Incisional hernia after laparoscopic colorectal surgery. Is there any factor associated?

BACKGROUND: Laparoscopic approach is related to, among others, educing abdominal wall complications such as incisional hernia (IH). However, there are scarce data concerning laparoscopic colorectal surgery (LCRS). The aim of this study was to evaluate related factors and incidence of IH following this approach. METHODS: A retrospective analysis of consecutive patients who underwent colorectal surgery with laparoscopic approach in a single center was performed. Patients with a minimum follow-up of 6 months, and also converted to open surgery were included. Uni- and multi-variate analyses were …

Risk of late-onset adhesions and incisional hernia repairs after surgery.

BACKGROUND: Long-term adhesion-related complications and incisional hernias after abdominal surgery are common and costly. There are few data on the risk of these complications after different abdominal operations. STUDY DESIGN: We identified Medicare beneficiaries who underwent endovascular repair of an abdominal aortic aneurysm from 2001-2008 who presumably are not at risk for laparotomy-related complications. We identified all laparoscopic and open operations involving the abdomen, pelvis, or retroperitoneum and categorized them into 5 groups according to invasiveness. We then identified l…

Open randomized clinical trial of laparoscopic versus open incisional hernia repair.

BACKGROUND: Incisional hernia is a common complication following abdominal surgery. Although the use of prosthetics has decreased recurrence rates, the standard open approach is still unsatisfactory. Laparoscopic techniques are an attempt to provide similar outcomes with the advantages of minimally invasive surgery. METHODS: Open randomized controlled clinical trial with follow-up at 1, 2, 3, 7, and 15 days, and 1, 3, and 12 months from hernia repair. The study was carried out in the surgery departments of three general hospitals of the Valencia Health Agency. OBJECTIVES: To compare laparosc…

Incisional hernia after repair of wound dehiscence: incidence and risk factors.

The true incidence of incisional hernia after wound dehiscence repair remains unclear because thorough long-term follow-up studies are not available. Medical records of all patients who had undergone wound dehiscence repair between January 1985 and January 1999 at the Erasmus University Medical Center Rotterdam were reviewed. Long-term follow-up was performed by physical examination of all patients in February 2001. One hundred sixty-eight patients underwent wound dehiscence repair. Of those, 42 patients (25%) died within 60 days after surgery. During a median follow-up of 37 months (range, 3-…

Effect of Monofilament Sutures on Incisional Hernia After Laparoscopic Colorectal Surgery: A Focus on Patients With Umbilical Hernia.

BACKGROUND/AIM: Incisional hernia (IH) is a common complication of laparoscopic colorectal surgery and is associated with risk factors such as obesity, diabetes, and smoking. Umbilical hernia is recognized as a strong predictor of IH; however, the effect of suture type on the occurrence of IH remains unclear. We aimed to evaluate whether the use of monofilament sutures reduced the incidence of IH, particularly in patients with umbilical hernias. PATIENTS AND METHODS: This retrospective cohort study included 223 patients who underwent laparoscopic colorectal surgery between January 2013 and De…

Incisional hernia repair in cirrhotic patients with ascites: a multicenter retrospective experience.

OBJECTIVES: Incisional hernia is a complication after abdominal surgery and is particularly challenging in patients with ascites because of increased intra-abdominal pressure, impaired wound healing, and infection risk. Evidence on surgical repair remains limited, particularly for incisional rather than umbilical hernias. METHODS: This retrospective multicenter study included 31 cirrhotic patients with ascites (22 males, 9 females; 45-72 years) who underwent incisional hernia repair between 2000 and 2025 at three tertiary General and Emergency Surgery Units. Liver disease severity was assesse…

Acute bilateral external iliac artery occlusion following a laparoscopic incisional hernia repair.

Acute iliac artery thrombosis is a rare complication of laparoscopic surgery that represents a significant risk to limb and life. This case describes an uncommon instance of acute-on-chronic external iliac artery thrombosis presenting immediately after laparoscopic gastrointestinal surgery. While rare, this presentation highlights an important perioperative complication in patients with underlying vascular disease.A man in his early 60s underwent elective laparoscopic ventral hernia repair. Postoperatively, he woke with severe left calf pain. Both lower limbs were pale, cool and pulseless. CT

A transcriptomic analysis of incisional hernia based on high-throughput sequencing technology.

PURPOSE: Incisional hernia is a common postoperative complication; however, few transcriptomic studies have been conducted on it. In this study, we used second-generation high-throughput sequencing to explore the pathogenesis and potential therapeutic targets of incisional hernias. METHODS: Superficial fasciae were collected from 15 patients without hernia and 21 patients with an incisional hernia. High-throughput sequencing of the fascia was performed to generate an expression matrix. We analyzed the matrix to identify differentially expressed genes (DEGs) and performed gene ontology and enr…

Open transversus abdominis release in incisional hernia repair: technical limits and solutions.

