Hartford Hospital as one of two Centers of Excellence that will coordinate and manage a statewide system for bioterrorism preparedness. This paper reviews the progress that Hartford Hospital has made in meeting this challenge. Highlighted are the d

Developing an information systems strategy for nursing.

With the rapidly changing health care environment and information technology advances, organizations need to engage in strategic, planned change in order to allocate limited resources, achieve the organization's goals, and fulfill its mission [1]. One of the most important aspects of the organization's planned strategies for change concerns the information systems. The involvement of the nursing department in this process is critical. This poster presentation will communicate how nurses can develop an information systems strategic plan that will enable them to play an active role as contributo

ED swings into action following helicopter crash. Cooperation with fire department was essential.

The crash landing of an Aero Med helicopter on the roof of a Grand Rapids, MI, hospital required the ED team to spring into action. Here are some lessons you can draw from their experience: You or your staff might need to assist fire or EMS personnel in transporting victims to the ED or evacuating the hospital. Even if your ED is not overwhelmed with patients, it might be necessary to go on lockdown to keep onlookers and other unwanted visitors out. Be prepared to discharge stable patients to make room for others with more urgent needs.
