Seven patients who sustained a displaced fracture of the capitulum humeri had been treated by surgery, using a volar approach and internal fixation with PDS pins. The advantage of this method is that by using the volar approach, the displaced capitulum could be reduced properly in its exact anatomical position. The internal fixation with PDS pins was solid enough to allow early postoperative movements. A second operation to remove any material was not necessary. In all cases the clinical and x-ray results have been excellent.

Thirty shoulders, in 20 patients, which had undergone core decompression for symptomatic avascular necrosis of the humeral head were reviewed 2 to 14 years later (average 5.6). Twenty-two showed good or excellent clinical results; the other eight shoulders had required arthroplasty. All 14 shoulders with stage I or II radiological changes (Ficat and Arlet 1980) at operation had good or excellent results. We advocate early core decompression for symptomatic avascular necrosis of the humeral head.

BACKGROUND: Understanding vascularity and assessing the risk of post-traumatic avascular necrosis are crucial for predicting outcomes and identifying optimal treatment options in proximal humerus fractures (PHFs). Until now, Hertel et al have been the only researchers to evaluate the intraoperative perfusion of the humeral head after fracture using Doppler flowmetry in a central single drill hole within the head. This pilot study aims to standardize the evaluation of intraoperative perfusion measurements in different areas of the humeral head in patients with PHF.
METHODS: In this prospective…

Sickle cell disease (SCD) affects millions of people worldwide. It is an autosomal recessive hemoglobinopathy that occurs due to a point mutation in the sixth codon that replaces glutamic acid with valine in the beta-globin chain. Avascular necrosis (AVN), also known as osteonecrosis, is one of its complications. In this report, we present a case of a 25-year-old female with sickle cell trait without any comorbidities who presented to us with pain in both shoulder joints for three months and was diagnosed with AVN in bilateral shoulder joints. Appropriate treatment can dramatically reduce pain

The objective was to analyze three-dimensional scapular positioning and glenohumeral centering of normal and atraumatic unstable shoulders. We hypothesized that changes of humeral head position correlate with alterations of scapular positioning. The shoulders of 28 healthy volunteers and 14 patients with atraumatic instability were examined in various arm positions using open magnetic resonance imaging. After segmentation and three-dimensional reconstruction, three-dimensional analyses of scapular positioning and humeral head position relative to the glenoid were done. The coefficient of corre…

INTRODUCTION: Short-term follow-up of angular stable fixation for proximal humeral fractures has been well documented in the literature. Longer follow-up series are difficult to find. However, especially regarding the risk of avascular humeral head necrosis longer follow-up series are high of clinical relevance.
METHODS: Forty-eight patients with a mean age of 66 years and treated with open reduction and angular stable internal fixation for proximal humeral fractures were followed up for a mean of 45 months. The clinical and radiographic follow-up (Constant Score (CS), age and gender related …

BACKGROUND: The use of uncemented humeral stems in total shoulder arthroplasty (TSA) is known to be associated with stress shielding. This may be decreased with smaller stems that are well-aligned and do not fill the intramedullary canal; however, the effect of humeral head positioning and incomplete head backside contact has not yet been investigated. The purpose of this study was to quantify the effect of changes in humeral head position and incomplete head backside contact on bone stresses and expected bone response following reconstruction.
METHODS: Three-dimensional finite element models…

PROBLEM: The vascularity of the articular fragment is of key importance for the final outcome in three- and four-part fractures of the humeral head. Displacement of the articular segment may compromise the arterial blood supply to the humeral head and result in avascular necrosis. There is still controversy as to whether three-and four-part fracture dislocations (articular fragment outside the glenoid) have an even worse prognosis than displaced three- and four-part fractures.
PATIENTS AND METHODS: Between January 1985 and May 1993, 102 patients with three- and four-part fractures of the hume…

PURPOSE: To evaluate the effect of loading the long and short heads of the biceps on glenohumeral range of motion and humeral head position.
METHODS: Eight cadaveric shoulders were tested in 60° abduction in the scapula and coronal plane. Muscle loading was applied based on cross-sectional area ratios. The short and long head of the biceps were loaded individually followed by combined loading. Range of motion was measured with 2.2 Nm torque, and the humeral head apex position was measured using a MicroScribe. A paired t test with Bonferroni correction was used for statistics.
RESULTS: Long h…

BACKGROUND: Reverse Hill-Sachs lesions (RHSLs) are compression fractures of the anteromedial humeral head that occur with posterior shoulder dislocations, often contributing to recurrent instability and failure of isolated posterior labral repair. Surgical intervention is indicated for large, engaging lesions to restore bone density and prevent recurrent dislocation.
INDICATIONS: Lemon wedge humeral head allograft reconstruction is a novel technique used for instances of recurrent posterior shoulder instability resulting in a large RHSL.
TECHNIQUE DESCRIPTION: Arthroscopic evaluation is used…

BACKGROUND: Avascular necrosis (AVN) of the humeral head can progress to joint collapse and marked functional limitation, often requiring surgical treatment. Total shoulder arthroplasty (TSA) and hemiarthroplasty (HA) are the main surgical treatment options, but their comparative clinical value remains a topic of debate. In particular, uncertainty persists regarding how functional benefit, complication risk, and implant durability should be balanced in contemporary practice. In this meta-analysis, we compared functional outcomes, complication rates, and revision risk between TSA and HA in pati…

Management of humeral head fractures presents numerous challenges, particularly in cases of separation fractures, which are relatively rare and often caused by high-energy trauma. Due to the complexity of these fractures and the absence of a standardized treatment algorithm, surgical intervention can be technically intricate and fraught with postoperative complications. In young patients, the decision to pursue open reduction and internal fixation must be meticulously made to achieve optimal outcomes while minimizing long-term sequelae. We present a rare case of a man, age 25 years, sustaining…

BACKGROUND: Large osteochondral lesions of the humeral head can result from locked posterior dislocations, avascular necrosis, and osteochondritis dissecans. Fresh osteochondral allograft (OCA) transplantation is a treatment option for young patients with focal osteochondral defects of the humeral head. The purpose of this case series was to assess graft survivorship, subjective patient-reported outcomes, and satisfaction among 7 patients who underwent OCA transplantation of the humeral head.
METHODS: We identified 7 patients who underwent humeral head OCA transplantation between 2008 and 201…