INTRODUCTION: Incisional hernias with a defect width of more than10 cm are considered complex. The European Hernia Society guidelines recommend that such hernias should only be repaired by surgeons with experience of component separation. The standard component separation technique now is posterior component separation with transversus abdominis release (PCSTAR). Questions are raised about the limits of this technique. METHODS: A literature search of publications on PCSTAR was performed for any references to the limits of this technique in open incisional hernia repair. We found 26 publicatio…

Recurrent incisional hernia repair: surgical outcomes in correlation with body-mass index.

BACKGROUND: Hernia recurrence rates after incisional hernia repair vary between 8.7 and 32%, depending on multiple factors such as patient characteristics, the use of meshes, surgical technique and the degree of experience of the treating surgeon. Recurrent hernias are considered complex wall hernias, and 20% of all incisional hernia repairs involve a recurrent hernia. The aim of this study was to investigate the outcomes after recurrent incisional hernia repair, in association with surgical technique and body-mass index (BMI). METHODS: All patients who had incisional hernia repair between 20…

Incisional hernia after surgery for colorectal cancer: a population-based register study.

BACKGROUND: Our knowledge on the incidence of incisional hernia and risk factors for developing incisional hernia following surgery for colorectal cancer is far from complete. METHODS: All procedures registered in the Swedish Colorectal Cancer Register (SCRCR) 2007-2013 were identified. Patients with comorbid disease diagnoses, registered at admissions and visits prior to the procedure and relevant to this study, were obtained from the National Patient Register (NPR). These diagnoses included cardiovascular disease, connective tissue disorders, liver cirrhosis, renal failure, diabetes, chroni…

Impact of incisional hernia development following abdominal operations on total healthcare cost.

BACKGROUND: Introduction of the category III CPT code (0437T) for prophylactic mesh augmentation (PMA) highlights efforts to reduce incisional hernia (IH). PMA's value in the context of value-based care requires understanding both the cost of IH development and the savings from prevention. We hypothesized large healthcare costs with IH development. Appreciating which subsets of patients are at highest risk for IH, and the subsets who have the costliest care is essential in targeting interventions for hernia prevention. METHODS: Retrospective cohort study utilizing data from Truven Health Anal…

Incisional Hernia After Peritoneal Dialysis Catheter Placement in a Patient on Sirolimus.

Hernias and peritoneal dialysis (PD) catheter leaks are frequent complications in patients on PD. Transplant recipients have multiple risk factors for delayed wound healing, such as use of corticosteroids and sirolimus, and the presence of uremia and diabetes mellitus. We report a rare occurrence of incisional hernia attributable to internal wound dehiscence after PD catheter placement in a patient on sirolimus.A 34-year-old Latino American man was started on PD training 4 weeks after placement of a PD catheter. Soon after completing training, he developed a large soft bulge close to the PD ca…

Does Abdominoplasty Add Morbidity to Incisional Hernia Repair? A Randomized Controlled Trial.

Background Abdominoplasty is considered an operation linked to a considerable rate of morbidity. The convenience of simultaneously performing an incisional hernia repair and an abdominoplasty remains controversial. Methods A total of 111 patients were randomized prospectively to compare isolated incisional hernia repair and hernia repair when combined with abdominoplasty. Primary end points were in-hospital stay and early morbidity. Secondary end points were late morbidity, recurrences, and quality of life. Patients were followed-up for 24 months. Results Duration of the surgical procedure dif…

A modified open intraperitoneal mesh (Garestin) technique for incisional ventral hernia repair.

BACKGROUND/OBJECTIVE: Incisional hernias (IHs) are a major problem following abdominal surgery. In an effort to resolve large IHs adequately, we herein present our own modified "open intraperitoneal mesh" technique, termed the Garestin technique. METHODS: We analyzed early postoperative complications (EPCs; wound infection, hematoma, and seroma) and late postoperative complications (recurrence) in 124 patients operated for IHs and recurrent IHs (RIHs) using our new technique. Our technique involved repairing hernias by preserving the hernia sac, which was later used to conceal the mesh that r…

Lower recurrence rate with full-thickness mesh fixation in incisional hernia repair.

PURPOSE: To share the lower recurrence rate achieved during long-term follow-up by repairing incisional hernias (IHs) with full-thickness fixation of onlay mesh. METHODS: We retrospectively analyzed 196 IH cases operated on by the same surgeon between 2002 and 2013. After exclusions (unrelated death, lack of follow-up), 154 cases were included. Abdominal examination findings, recurrence dates (if accessible), and imaging results were obtained from computer records and evaluated. Intraoperatively, all hernial sac adhesions were separated to reveal the anterior abdominal wall, and full-thicknes…

Introduction of 2-octyl cyanoacrylate (Dermabond®) for incisional hernia mesh repair.

PURPOSE: We developed a new technique, sutureless mesh fixation, using 2-octyl cyanoacrylate (Dermabond®, a surgical tissue adhesive) for incisional hernia repair. The objective of this article is to introduce the new technique and to examine whether the technique provides sufficient resistance to abdominal pressure. METHODS: We conducted two tests using a porcine model, a traction experiment and artificial pneumoperitoneum test. In the traction experiment, the adherence properties of Dermabond® with mesh and peritoneum were examined using a tissue fragment from a pig. In the artificial pneum…
