Study of the genetics of the malignant hyperthermia syndrome began in families in which both malignant hyperthermia (MH) episodes had been experienced and individuals had strongly positive contracture tests diagnostic of susceptibility to MH. Linkage studies associated this MH phenotype to the ryanodine receptor gene (RYR1) at chromosome 19q13.1 in many families. Although the MH phenotype is not always linked to chromosome 19, the RYR1 has remained the focus of experimentation. Other candidate genes exist, but few MH-susceptible families have variants of these genes. Hundreds of MH-susceptible…

The genetic evaluation of the ryanodine type one receptor (RYR1) gene is unlikely to be a useful screening test of malignant hyperthermia susceptibility. But when significant suspicion of malignant hyperthermia has been raised by well-documented clinical events or strong family history, the genetic evaluation of RYR1 could secure a diagnosis and indicate appropriate treatment for both the index patient and many relatives of all ages, including the youngest.

The results of hyperthermia in the malignant tumors were reviewed. It is well known that hyperthermia enhances the effects of radiotherapy and chemotherapy for cancer patients. The clinical study on hyperthermia has been performed in the world since 1975. Several randomized clinical study revealed the benefits of hyperthermia in the head and neck cancers, recurrent breast cancers, and uterine cervical cancers. However, there are many difficulties to perform the hyperthermia in Japan because of lack of man-power and financial aid. Further clinical trials were recommended to obtain the evidences…

The opinion from most of the current data is that malignant tumors may be more heat sensitive than normal tissues. Hyperthermia has been tested in both randomized and non-randomized trials in gynecologic cancers, but has not found a place currently, as there is scarce data. As a consequence, adding this tool into the conventional methods of cervical cancer and ovarian cancer treatment has not been supported. Hyperthermia modifies the effects of not only ionizing radiation but also a number of chemotherapeutic agents. The use of hyperthermia in cervical cancer is of interest to the radiotherapi…

Malignant hyperthermia (MH) is a hypermetabolic syndrome triggered in genetically susceptible individuals when certain potent inhalation anesthetics or succinylcholine are administered. The signs of MH include a greatly increased body metabolism, muscle rigidity and eventual hyperthermia that may exceed 110 degrees F. Death can result from cardiac arrest, brain damage, internal hemorrhaging or failure of other body systems. Dantrolene is given until the manifestations of MH abate. The initial dose is 2.5 mg/kg body weight; and repeated as needed. High-risk patients cannot always be identified …

Hemorrhage in the midbrain and/or pons in patients is often associated with increased metabolism, resulting in hyperthermia. We have recently reported that hyperthermia develops in anesthetized rats following prepontine knife-cuts or procaine microinjections into the midbrain or upper pontine region. It was concluded that the hyperthermia in the animals was caused by the removal of a tonic inhibitor mechanism of heat production that exists in the lower midbrain. The present paper proposes a new hypothesis that the hyperthermia in patients with brainstem hemorrhage is caused by disinhibition of…

Psychostimulant drugs such as amphetamines, amphetamine derivatives, and cocaine produce a variety of potentially lethal effects, and an understanding of these toxic effects is important for emergency physicians. While some effects of psychostimulant poisonings such as cardiovascular compromise and seizures have been discussed extensively, other metabolic derangements such as hyperthermia are less well characterized. In fact, hyperthermia is a common feature of severe-to-lethal poisonings and may be the primary mode of demise in some patients. Animal studies confirm that drug-induced hyperther…

Malignant hyperthermia (MH) is an anesthetic agent-induced hypermetabolic state. Human beings and several other animal species, including dogs, have been described to be genetically predisposed to development of MH. The halothane-triggered MH syndrome was characterized in genetically predisposed dogs, and in vitro contracture sensitivity of biopsied gracilis muscle exposed to halothane and caffeine was quantitated. Within 1 hour of halothane administration, each MH-susceptible dog developed rapid increases in CO2 production and rectal temperature. Reversal of the hypermetabolic state was achie…

Malignant hyperthermia (MH) is a pharmacogenetic disease in man and animals. It primarily involves skeletal muscle tissue, but other tissues might be affected to a lesser degree. Calcium homeostasis in muscle cells is upset in susceptible individuals, so that various agents and circumstances can increase the free, ionised intracellular calcium concentration to damaging levels. The primary defect is not known at present, but is believed to involve an abnormally sensitive calcium-induced calcium release mechanism. Thus small, localised increases in calcium concentration releases more calcium so …

We have described two infants, aged 2 months and 5 months, who had malignant hyperthermia, an entity not generally believed to occur during infancy. Both children were under general anesthesia when MH occurred, and both responded well to administration of dantrolene and sodium bicarbonate.

This paper mainly reports our clinical findings so far obtained from thermotherapy performed as part of a multimodality treatment for gyneco … emotherapy was found to have a more favorable chemotherapeutic effect and prognosis. RF radial-type hyperthermia with radiotherapy was proved to be more effective for reduction of tumor size and histopathological effect than radiotherapy alone.
