Widespread antenatal screening has resulted in increased detection of anomalies of the kidneys and urinary tract. The present guidelines update the recommendations published in 2000. Antenatal hydronephrosis (ANH) is transient and resolves by the third trimester in almost one-half cases. We studied the correlations between diuretic renographs and kidney function in experimental hydronephrosis in rabbits. Features of furosemide-stimulated 99mTc-diethylenetriamine-pentaacetic acid renographs were compared to the growth rate, thirst test and endogenous creatinine clearance rate in a chronic solitary-kidney animal model. In the present study, the frequency and the mechanism of hypertension associated with unilateral hydronephrosis (UHY) were investigated in 115 patients. Hypertension (blood pressure greater than 140/90 mm Hg) was found in 20% of 101 consecutive patients with UHY. Twenty-six patients with UHY and hypertension were followed for 35 months and the effect of surgery on blood pressure was analyzed. Partial obstruction of one ureter was created in newborn rats and its effects were studied in the adult rat. The obstructed renal pelvis was found to be enlarged 2-3 times. The weight of the kidney on this side was 85% of the contralateral one. Thirty-one fetuses with hydronephrosis and 14 fetuses with spinal dysraphism were sonographically examined. In only one instance was spinal dysraphism associated with hydronephrosis. This single case was identified in a term pregnancy. There was no statistically significant association of urinary tract dilation and open spinal defects in utero. This observation tends to confirm the reports that urinary tract dilatation associated with spinal dysraphism is less frequent when neonates and children are compared. the progressive decrease in this association appears to extend to the fetal period.

Fourteen children are described in whom primary ureterocalicostomy was performed for severe hydronephrosis due to pyeloureteral obstruction. Seven of them presented a horseshoe kidney. After a follow-up varying from 1 to 6 1/2 yr, clinical and radiologic improvement was obtained in 12 patients. Exclusion of decompensated renal pelvis and formation of strictly declivous drainage system, especially benefiaial in the case of horseshoe kidney, are two advantages of this procedure. There has been considerable interest in and controversy over the cause and management of this entity.
